Sigma Institute of Health Careers, Inc. is dedicated to excellence in education and is committed to the delivery of relevant, in-demand programs and provides an academic environment that fosters professional growth,

development, and placement.

STATEMENT OF PURPOSE

SIGMA will endeavor to discover, develop, empower, and educate individuals with the potential of serving in the health field.

PHILOSOPHY

We at SIGMA believe that education is a means of transportation to a higher level; therefore it is the beginning of the journey to success. Without the right plans and tools, how can you build? You must have a

solid foundation to begin with. SIGMA hopes to give each student that foundation which they need to enable them to soar to new heights. Out of this belief comes our motto:

HISTORY OF THE INSTITUTION Sigma Institute of Health Careers, Inc. was established by South Florida nurse, Dr. Vanilyn Brown-Daley. She has risen to the highest level in professional nursing, in charge and administrative positions. Dr. Daley has had forty plus years in the medical field, with a passion for all people and a concern for the health of their community. She decided to operate a health career training institution to serve interested members of their community who desire a career in health service. In 2012, the Commission for Independent Education granted licensure (License # 4701) for the Sigma Institute of Health Careers, Inc. to provide education in Nursing and Practical Nursing in the heart of Royal Palm Beach. The campus was relocated to Coconut Creek in July of 2013. Sigma was relocated to Oakland Park in September 2016. Sigma offers one diploma program and one Associate Degree program. With the full participation of individual citizens, the collaboration of business, industry and educational groups, Sigma will continue to strive to promote individual student development and to improve the quality of life in the communities we serve.

PRACTICAL NURSING PROGRAM (1350 HOURS)

(Clock hour defined as being 50 minutes of directed, supervised instruction and appropriate breaks)

OBJECTIVE Graduates of this program are eligible to apply to the Florida Board of Nursing to take the NCLEX examination to become Licensed Practical Nurses. Licensed practical nurses (LPNs) provide basic nursing care. They work under the direction of registered nurses and doctors. Employment of licensed practical nurses is projected to grow 16 percent from 2014 to 2024, much faster than the average for all occupations. As the baby-boom population ages, the overall need for healthcare services is expected to increase. LPNs will be needed in residential care facilities and in home health environments to care for older patients. Licensed practical nurses work in many settings, including nursing homes and extended care facilities, hospitals, physicians’ offices, and private homes. Most work full time. COURSE DESCRIPTIONS T = Theory / L = Lab / C = Clinical

PNP100 – Health Career Core Course T - 45 hrs/L - 15 hrs This course includes the introduction of basic concepts and skills generic to all health care professionals. It includes an orientation to school, program, study and test taking skills, the health care delivery systems, communication skills, legal and ethical responsibilities, wellness and diseases, safety and security, emergency situations, computer literacy, medical terminology, knowledge of blood borne diseases including HIV/AIDS, Infection control includes the following skills: hand washing and gowning, gloving, and masking. Healthcare Provider CPR (BLS), first aide, monitoring and recording vital signs, and domestic violence, are presented along with a review of mathematics and science related to health care. PNP101 – Body Structure and Functions T - 25 hrs/L - 5 hrs Normal anatomy and physiology of the human body is studied. Course content progresses from the simplest form of life, the cell, to the more complex structures and functions of the human body. Course content is reinforced in succeeding courses. The course also includes growth and development, related to the stages of adult-hood, the aging process and appropriate nursing interventions, using critical thinking skills, safety in the geriatric environment, use of assistive devices, and elder abuse. Erickson’s basic patterns of psychosocial development are explored. The student is introduced to cultural and environmental influences that affect the adult patient. Dementia and Alzheimer’s disease with the appropriate nursing care, related to signs and symptoms as presented by the patient, are incorporated in the course. PNP102 - Fundamentals of Nursing I T - 30 hrs/L - 15 hrs This course focuses on the basic personal care needs of the patients with the measurement of vital signs (TPR and BP), incorporating standard precautions and applications of safe nursing practice. It includes the performance of patient care procedures, physical comfort and safety functions specific to nursing.

