Functionality/Service Overview
Medvivo™ HomePod facilitates telehealth services, specifically designed for the monitoring of Long Term Conditions at home and to enable and promote self-care.
Description of Medvivo ™ HomePod: The diagram below describes at a summary level how the service works:
The Medvivo™ HomePod is a tablet computer used within a patient’s home. It uses questions / protocols to take patients through a daily set of tests and readings from paired medical devices appropriate for the monitored condition (such as blood pressure meter, pulse oximeter, weighing scales and other peripherals). These are communicated via mobile or broadband to the Medvivo central system (CUI). As a result, alerts may be created where certain questions are answered in a way that causes concern, or if vital sign readings are out of pre-defined patient tolerances.
Within the Medvivo™ HomePod service the central Medvivo CUI system is used by responsible clinicians to review alerts. They then have all of the relevant contact details for the patients and other clinicians to manage the alert and agree an action or response, which they then record within the CUI. Responsible clinicians may be Medvivo clinicians (if using Medvivo’s Long Term Condition Monitoring Service), but also will include practice-based or community-based specialist nurses, or other nominated staff. All of this information is then available to the GP through the CUI, and will be available through their PCS in a future release. The central Medvivo CUI system is accessible through a web service.
Summary of Medvivo ™ HomePod Service:
The diagram below describes what is included in the Medvivo™ HomePod service:
Meeting the Implementation Challenge
Medvivo has significant experience in addressing meeting the challenges of implementation and has developed a suite of documentation, tools, training courses and events to support a client seeking to rollout the service. The suite includes:
Referral pathways to assist in the selection of patients
Patient acceptance forms
User guides for the HomePod (for patient use) and for the Web CUI web-browser access to the central database (for clinician use)
A detailed training workshop for GPs and clinicians in the technology, the data and the processes to be followed typically including discussion with one of the Medvivo nurses they will be working with
A patient launch event, including clinicians and other stakeholders
Key benefits of Medvivo ™ HomePod:
The Medvivo™ HomePod service has been shown to
reduce GP appointments
reduce GP home visits
reduce unplanned hospital admissions
reduce attendance at A&E
reduce re-admission rates
reduce length of stay in hospital
increase patient engagement and care plan adherence
Key features of Medvivo ™ HomePod:
Easy to use – the HomePod is a touch screen, designed to be intuitive and leads the patient through the required questions and readings, with large text and graphical display
The HomePod is a flexible ehealth system that enables personalised care plans – the questionnaires can be updated at any time remotely
Educational resources such as documents, videos and web sites are available through the HomePod to promote self-care
No data is stored on the HomePod and is securely transmitted to the central database
Results are available in real-time for clinician review
SIM card included with no hidden line rental fees
Access through the internet and Medvivo’s WebCUI software to the central CUI database to access patient information (5 licences included per 100 HomePods)
Access through the internet and Medvivo’s Metrix software summary statistics and service information data (5 licences included per 100 HomePods)
HomePod with Video consultation (Medvivo ™ Connect)
The HomePod service can optionally be extended by facilities providing secure consultation from the clinician to the HomePod using video services.
Video consultation assumes Wifi availability for the patients, as this will support better picture quality and reduce costs. 3G will also be provided as an option. Clinicians will be able to use a
normal desktop or laptop computer as the video consultation will be a flexible, incremental, web-enabled videoconferencing facility provided by Medvivo as part of the total solution. The system will only allow clinicians to access patients with whom they have a legitimate relationship.
Key features and benefits of Medvivo ™ Connect
Easy to use one button connection between clinician and patient
Reduce the need for home visits as initial visual assessment can be performed remotely
Improved quality of care through remote face-to-face interaction
Secure connection
No additional hardware required
Current Integrations with Principal Clinical Systems:
None currently
All PCS integrations will be supplied in future releases.Support Standard Service Performance & Support:
Our technology enabled care services are accredited to platinum level by the Telecare Services Association (TSA), which defines minimum standards for SLAs and response.
Service contact and support will be provided through the support desk.
Clients will be allocated a Medvivo Client Service Manager to provide additional service management support and escalation. Regular service reporting will be supplied by the Client Service Manager, which will include both the IT elements of the service and clinical elements.
Availability Standard: 99.5%
The HomePod posts its data to a central database but should this database not be available it retains the information and will post it with the next set of data. The availability standard reflects the availability of this central database (‘the CUI’).
Availability = (total service hours - unscheduled downtime hours) / (total service hours)
This is calculated per calendar month. Service hours are from 09:00 to 17:00 Mon to Fri excluding public holidays.
Incident Fix Times Standard: 2 days
Typical fixes requiring visits are to check or replace peripheral medical devices, or where the patient needs additional support in understanding or resolving an issue. We use our own highly-trained engineers, or that of our one partner subcontractor, to ensure the quality of the service, the capability of the staff and an awareness of safeguarding issues.
Helpdesk Standard: 09:00 to 17:00 Mon to Fri excluding public holidays
Most calls can be resolved over the telephone, or by gaining remote access to the HomePod to fix the incident. Remote access also allows remote training and advice to be provided when required.
Dependencies Infrastructure dependencies
Patients must have a mobile signal available from within their home OR broadband. Patients using Medvivo™ Connect for video contact are recommended to use broadband.
Clinicians will require access to the WebCUI, and optionally to Medvivo™ Connect for video contact with patients. WebCUI requires access to an N3 connection to attach to the application and database.
