• Health information exchange (HIE) is the transmission of healthcare-related data among organizations agencies according to national standards. HIE is an integral component of the health information technology infrastructure under development in the US and the associated National Health Information Network (NHIN).
• To meet requirements, HIE technology must enable reliable and secure transfer of data among diverse systems and also facilitate access and retrieval of data. The overall purpose of HIE is to improve healthcare delivery and information gathering.

HIE – OneHealthPort provides organizations a secure means to
share standardized messages with trading partners on a system-to-system basis. The HIE supports multiple formats, allowing a wide range of organizations to easily exchange both business and clinical information. IT is designated by the State of Washington as the statewide health information exchange.
– The eHealth Exchange is a group of federal agencies and non-federal organizations that came together under a common mission and purpose to improve patient care, streamline disability benefit claims, and improve public health reporting through secure, trusted, and interoperable health information exchange (includes VA and DOD).

• Direct email exchange works using a HISP (Health Internet Service Provider) to handle the email exchanges and carry out the encryption/decryption and digital signing of each message and attachment.
• Each sender and receiver must have a unique Direct address with the word “direct” in the address line, e.g. [email protected].

• Issue: Obtaining directories from other sites– Currently Epic shares Epic directories with other
Epic sites but this may not be true for all vendors.– Surescripts customers (often Epic customers) will
also share directories.– Other vendors may leave it up to their customers
to obtain/share.
• Issue: No consistent format– Each vendor has different required fields and
formats – Vendors may not have firm process for extracting &
importing directories

• OneHealthPort – May develop a directory (will not be available in near future).
• It is up to the individual organizations to contact other organizations and share.
• Hospitals may need to send SOC to nursing homes and other organizations that do not have an EMR that can accept the documents. May need to supply software/hardware.