PNP103 – Clinical Practicum I C - 135 hrs This course will provide the student with actual hands on experience in a clinical setting. Clinical rotations will include Nursing Home (minimum of 20 hours) and Adult Daycare settings. Clinical assignments are task oriented, and designed to assist students to develop manipulative skills in providing personal care to patients with emphasis on the Geriatric patient. The clinical skills check-off list, and performance evaluations are tools used to measure students’ performance. Students will be given opportunity to utilize the nursing process in, assessment, planning care implementation and evaluation of care. PNP104 - Fundamentals of Nursing II T - 40 hrs/L - 5 hrs This is a fundamental knowledge and skills course, utilizing scientific principles. The course Includes critical thinking and decision making skills. A foundation for specific documentation in charting and reporting, and assisting with the collection of patient data will be emphasized. The admission, transfer, and discharge processes, hot and cold applications and monitoring oxygen therapy will be studied. The individual nutrients essential for optimal physical and mental health will be studied and Incorporated into the Food Pyramid Guide. The end products of protein, carbohydrate, and fat digestion will be introduced. Economic practices in purchasing, storing and preparing food for the individual and the family will be discussed. Serving a meal tray and feeding a patient are skills to be accomplished satisfactorily by the student. Standard hospital diets will be studied and diet modifications for the standard hospital diets are introduced. Prerequisite: Passing grade PNP.FNI

PRACTICAL NURSING PROGRAM (Continued)

(Clock hour defined as being 50 minutes of directed, supervised instruction and appropriate breaks) PNP105– Pharmacology T - 35 hrs/L - 10 hrs This course includes calculations, preparation and administration of medications, drug classifications, resources and nursing implications. Safety and legal/ethical responsibilities in the drug administration are stressed. Students are given the opportunity to administer selected medications under direct supervision of the instructor in subsequent courses. Observation, communication and documentation are emphasized. Course content is reinforced and expanded in succeeding courses. PNP106 - Medical-Surgical Nursing I T - 40 hrs/L - 5 hrs This course is designed to provide the student with concepts specific to pre and postoperative care, including pain management. Mental health nursing is emphasized with focus on therapeutic communication skills, chemical substance abuse, depression, anxiety, and personality disorders. An overview of the oncology patient will be introduced. PNP107 – Clinical Practicum II C - 135 hrs. This course is designed to enable students to gain clinical experience in an acute Medical/Surgical unit. Clinical are graded on a Pass/Fail basis. PNP108 - Medical/Surgical Nursing II T - 110 hrs/L - 25 hrs This course includes the disorders of the body systems. Nursing assessment, diagnostic tests, nutritional modifications, signs and symptoms, pathophysiology, psycho-social aspects, medical management, and appropriate nursing interventions will be studied. The study of each body system centers on health alterations which affect the essential activities of daily living and human function. Appropriate nursing interventions are included in each course. PNP109 – Clinical Practicum III C - 135 hrs. Students continue Medical/Surgical clinical practice to solidify their theoretical knowledge gained in an acute med/surg unit. Clinical are graded

PNP110 - Maternal-Child Nursing T - 100 hrs/L - 35hrs Pregnancy and childbirth are viewed as a normal process. Emphasis is placed on the care of the mother from the conception through involution, and care of the newborn. Integral parts of this course includes: body structure and function, growth and development, medical/surgical asepsis, nutrition, pharmacology, multi-cultural factors, and safety. Fundamental Nursing Skills and Practices are incorporated and skills pertinent to obstetrics are taught. The students will learn to recognize the normal and abnormal physiological changes which can occur during pregnancy and childbirth. Interpersonal relationships, legal/ethical aspects, patient education, discharge planning, ethnic and cultural practices related to pregnancy and childbirth are discussed, as well as teenage and other high-risk pregnancies. Beginning with growth and development from neonatal through adolescence, common diseases and disorders of children are studied. The student learns to provide care to children, taking into account, size, level of comprehension and altered health state. Emphasis is placed on nurse/child/parent relationship and safety in the delivery of nursing care. The students’ knowledge of medication is expanded to Include consideration of dosage alteration in the administration of medication to children. The concept of play therapy, in providing care, is a component in the course. Community, state, and federal agencies, concerned with care and well-being of children are discussed in this unit. PNP111 – Clinical Practicum IV C - 135 hrs Utilizing the nursing process in the clinical setting, the student will provide support and offer instruction for the care of the new family unit. Clinical rotations are mainly in obstetrical departments with emphasis on mother-baby care, and observational experiences in labor and delivery suite. Student will gain knowledge of common diseases and disorders of children. Utilizing hospital pediatric department, physicians’ offices, well-baby clinics and care centers.

PRACTICAL NURSING PROGRAM (Continued)

(Clock hour defined as being 50 minutes of directed, supervised instruction and appropriate breaks) PNP112 – Clinical Practicum V C - 180hrs The hospital based experience will provide clinical patient assignments which are increased in complexity, with emphasis placed on the students’ ability to prioritize and organize nursing care for multiple patients. Medication administration techniques are refined by the student. The 90 hours community based experiences will be observational only and explore a variety of community based settings. These experiences will assist the student in assuming the responsibilities of an entry level practitioner on successful completion of the licensing exam.

