For many non-profit organizations, this past year has been challenging as the economy remains weak and the demand for our ser-vices continues to rise. Asian Health & Service Center (AHSC) is mission driven and our strategies are to integrate our services, to collaborate with partners and volunteers, and to strive to achieve the best for our community. We simply emphasize on what we can do, and we do it well.

Ethnic Senior Outreach & Meals Services: AHSC has operated our Asian ethnic lunch program since 2009 which draws isolated seniors out of their homes and provides them with nu-tritious Asian lunches, as well as opportunities to socialize with oth-ers, get involved in other ac-tivities, and learn more about community resources. We be-gan by serving 90 meals per week; however, the response was overwhelming. This year, we served 14,581 meals, av-eraging 280 meals per week. We thank the supporters of this program, Multnomah County Aging & Disability Ser-vices, which provides funding, and our partners from neighborhood ethnic restaurants! Additionally, we could not have accomplished this without the donation of 820 pounds of rice, 5,500 plates, 5,000 cups, and 35,000 napkins from an anonymous wholesale distributor.

Community Engagement: Integrated & Holistic Service Model: In 2010, AHSC broke our record for annual services by pro-viding 43,070 unique service encounters. With strong connection to the Asian community, we currently have 6,047 registered clients. We welcome everyone who seeks services from AHSC through an array of culturally sensitive and linguistically appropriate services that address clients’ health, wellness, and social needs holistically. Currently, we have 12 groups meet weekly in Portland, Beaverton office and a senior complex in down town to engage and integrate services. This is made possible by AHSC’s committed team of 40 community health workers

and mental health professionals and our partnerships with the State and Multnomah and Washington counties that help us tailor pro-grams to best serve the community. Thanks to the facility support from City of Beaverton, we are able to deliver similar services in both of our Portland and Beaverton offices.

Healthy Kids: Since launching this program in 2010, our staff actively reaches out and engages families to promote the Healthy Kids Program, which expands access to health insurance for children. AHSC has successfully assisted over 400 families to enroll in the Healthy Kids Program. We also help families navigate the system to find a medical home. This April, AHSC was presented the “Cultural Leadership Award” by the Office of Healthy Kids of Oregon Health Authority.

Asian Wellness Connection: This United Way-funded project has deepened AHSC’s connection and partnership with com-munity health clinics in the last three years. The program provides information and referral, case-management services, Living Well with Chronic Diseases pro-grams, and seamless referral to our mental health program. In addition, we have established Patient Advisory Committees that meet quarterly to provide input on their experience and expectation on healthcare services. Currently, we are in the process of finalizing the bilingual Patient Handbook Book that covers Chinese, Korean and Vietnamese, which will facilitate communication between patients and health care pro-viders who don’t speak the same language.

Community Chinese Medicine and Naturopathic Clinic: AHSC’s model integrates our mental health services with health and wellness services. In September, we deepened our partnership with National College of Natural Medicine (NCNM) by expanding the days and scope of the low cost Chinese Medicine & Naturopathic Medi-cine clinic offered onsite to three times a week at AHSC.

Helping you, Helping Me: This Northwest Health Foundation-funded project is a pioneering homecare worker training and re-ferral program. This program engages and trains Asian community members who have limited English proficiency to become homec-are workers. This program will have three-fold positive impact in our community: 1) on the isolated seniors who need culturally sensitive and linguistically appropriate homecare, 2) on the family members of the seniors, and 3) on community members who will now have another path to employment.

Yu Miao Chinese Immer-sion Preschool: The Preschool has a vision of developing great leaders for the future. The Chi-nese Immersion Preschool pro-vides children and families oppor-tunity to learn Mandarin Chinese and Chinese culture in a nurturing and stimulating environment. We

believe our preschoolers will be better equipped to meet the oppor-tunities of the global world. As of our 11th graduation this June, 85% of our graduates continue learning Mandarin Chinese at Woodstock Elementary School.

如同其他的非營利組織,由於經濟不景氣加上服務需求提升,過去的這一年充滿了挑戰。華人服務中心(AHSC)依然秉持著我們的宗旨,專注於我們的願景以提供更好的社區服務。 華人服務中心的策略是整合我們的服務並與社區夥伴和義工共同合作以提供最佳的服務。我們重視社區的需求,並且致力於解決各種需求。

長者免費午餐服務:感謝麥諾瑪縣老人與殘障人士服務部的資金支持和鄰近的餐館合作!華人服務中心於2009年開始提供長者午餐供應,旨在鼓勵獨居長者參加並從而獲得社區資源和參與社 區 活 動 。 我 們 起 初 的計 劃 是 每 週 提 供 9 0 份 午餐。然而,由於此服務受到熱烈的支持,我們在過去的這一年內總共供應了14,581份午餐,平均每週280份。特別感謝一位匿名的批發分銷商捐贈了820磅米,5,500個盤子,5,000個杯子,和35,000份餐巾紙使得此服務得以持續進行。

