East End Hospice announces the projected opening of the KanasCenter for Hospice Care for fall 2015, seeking candidates for thefollowing positions for inpatient and community settings:
• Palliative Care Nurse Practitioner - Provide comprehensive health as-sessments, diagnosis, and prescribe treatments under the EEH protocol forthe management of acute end of life illness. Current NYS NP license & reg-istration required. Minimum 2 years’ experience as an NP in an acute caresetting required; Hospice or Palliative Care experience strongly preferred.
• RN - Provide primary nursing care as part of interdisciplinary team ofprofessionals & volunteers. Minimum 2 years RN experience in an acutecare setting required; BSN and Hospice experience preferred, current NYSRN license & registration required.
• Social Worker - Provide group and family counseling, crisis interven-tion, benefit determination, assistance with social and economic needs,advocacy, information and referrals. Requirements include LMSW and cur-rent NYS license & registration required, LCSW preferred, minimum 2 yearsexperience as medical social worker.
• LPN – Under the direction of the RN, assists in the implementation ofthe plan of care, identifying patient problems and implement interventions,including pain and symptom management. Minimum 2 years experience as
an LPN in an acute care setting and current NYS license required; Hospiceor Homecare experience preferred.
• Hospice Aide - Follow the plan of care as established by the primarynurse, performing services to meet the patient’s personal care needs.Qualifications include certificate of completion of New York State Depart-ment of Health approved Home Health Aide Training Program and previousHospice or Homecare experience.
• Facilities Maintenance Technician - Performs a variety of generalmaintenance work including plumbing, electrical, carpentr y, maintenanceand other duties related to the upkeep of the facilities. Interact with staff,
tradespeople, patients and their families.
Weekends may be required on a fixed or rotating basis. Valid New York Statedriver’s license and proof of current automobile insurance required for all po-sitions.