PNP113 - Graduate Transition T - 90 hrs This course prepares the student for transition to the graduate role. Preparation for licensure is accomplished by the student completing application for state board examination. Further discussions will take place regarding the role of the Florida Board of Nursing and the purpose and content of the Nurse Practice Act. Career opportunities are explored. The home health experiences will be observational only. The student will have an opportunity to compare the variations in the provision of care in the patient’s home and in an institutional setting.

NURSING PROGRAM ASSOCIATE OF SCIENCE DEGREE IN NURSING

OBJECTIVE

Graduates of this program are eligible to apply to the Florida Board of Nursing to take the NCLEX examination to become Registered Nurses. Registered Nurses (RNs) provide and coordinate patient care, educate patients and the public about various health conditions, and provide advice and emotional support to patients and their family members. Employment of registered nurses is projected to grow 16 percent from 2014 to 2024, much faster than the average for all occupations. Growth will occur for a number of reasons, including an increased emphasis on preventive care; growing rates of chronic conditions, such as diabetes and obesity; and demand for healthcare services from the baby-boom population, as they live longer and more active lives. Registered nurses work in hospitals, physicians’ offices, home healthcare services, and nursing care facilities. Others work in correctional facilities or schools, or serve in the military. MAC110 - College Algebra 3.0 Credits This course develops the students’ understanding of mathematical methods and further explores the application of Algebra to Healthcare. It introduces the student to data analysis. ENC110 - English Composition 3.0 Credits This course will teach the students skills in analyzing the rhetoric of prose passages in composing & writing essays in various rhetorical modes, with a strong emphasis on expository, analytical, and argumentative writing as well as personal and reflective writing. Research methods and library skills will be introduced, and a documented paper is required. COM220 - Communications 3.0 Credits This course introduces the concepts of communication and the importance of communication skills in their professional and personal lives. BSC105 - General Biology 3.5 Credits This course focuses on the scientific study of the biological, psychological, and social changes, which occur over the human life span from conception until death. Topics addressed in this course include the effects of heredity and culture on human behavior and mental processes during prenatal development, infancy, childhood, adolescence, and adulthood. CHM103 – Chemistry for Health Related Sciences 3.0 Credits This course examines basic chemical actions within the body. This includes atoms, masses, PH, energy, amino acids, enzyme function, metabolism/catabolism, and genetic actions. The nature of immunity is also examined.

PSY110 - Psychology 3.0 Credits A broad overview of the general field and fundamental principles of psychology is provided in this course. All topics offer application of information to real life settings, as well as a better understanding of the individual as a learner.

MCB200 - Microbiology 3.5 Credits This course examines the microscopic structures within the body. Microbes, Fungi and Viruses are studied with their uses and actions in the body and are discussed. Microbes are correlated to diseases within the human body.

HCC100 - Health Careers Core 6.0 Credits This course includes the introduction of basic concepts and skills generic to all health care professionals. It includes an orientation to school, program, study and test taking skills, the health care delivery systems, communication skills, legal and ethical responsibilities, wellness and diseases, safety and security, emergency situations, computer literacy, and medical terminology, knowledge of blood borne diseases including HIV/AIDS. Infection control Provider CPR (BLS), first aide, monitoring and recording vital signs, and domestic violence, are presented. BSC293C - Anatomy & Physiology I 3.5 Credits An introduction to the structure and functions of the human body is provided. Topics include chemistry, histology, and study of the integumentary, skeletal, muscular and nervous systems. A grade of C or higher is required for this course to be used as a General Education course.

(Continued) BSC294C - Anatomy & Physiology II 3.5 Credits A continuation of A&P I, the circulatory, endocrine, digestive, excretory, respiratory, and reproductive systems of the body are studied. A grade of C or higher is required for this course to be used as a General Education course. SOC110 - Sociology 3.0 Credits This course reviews definitions of sociology then explores social systems of the family and the community. It further examines religious beliefs and cultural norms both globally and locally. NUR114 – Mathematics 1.0 Credit This course provides an intensive review of mathematics operations involving fractions, decimals, percents, ratios and proportions. Units and measures in apothecaries', metric, and household systems are also discussed with a major emphasis upon application for the calculation of both oral and parenteral drug dosages. DIE220 - Nutrition for a Healthy Lifestyle 1.0 Credit This course examines nutrition principles across the life span. Nutrients are grouped and classified. The implications of nutrition in America is also discussed and related to illnesses. NUR114 – Pharmacology for Nursing 2.0 Credits This course includes basic mathematics, calculations, preparation and administration of medications. Drug classifications, resources and nursing implications are also examined. Observation, communication and documentation are strongly emphasized. Ethical/Legal aspects of pharmacology are discussed. NUR121 – Medical-Surgical Nursing I 4.0 Credits This course orientates the student to health care with an emphasis on the profession of Nursing from a global, national and local perspective. It includes the focus of Nursing, history of Nursing, scope of the Registered Nurse, Ethics, legal aspects of Nursing, client rights and responsibilities in Health Care, Personal Health & Hygiene and Communication. It also encompasses the area of role definition; providing/managing care of individuals and groups utilizing goal attainment to reach an optimum state of health and wellness. This course will enable students to develop confidence and self-esteem. Grooming and deportment are essential parts of