社區中醫和自然醫學療法診所:我們的服務模式旨在整合公共衛生和精神健康服務。今年9月開始,我們與國家自然療法醫學院(NCNM)合作,推廣低收費的中醫診所和自然醫學療法診所。每週提供三天門診。

助人助己,家居照護計劃:獲得西北健康基金會(Northwest Health Foundation)的贊助,中心推出為期三年的家居照護員培訓和就業機會的領航計劃。助人助己計劃針對中年亞裔人士其英語能力有限,培訓其成為州政府認證的家居照護員,並轉介安排就業機會給需要家庭照護的亞裔家庭和長者。此計劃有三重影響:1)獨居老年人,2)老人的家庭成員,以及3)潛在的勞動力。

育苗學前雙語融入學校:學校的願景是培育學員成為未來的領袖,奠定美好未來的基礎。學前雙語融入學校提供一個兒童及其家庭學習中文和了解中華文化的環境。我們相信此培育能讓學員們在全球化的趨勢下具備跨國文化的優勢。今年六月學校舉辦了第十一屆畢業典禮,其中有百分之八十五的畢業生在伍德斯托克(Woodstock) 小學繼續學習中文。

수요는계속늘어나서저희에게도어려운해였습니다.아시안

보건복지센터는임무를다하고자하며,또지역사회를위해서

더좋은서비스를하고자우리의이상과목표에촛점을맞추고

있습니다.저희센터의전략은파트너와자원봉사자들과의협

력을통해서비스를통합하여최선의것을성취하고자하는것

입니다.저희는저희가할수있는것들을강조하며또그것을잘

하고있습니다.

아시안노인지원활동과무료식사서비스:멀티노마카

을찾는것을돕고있습니다.지난4월에저희센터는헬씨키즈

본부로부터“문화지도자상”을받았습니다.

마련된프로젝트로,자택요양간병인을훈련시키고소개하는3

년짜리시범프로그램입니다.이프로그램은자택요양간병인

이되기에영어에한계가있는중년의아시안을훈련하여자택

요양간병인이되도록하는프로그램입니다.이프로그램은세

부분에영향을미칩니다.1)소외된노인들2)소외된노인들의

가족들3)잠재적인력

유미아오중국유치원:이유치원은미래에훌륭한지도자를만

든다는이상을계속유지하고있습니다.이유치원은어린이들

과그가족들에게교육적이며활기찬환경을통해서만다린어

와중국문화를배울기회를제공합니다.저희는유치원생들이

지구촌시대의새로운요구에부

응하기위해서스스로를보다잘

준비할수있다고믿습니다.지난

Cũng giống như bao nhiêu tổ chức từ thiện khác, năm nay là một năm đầy khó khăn trong khi nền suy thoái kinh tế văn hóa đòi hỏi dịch vụ tăng cao. Nhiệm vụ của Trung Tâm Y Tế và Dịch Vụ Châu Á là cần tập trung để nâng cao sự giúp đỡ cho cộng đồng. Những chiến lược của Trung Tâm ASHC là liên kết mọi dịch vụ, cộng tác với nhiều thành viên cùng những tình nguyện viên, và làm việc một cách tốt nhất. Chúng tôi thật sự chú tâm vào những việc chúng tôi có thể làm, và chúng tôi sẽ hoàn thành những việc ấy trong cách tốt nhất.

Chương trình Dân Tộc Người Già và Bữa Ăn Dịch Vụ : Cám ơn sự hỗ trợ tài chánh từ dịch vụ cho người tàn tật và người lớn tuổi của hạt Multnomah, và những công tác hỗ trợ từ những nhà hàng dân tộc ! Chúng tôi khởi đầu Bữa Ăn Trưa Truyền Thống từ năm 2009 nhằm vào mục tiêu để mời những người nghèo đói lớn tuổi đến dùng bữa, để giúp đỡ họ gặp gỡ nhau, và giúp họ làm bạn với nhau. Với sự bắt đầu, chúng tôi cho rằng có thể cố gắng phục vụ khoảng 90 bữa ăn mỗi tuần; tuy nhiên, mọi người đến rất đông, hơn sự dự định của chúng tôi. Trong năm vừa qua, Trung Tâm đã phục vụ 14,581 bữa ăn, trung bình là 280 bữa ăn mỗi tuần. Chúng tôi giúp đỡ thành công hơn 400 gia đình ghi danh vào chương trình này. Chúng tôi cũng giúp họ thông qua những hệ thống để tìm bệnh viện. Trung Tâm đã lãnh được bằng khen về ‘Tinh Thần Lãnh Đạo Dân Tộc” của Văn Phòng Thiếu Nhi Mạnh Khỏe.Chương Trình Người Á Châu Khỏe Mạnh Liên Hợp : đây là chương trình được sự ủng hộ tài chánh từ United Way cho chúng ta kết nối với những trung tâm y tế cộng đồng trong 3 năm qua. Nhờ có những dịch vụ bao trọn gói, sống khỏe mạnh với những căn bệnh già, bệnh tinh thần, chúng tôi đã thành lập được Hội Đồng Khuyến Ngân và hoàn tất Sách Giúp Đỡ Người Bệnh với 2 ngôn ngữ khác nhau.

