TEAM ASSESSMENT FORMINSTURCTIONS: Before you begin your praxis journey, it is important that you first identify the tools that you can use to make your plans happen. This Team Assessment Form will help you do that. Follow the instructions per column and then fill out the table. Delete the sample item before you submit your accomplished form.

Johanna Alexis Donacao (Batch 10), UPD BA Linguistics(1) I am good at communicating to other people. I can easily express my thoughts to other people, especially to my groupmates. (2) I am kind of an extrovert person. Most of the time, I am not shy, especially to my co-participants. (3) I am an encourager. I can boost other people when they are down and tell them they can do it.(1) I am a person who is most of the time late at my appointments. (2) I must admit that I am already used to the culture of cramming and doing stuff just before the deadline.(3) Most of the time, I am busy and occupied with many things like academics and extra-curricular activities.(1) I value my relationship with God. I devote my time in serving Him, especially during Sundays.(2) I value my family and give time to them especially when there are important events and/or family emergencies.(3) I value education. I believe that we must not sacrifice education for any other less important things.

Jessa Mae M. Mangyao (Batch 10), PUP, BS Chemistry(1) I believe I am a good communicator. I share my ideas to the group and ask the opinions of my groupmates as well for the betterment of the decisions we are to make. (2) Another thing is my flexibility with people. Being able to get along with them becomes a key for them to open up themselves too.(3) Having an open mind helps a lot in group planning. (1) I am usually forgetful when I am pre-occupied with other tasks. And so they are times I don't get a thing done without someone reminding me to do so. (2) Thus, I also need to do better in managing my time.(3) So as setting my priorities.I value God, my family and my friends. I value communication and time spent with them.

Judy Ann Mison (Batch 10), UPD, BS Interior Design(1) I am dedicated to everything I do. (2) I do my best to try for alternative solutions when conflicts occur. (3) I work fast and I see to it that I am able to convey my ideas for the project.(1) I cannot manage stress and my time. (2) I have the tendency sacrifice the quality of something just to make it on time. (3) I am not reachable at certain times.I value relationship with my peers and family. I believe that relationship between these people shall not be compromised when doing the project. I also believe that one there will always be something to learn in new projects, whether they are good learning or mistakes. I give importance to how the society is affected and we can help them. I believe that a simple awareness of things will make a change in the society.

Patricia Angela Ocampo (BATCH 10), FEU Diliman, BS Accountancy(1) I persevere in everything I do.(2) People always tell me that I am resourceful and can always find alternatives.(3) I am skeptic in every details.(1) I easily feel pressured every time there are lots of work to do. I cannot focus on my current task because I am worried that there are more tasks awaiting that should be done. (2) I am not good in explaining things orally, I am more in writing than speaking.(3) There is always a doubt in everything I do. There is always a fear that what I did is not enough. I am not confident that what I did is good.I value all the things I have now. I believe that there are reasons why things were and were not given to me. I value God and all the instruments He has used as well as may family and friends.

April Grace H. Villar (Batch 10), PUP, BS Psychology(1) I can multitask. I see to it that I can do my job in school, in praxis and to the other organizations Im part of, and still do well.(2) Im usually sensitive to the needs of others. I always consult other people before making a decision.(3) Self-control. I can still focus on a task even under pressure or when conflict arises.(1) I tend to forget other tasks when I focus on a task Im currently doing.(2) I think Im not a very good communicator. Its hard for me to express my ideas to people I dont know well.(1) I value God, my family, friends, all the people that are important to me.(2) I value my relationship to them. Theyre my strength.

All of us in the group have had various ups and downs in our past praxis projects which had inspired us for this year's praxis project. Our goal holds us as one.Given that all of us in our group are Seniors, we rarely meet personally due to schedule conflicts and heavy loads due to thesis, research, etc.We value being in Pathways. This is why we chose to tell our Praxis experiences by Animation-AVP.

Despite the lack of time to meet personally, we usually do group chats to update one another.No expertise in video editing. But we wanted to show our experience in Pathways through animation/AVP, and were willing to explore and learn.We value our education and we value Pathways that is why we choose Senior Track. We wanted to share our insights and experiences in Pathways.

Come from different courses. We can offer different experiences/insights/concepts/ideas.This years praxis project is new to us, and we are adjusting on it.We value awareness. We value excellence. This is why we chose to make a video about leadership, and its something that we believe that we should share to other people regardless of their age, status, gender, etc.

We attended most of the camps.We value our health. So when one is sick, we understand that she could not attend our meetings.

Each one of us comes from different disciplines, thats why we have different inputs for the project. I can say that the team is creative. We are also good in tracking records and updating each other.We value God and our own beliefs. We respect each others beliefs and the time devoted for church.

I think our team is comfortable to work with each other because we have known each other quite well. We are open to suggestions or corrections of one another.