development, life skills and personal presentation. It also covers the importance of volunteerism and community service. NUR121L – Medical-Surgical Nursing I Clinical 4.0 Credits This Clinical accompanies NUR1211. This course describes the development of the student in areas of basic nursing skills. It examines alternative, complementary, and holistic approaches to patient care. It also examines the profession of nursing, ethical issues, the nursing, process, models of care, caring in healthcare, and the control of infectious diseases. NUR122 – Medical-Surgical Nursing II 4.0 Credits Application of the nursing process while providing nursing care for adult and elderly clients with common medical conditions. Study and care of the surgical patient from admission through discharge from the hospital. NUR122L – Medical-Surgical Nursing II Clinical 4.0 Credits The clinical accompanies NUR1212. Application of the nursing process while providing nursing care for adult and elderly clients with common medical conditions; study and care of the surgical patient from admission through discharge from the hospital. NUR252 – Psychiatric Nursing 3.0 Credits This course studies mental health and illness with focus on therapeutic communication skills, chemical substance abuse, depression, anxiety and personality disorders, and social/cultural norms are discussed in relation to mental illnesses. NUR252L – Psychiatric Nursing Clinical 1.0 Credits This Clinical accompanies NUR2520. Students will apply the theory learned to a clinical setting. The essential features of this clinical experience are professionalism, skills and competency development, scope of practice for psychiatric nursing, professional communication skills, and therapeutic use of self. NUR242 - Maternal-Newborn Nursing 3.0 Credits This course introduces the student to the care of the OB and Newborn Patient. The student studies normal and abnormal

This course will allow the student to integrate theory into practice with emphasis on care of the Parent/Child in Hospital Units and Community Settings. NUR231 - Pediatric Nursing 3.0 Credits This course is designed to provide an understanding of growth and development through the stages of childhood and the application of the nursing process. NUR231L - Pediatric Nursing Clinical 2.0 Credits This Clinical accompanies NUR2310. This Clinical focuses on maintenance, and the prevention of illness, care of the pediatric client, and deviations from the normal state of health. The definition of client care includes performing assessments, and providing client/family education. Topics include: health maintenance and prevention of illness; care of the individual as a whole; and deviations from the normal state of health in the pediatric client; client care, treatments, pharmacology, medication administration, and diet therapy related to care, and standard precautions. NUR271 - Management and Leadership in Nursing 3.0 Credits The course explores management and leadership theories and the application of them to Nursing. Participants will examine organizational behaviors, and develop conflict resolution, collaboration, planning, and decision making skills. In addition this course will cover the role and definition of a nurse preceptor and introduce learning styles and teaching methods and explore assessment and evaluation of self as a preceptor and the student. NUR271L - Management and Leadership in Nursing Clinical 1.0 Credits The Clinical accompanies NUR2718. This Clinical will allow the student to apply management and leadership skills in the hospital setting. NUR273 - Graduate Transition 1.0 Credits This course is designed to assist the student to transition from student to assuming the role of the Registered Nurse. REFUND POLICY

A. Should student be terminated or withdraw for any reason, all refunds are made according to the following refund schedule:

1. Cancellation must be made in person or by Certified Mail. 2. All monies will be refunded if the applicant is not accepted by the school or if the student cancels within three (3)

business days after signing the enrollment agreement and making initial payment. 3. Cancellation after the third (3rd) Business Day, but before the first class, will result in a refund of all monies paid, with

the exception of the registration fee, which is $150 for Diploma & Associate Degree Programs. 4. Cancellation after attendance has begun, but prior to 20% completion of the semester, will result in a Pro Rated refund

computed on the number of hours completed to the total semester hours. 5. Cancellation after completing 20% of the semester will result in no refund. 6. Termination Date. The termination date for refund computation purposes is the last date of the actual attendance by

the student. 7. The refund process will be initiated once a withdrawal notice is received in writing. The refund process will be

completed in 30 days after receipt of withdrawal/cancellation notice, unless prior written arrangements are made with administration.

B. Should the school cancel courses or programs, the school will notify the enrolled student by phone and certified mail. Tuition and registration fees paid for the specific course or program cancelled will be refunded.