Thuốc Trung Hoa và Y Tế Dân Tộc Cộng Đồng : Chúng tôi đề cử mô hình liên kết giữa sức khỏe cộng đồng và dịch vụ sức khỏe tinh thần. Trong tháng 9, chúng tôi đã mở rộng hội thành viên với Trường Cao Đẳng Thuốc Dân Tộc (NCNM) và mở rộng thêm phòng khám hữu nghị cho thuốc Trung Hoa và thuốc dân tộc.

Giúp Bạn, Giúp Tôi : được hỗ trợ tài chánh từ cơ sở Sức Khỏe Tây Bắc, đây là chương trình giúp huấn luyện người quản gia và giới thiệu người làm việc. Chương trình này giúp và huấn luyện những người Á Châu Trung Niên không biết nhiều tiếng Anh có thể tìm được việc làm quản gia. Với chương trình này có ba điều lợi ích : 1) giúp người già đơn chiếc; 2) giúp cho những thành viên trong gia đình của họ; 3) giúp tăng cao việc làm cho người dân.

Trường Mẫu Giáo Trung Hoa Yu Miao : Trường Mẫu Giáo vẫn tiếp tục phát triển những người lãnh đạo giỏi cho tương lai. Trường Mẫu Giáo Trung Hoa cung cấp cơ hội cho mọi trẻ em và gia đình để học tiếng Hoa Phổ Thông, và nền văn hóa Trung Hoa thông qua những môi trường giáo dục khuyến khích. Chúng tôi tin tưởng rằng học trò mẫu giáo của chúng tôi sẽ chuẩn bị cho tương lai tốt hơn trên toàn thế giới. Trong kỳ ra trường lần thứ 11 vào tháng 6 vừa qua, 85% học trò sẽ tiếp tục học tiếng Hoa Phổ Thông tại trường tiểu học Woodstock.

Since 1993, the Office of Multicultural Health and Services has conducted a variety of activities focused on addressing health disparities in Oregon. Throughout its history, OMHS has provided consultation to pro-grams within the Department of Human Services and the Oregon Health Authority, local health depart-ments, higher education programs, faith- and community-based organizations, universities, ethnic media outlets, Area Health Education Centers (AHECs), health and community advocacy organizations, and others working to improve the health of all Oregonians.Through policy development, training and consultation, and community engagement and capacity-building, OMHS continues to promote equity in all aspects of the Oregon Health Authority and with Oregon’s focus on Health Systems Transformation to achieve OHA’s Triple Aim to improve the lifelong health of Oregonians, in-crease the quality, reliability, and availability of care for all Oregonians, and lower or contain the cost of care.

SERVICES- Recruitand Training for Homecare Worker. - Recruite people who want to work as Homecare Workers. - Provide free homecare worker training classes.- Offer First Aid & CPR Certification class. (Oregon Red Cross)- Refer trained Homecare Workers to families, seniors or State

Home Care Program- Homecare Referral and Matching for Seniors.- Assist in matching with seniors with trained homecare workers.- Provide employment materials, documents, and resources assistance.

AHSC established home care worker training and referral program to train Asian community members to become homecare work-ers and then make linguis-tically and culturally ap-propriate matches to Asian families and seniors in need of in-home care and com-panionship.

Organizing the Asian Community Health Fair is one of the ways to real-

ize our vision. The last 9 Health Fairs have reached 4,590 people and

provided free screening to 2,286. The 10th Annual Asian Community

Health fair provided 1,241 screenings in five hours on August 20th,

2011. 396 people and their families registered for needed services,

including Blood glucose, blood pressure, cholesterol, clinical breast ex-

amination, vision and glaucoma screening, hearing screening, dental,

mental health, classical Chinese medicine and immunization.

This year’s fair has a new partner-“Let’s Get Healthy!”(LGH), an educa-

tion and research program based out of Oregon Health and Science

University. This program brings free, fun health exhibits and health

screenings. Participation is voluntary and LGH collects data anony-

mously. The health data will help scientists to study the relationships

among different health factors. This is the pilot year of partnership be-

tween AHSC and LGH. The experience learned this year will guide LGH

to be more cultural appropriate to reach Asian community next year.

Another new partner this year is the Korean Metro Lion’s Club. Their

support helps bring the Lion Club Mobile Health Screening Van. To-

gether with the mobile dental van from Comfort-Care Dental, and Red

Cross mobile Disaster Relief van, this year’s fair has created a new im-

age to the community.

and Science University, National College of Natural medicine, Oregon Pub-

lic Health Division of Oregon Health Authority, community leaders and
