Re-cap: Model Design• $2.5 million for a 12 Month Project to Develop a

State Health System Innovation Plan

• Description of the state’s health care environment and plan for improving population health

• Develop a Value Based Payment and/or Service Delivery Model and the plan for transformation

• A Health Information Technology Plan- supports transformation

• Develop the Innovation Plan- potential foundation for a Delivery System Reform Incentive Payment [DSRIP] Plan

Arizona’s Focus•Leverage and accelerate coordination and integration between physical health and behavioral health providers

•Improve justice system transitions through HIT infrastructure and health plan interfaces to coordinate coverage and care with the Arizona Dept. of Corrections, jails, and probation systems

•Enhance and develop regionally based care coordination models for the AIHP, collaboration with IHS, 638, and non-tribal providers to support infrastructure development that will reduce system fragmentation

•Mercer serving as project consultant…assisting with stakeholder engagement and Plan draft

•Wide range of stakeholder meetings are being held and scheduled

•State Population Health Assessment section based on ADHS State Health Assessment document