C. Disclaimer: The School cannot guarantee employment. The Enrollment Agreement and/or contract in addition to the catalog shall be a binding agreement between the school and the student. The agreement shall not be binding until it is completed by the student and accepted by a school representative.

LIBRARY/MEDIA POLICY

A. LOANING POLICY 1. All book and resources must be loaned by administration/staff. 2. All books must be signed out by persons loaning them. 3. Books and resources may be loaned to students and staff free of charge for five (5) days. After the five days

book(s) must be returned and re-issued 4. If books are not returned after five days, a fine of one dollar ($1) per day will be charged. 5. Liability as follows: 6. Loss of books – Replacement value

7. Reference Books and Resources are only to be used on school premises and are subject to the same policy and procedures.

Media Services provide Internet use, Computer use, access to library access, and reference material access. 3. For internet use, students will have access to computers available in the computer lab. Students can also access

the internet using the institutions wireless network via personal Wi-Fi capable laptop. 4. Reference material, books, videos, tapes, CD-ROM’s, and/or audio CD’s are available for student use and must be

checked-out and returned in a timely and proper manner. 5. The Administrator of the institution approves and controls all budgetary support for Media Services. 6. Media Services Direction/Coordination

a. Director of Student Services is responsible for coordination and oversight for all media services. 7. Administrative Assistants will:

a. Provide access of media services to students. b. Assist students with questions regarding computers, internet access and/or library books and resources. c. Assist students and instructors in completing appropriate forms necessary for checking out material from

the library or equipment. d. Assist students with information to access the institutions wireless network for internet access via Wi-Fi

capable laptop. 8. Any suggestions, complaints, questions, or concerns regarding Media Services by students, may be expressed via

written evaluation, which are available upon request. 9. The institutions Quality Improvement Team in the next available quarterly meeting will review all evaluations and

revisions are made, if necessary.

ADMISSION REQUIREMENTS

High School Diploma or GED Equivalent

Personal interview with Administrator or designee, who will assess or review their qualifications for admissions.

Take the Wonderlic Scholastic Exam (SLE) and achieve a minimum score of 15.

Sign enrollment agreement and make deposit.

Must show proper identification (Driver’s License, State ID, etc.), and school documents if necessary.

Must be 17 years or older.

No criminal record preferred. If applicant has pleaded “guilty”, “no contest” or has been convicted of a crime, the date of offense, seriousness or nature of violation or rehabilitation will be explored and considered.

If proper identification, documents and/or requested information are not provided, we, at Sigma reserve the right to reject that applicant.

Failure to fulfill admission requirements will result in denial of admission to the school, which will be documented by the Institution. The document will be kept on file for at least one year.

TRANSFER OF CREDIT

Sigma Institute of Health Careers, Inc. may accept transfer credits from similar programs run by licensed institutions. The Sigma Institute of Health Careers, Inc. cannot guarantee that its credits will transfer to another institution. Students are advised to check with the receiving school before assuming any or all credits will be transferable.

CLASS STARTING AND ENDING DATES

A student may enter the school at the beginning of any program (the first day of class). The date of completion is determined by the number of hours completed. All students are expected to complete the required credits/hours without exception.

CERTIFICATION OF PROGRAM COMPLETION:

Students who have successfully completed a program will receive a Practical Nursing Diploma or an Associate of Science in Nursing Degree in their respective program of study.

SCHOOL PIN:

Students may purchase a school nursing pin upon successful completion of the Nursing program.

RECOGNITION NIGHT:

There will be one formal ceremony each year for all graduates of the Institution. All graduates, with a designated number of family and/or friends are welcome to attend.

GRADING To evaluate student academic progress a written examination will be given during and/or at the completion of each semester, program, and/or course. The written portions of the examination are graded according to the following schedule.

A 94 – 100% Excellent B 87 – 93% Good C 80 – 86% Satisfactory D 0 – 79% Unsatisfactory

1. Students will sign a written grading criteria provided at the beginning of each term. The written grading criteria will be explained by the instructor at the beginning of each course. The student is required to maintain a passing grade of 80% in each term/course/semester.

2. Interim/Progress evaluation issued to students at mid-point of a term who are not progressing satisfactorily.

3. Cheating is defined in the dictionary as “to violate rules and regulations” or “conducting matter fraudulently for profit to oneself.” Cheating or lying is unacceptable behavior. Students found cheating or lying will be subject to suspension or expulsion.

Students are expected to maintain a grade of A, B, or C in order to have a satisfactory academic progress. In addition students are graded pass or fail during their clinical rotation based on their performances by the instructor’s evaluation of the quality of work, demonstration, and the student’s ability.

