have read and understood the information contained in the Officials’ SAFETY BRIEFING NOTES which are in the Marshals Handbook available for me to read on the Motorcycling Australia website www.ma.org.au prior to the 2018 FIM Motul Superbike World Championship, Phillip Island (the Event) or a previous event, and will abide by the advice given at all times.

I have also read and understand the information provided to me in the INTRODUCTION TO EVENT ORGANISATION AND OFFICIAL DUTIES for the Event, which is incorporated in the Marshals Handbook, which has previously been made available to me.

I will attend all scheduled briefings at which I am required to attend or, if unable to attend for any reason, will notify the organisers either direct or through my senior officials.

I understand the role that has been offered to me and agree that I will not participate in this role unless I have been properly instructed in the role and am familiar with the requirements of it.

I believe that I am medically fit and able to carry out the role allocated to me and, that if this changes at any time, I will report this to the organisers either direct or through my senior officials.

By the presentation of this signed note, I am requesting the appropriate credentials for my role at the Event, and agree that I will not pass these credentials on to any other person, nor abuse the privileges provided by such credentials. I further agree to comply with and be bound by the conditions of entry (available at www.phillipislandcircuit.com.au and at entrances to the Event), conditions of credentials and any other applicable terms or conditions in respect of my involvement at the Event.