DISCLOSURE

All programs by the Sigma Institute of Health Careers, Inc., of Broward County, Florida are given direction by the Commission for Independent Education and The Nursing Programs by the Florida Board of Nursing. Graduates from the Practical Nursing program may be eligible to apply to the Florida Board of Nursing to take the NCLEX-PN Examination to become Licensed Practical Nurses. Graduates from the Nursing (AS) program may be eligible to apply to the Florida Board of Nursing to take the NCLEX-RN Examination to become Registered Nurses. Continued admission is based upon continued licensure of the Institution and the programs by the Commission for Independent Education and program approval by the Florida Board of Nursing for the nursing programs. Sigma Institute of Health Careers reserves the right to determine readiness of students to sit for the state licensure examination through a series of assessments and activities required for eligibility (See Nursing Student Handbook for Requirements). Graduates who have met academic requirements but do not demonstrate state licensure examination readiness will not be eligible to take exams or practice in their field.

CRITERIA FOR PROGRAM PROGRESSION

1. Students will be allowed one (1) retake on final examinations.

* Upon passing a retake final examination the highest achievable score will be 80%.

2. Students who fail the Final exam for the second time will be required to retake course(s) and meet current admission and financial criteria. The student may be required to complete a remediation program. Students may need to complete individualized re-entry prescription which is based upon individual need.

3. Prior to re-entry, students who fail their clinical coursework will be required to take a Clinical skills test or repeat the clinical experience. A student skill checklist that is aligned with the skills a student should have attained during the term or course will be used.

4. Students will be allowed two (2) chances to retake courses. If a student does not pass the course on the third (3rd) chance the student may be removed from the program and/or Institution. Student is responsible for tuition and fees each time the course is taken.

Special Criteria for Nursing Program 1. Students must successfully complete each course before

progressing on to the next course. 2. Students who fail a course examination are given the

opportunity to retake it within one week. Students who are not successful on the retake examination are enrolled in a mandatory remediation program at course cost. The remediation course is 25% of the total clock hours of the failed course (e.g. a course offering 2 credits equals 32 clock hours, therefore remediation must be a minimum of 8 hours spread over a 4 week period). A mandatory test will be administered by Sigma at the end of remediation.

3. Students who are not successful in the remediation course must retake the entire course the next time it is offered and meet current admission and financial criteria.

4. Students will be allowed two (2) chances to retake courses. If a student does not pass the course on the third (3rd) chance, the student may be required to complete a Practical Nursing program before reentry into the Nursing program.

5. Transferees or applicants for re-entry into the Nursing Programs, who have had a break in their instructional continuum, may also be theoretically and/or clinically assessed using skills and/or written test.

6. Students enrolled in the Nursing program will be required to attend an Exit review as prescribed by the school. The student will be required to take and pass pre-exit examinations before receiving clearance to take the final exit exam. Students who are unsuccessful must meet with administration to discuss options.

GRADUATION REQUIREMENTS

An AS Degree or Diploma is presented to the Student that has:

1. Maintained at least passing grade of “C” 2. Successfully completed all required courses. 3. Attended all class and clinical hours 4. Fulfilled all monetary obligations to the school. 5. (Nursing Program Only). Obtained a minimum of 90% in

an NCLEX Diagnostic Readiness test, after completion of a review program

The above requirements must be met before the awarding of a Degree or Diploma.

HOURS OF OPERATION SCHOOL / OFFICE / LIBRARY HOURS Hours of operation for the school are 9:00 am – 9:00pm, Mondays through Thursday. Office/Library hours are from 9:00am – 9:00pm Monday – Thursday. School / Office / Library closed Saturdays and Sundays.

1. Lunch and dinner breaks are set at instructor’s discretion.

2. Clinical schedule are set up based on Community area experience availability. Therefore, students may be required to attend a clinical schedule that includes weekday and weekends.

Sigma reserves the right to change any of the above if administratively necessary without prior notice.

MISCELLANEOUS SERVICES

A. Housing The school does not maintain housing for students. A list of reliable realtors and rental properties in close proximity to the school will be provided the student that requests housing assistance at the time of enrollment. B. Student Records Student Records are permanently retained by the school and are available to students upon individual request. Student records will be provided to potential employers only after written request has been made by the student. C Student Placement Students are assisted with placement and furnished names and addresses of employment possibilities. Inquiries made to the school from potential employers will be posted on the bulletin board. The school will assist the student with employment to the best of its ability, but cannot guarantee employment. D Financial Aid Sigma Institute of Health Careers, Inc. does not offer financial aid.

GENERAL RULES & REGULATIONS

A. Background Checks/Drug Abuse Testing Please be advised that as a part of the application process, a series of background checks and Drug Abuse Testing must be done and submitting to checks does not in any way guarantee admission to any of Sigma’s programs. Sigma further reserves the right to offer admission on a conditional basis or to outright deny admission based on results of the background check. Each applicant will be reviewed on any individual basis.

B. Attendance / Class Cuts After five unexcused absences, the student is counseled and placed on probation. Should the student have another unexcused absence while on probation, the student is counseled and may be terminated. Class cuts without prior approval from the instructor of chief administrative officer are not permitted and are considered an absence for the entire class period.

C. Tardiness A student arriving after attendance has been taken in consideration late and will be marked absent unless the instructor considers the reason for tardiness legitimate. Each class time missed in excess of 15 minutes must be made up by the student prior to graduation.

D. Conduct Students are expected to conduct themselves in a professional manner at all times. Foul language, possession of un-prescribed drugs or alcoholic beverages, and disrespectful behavior is considered unsatisfactory conduct and may be grounds for dismissal. A student who conducts him/herself in a manner detrimental to the school, staff, or other students will be terminated. Theft of property from the school or other students is grounds for immediate dismissal.

E. Make-up Work Students who have been absent for any reason are required to make up any missed classes before proceeding to the next course of study. A student may make up missed time by adding more hours to their day or attending additional classes which are conducted for students who have been absent or a student who wishes extra practice time. F. Probation A Student that does not adhere to the attendance policy will be placed on probation for 30 days. If a student is absent while on probation, he/she will be counseled by the Chief Administrative Officer, which may result in termination. Probation periods are not applicable for academic progress. G. Interruptions in Training / Termination A student is not terminated for failure to learn the required skills. A student that has not successfully completed the examinations at the end of each course is encouraged to attend the additional class session for extra practice. The student is permitted to re-take the examination within a reasonable time as determined by the instructor. If the student fails to achieve a passing grade the second time, he / she is counseled by the Administrative Officer on the advisability of continuing and that if he/she decides to continue, a Certificate of Completion may not be awarded. H. Leave of Absence A student may be granted a leave of absence not to exceed two semesters. A request for a leave of absence must be in writing and the date of expected return must be specified. If the student does not re-enter within the specified time and has not notified the school, the student’s contract will be terminated and he/she is granted a refund according to the Refund Policy. I. Re-entry A student that has canceled or has been terminated and desires to re-enter the program of study must notify the school and follow the required admission procedures. A student that was terminated for any reason must have an interview with the Administrative Officer and show cause why he/she should be reinstated. The decision of the Administrative Officer is final. J. Student Services Student Services Staff are available to provide academic advisement, financial aid advisement, personal advisement, and placement services.

GENERAL RULES AND REGULATIONS HEALTH: Health care is the responsibility of each student. Therefore, students are required by the health industry to carry health/accident insurance. A statement indicating that you have coverage is required. Students who do not have health/accident insurance, will be provided with information. Physical examination, TB skin test (PPD) or chest x-ray, and immunizations/titers are required upon entrance and must be on file in the student’s folder. A student with an incomplete health record will not be permitted in the clinical area. Clinical agencies have the authority to request health record information. The instructor should be immediately informed of the following: illness, medications in use and accidents or incidents which occur in the school or clinical facility. PREGNANCY: A student who is pregnant shall inform the administrative officer of her condition. At the beginning of each trimester of pregnancy, written permission from the student’s obstetrician/midwife is required stating that she is physically capable of performing all nursing duties without restriction/limitations. The safety of the expectant mother and the unborn child is ultimately the responsibility of the student. After delivery, the student may return to the program with written permission from her obstetrician/midwife stating that she is physically capable of performing all nursing duties without restriction/limitations. SMOKING: No smoking allowed at Facility or Clinical areas. FEES Please see payment schedule and refund policy TRANSCRIPTS Students who require official transcripts must submit a Transcript Request Form which is available at the Institution. Please be advised that requests may take two to four weeks to be completed and for transcript copies to be made available. Fees may apply. PART-TIME WORK: It is recommended that full-time students work be limited to twenty (20) hours per week. In the event of unsatisfactory progress, a student may be counseled to consider other options. It is illegal for a student to work as a Licensed Practical Nurse while enrolled in the PN program. It is illegal for a student to work as a Registered Nurse while enrolled in the Nursing (AS) program.

MEALS: Eating is to be confined to designated eating areas. Food, beverages, candies and chewing gum are not permitted in classroom or clinical areas. TRANSPORTATION: Students are responsible for their own transportation to and from Institution and/or clinical facilities. PARKING: On site parking is available at the campus. When in clinical areas, the student shall follow each centers’ parking policy. TELEPHONE: Classroom and clinical telephones are not for student use. No personal calls or messages are to be made or received during school hours. In case of emergency, have the caller telephone the school or clinical facility to notify the instructor who will deliver the message. Please inform your family and/or others concerned regarding this policy. CELLULAR PHONES: Cellular phones may not be audible and may be used only for emergencies. Cellular phone usage is prohibited in the classroom and clinical areas. CLASSROOM ENVIRONMENT: Students are responsible to maintain a clean and tidy classroom/laboratory area. Food or beverages are not permitted in the classroom or clinical areas. ABSENTEEISM AND TARDINESS: 1. The student is required to notify the school or clinical

facility at least thirty (30) minutes before the expected time of arrival if he/she finds it necessary to be absent or tardy.

2. The student is responsible for all material covered in class/laboratory.

3. Written verification from a Physician permitting resumption of total nursing activity is required after a medical absence of three (3) or more days.

CONFIDENTIALITY: The student understands that any and all information obtained while in clinical classroom areas or any information learned while working/studying in a health care facility will remain confidential at all times.

General Rules and Regulations (Continued)… DRESS CODE: Students have the responsibility to learn and observe the basic standards of appropriate dress, personal cleanliness, modesty and good grooming. Nursing students, by the nature of their chosen vocation are required to wear specified uniform and other items of dress which are not offensive or hazardous to the health and safety of themselves or others. 1. Daily personal cleanliness should include: A. Bathing B. Use of a suitable deodorant C. Freshly laundered and wrinkled-free clothing, shoes polished and laces clean D. Oral hygiene and care E. Scrupulously clean, fingertip length nails: only clear colorless nail polish is permitted. F. Hair while in uniform must be: 1. Clean 2. Worn above the shoulders and away from the face 3. Hair style and coloring should be conservative and professional 4. Conservative device may be used to secure hair. Consult Instructor as to appropriateness if in doubt. G. Facial Hair (Male student) 1. Clean shaven at all times 2. Beards: clean, neatly trimmed and not more than one inch in length 3. Mustaches-clean, neatly trimmed and not to extend below the upper lip level 4. Sideburns-trimmed, evenly tapered and not to extend lower than the earlobe 2. Cosmetics and Jewelry: A. Make-up 1. Minimal eye and facial makeup 2. No false eyelashes B. Perfumes - Minimal use only C. Jewelry: (in restricted areas, hospital policy will be observed) 1. Rings: none with exception of wedding band 2. Earrings: ball post ¼” diameter, white, gold, or silver color. Only one earring per ear. 3. Bracelets/ankle bracelets and Necklaces are not permitted 4. No visible body piercing/jewelry permitted with the exception of earlobes (I.e. tongue, nose, eyebrows) 3. Uniform (Women) A. Top - White or Royal blue scrubs B. Skirt - White or Royal blue C. Pants - White or Royal blue scrubs D. Lab (Consultant) Jacket 1. White color 2. Long sleeves 3. Notched collar with lapels 4. Program emblem: Blue/White motif 5. Jacket may not be worn during patient care 6. Other jackets or sweaters are not allowed E. Stockings 1. Must be worn with dress or skirt at all times 2. Clean, free of runs or holes 3. Plain white/skin color hose only 4. May wear plain white socks with pants

Ball-point pen: black ink to conform to charting policy of the health care facility E. Stethoscope 6. Conformance: Any student who does not conform to the uniform standards is subject to dismissal from the classroom or clinical areas and will be counted absent.

STUDENT COMPLAINT PROCEDURE

Students who want to appeal academic or disciplinary actions shall submit a written complaint request for reconsideration to the school. Upon receipt of the appeal, a copy will be forwarded to the instructor, and the administrator of the school and the senior advisor will be notified. The student will be contacted by school personnel within 5 (five) business days and given an appointment to meet with the instructor. During the instructor/student meeting, steps will be taken to investigate and resolve the issue(s). A decision for the appeal will be provided at the end of the meeting and written copies will be available within 2 (two) business days. If the student is not satisfied with the decision, or the issue is not resolved, he/she can request that the appeal be forwarded to the Senior Advisor, which will occur within 2 (two) business days, to request a meeting. A meeting with the senior advisor will be arranged within 10 (ten) business days by school personnel. During the Senior Advisor/student meeting, the presenting issue and the outcome of the instructor/student meeting will be addressed and steps will be taken to resolve the issue. A decision for the appeal will be provided at the end of the meeting and written copies will be available within 2 (two) business days. If, after meeting with the Senior Advisor, the issue is still not resolved and/or the student is not satisfied, the student can request to meet with the President of the School This meeting will be arranged by school personnel within 10 (ten) business days. The total appeal process, through the school, shall be completed in no more than 30 (thirty) business days. If the student is not satisfied with the Institution’s decision, he/she may forward the appeal to the address below:
