Introduction to the Secure Messaging Desktop

The Secure Messaging Desktop (SMD) enables you to print and send anything to anyone securely. SMD is a small application for Windows®-based computers. When you install SMD, a virtual printer called Secure Messaging PDF is created and added to your list of printers. This printer doesn’t print on paper; it prints PDF files which can be attached to a secure message and sent to anyone.

In addition to its unique print functions, the SMD also includes the functionality of Secure Messaging embedded in the application. This means that it has all the conventional functionality available in traditional web email applications. Messages can be sent and read. Files can be attached and saved. Unlike web email, the SMD enables you to track sent messages to see whether or not they have been read.

In addition, the software can process incoming messages like a secure incoming fax machine. Messages or attachments received in the SMD can be saved automatically to a pre-selected folder or printed to a pre-selected printer.

When Secure Messaging is embedded in an electronic medical record or practice management system, users can also use the SMD virtual printer. Items such as reports or summaries that are not EMR or PM documents can be printed to the Secure Messaging PDF printer. In fact, any application that can print can use the Secure Messaging PDF printer.

Overview You can install the <smd> in two ways: At the workstation, by downloading and running the file SMD.exe.

This is the method used when the owner of the workstation self-installs the <smd>. In some cases, administrators will also use this method.

The advantage of this method is that all prerequisites are included as part of the <smd> installation.

The disadvantage is that a person must proceed through each page of the installation wizard, which may be time consuming for an administrator who must perform installations at numerous workstations

By remotely logging in to the workstation, and using a command line to run the file sbc<version number such as 5.0.4.3>.msi.

This is a method often adopted by administrators. The advantage of this method is that an administrator can remotely access a

workstation and can run the installation from a command line, providing the necessary parameters at the outset and not having to view each page of the installation wizard.

The disadvantage is that all prerequisites must be in place before running the installation. This is because not all of the prerequisite installation parameters can be specified from a single command line.

The sections that follow describe the prerequisites and the two installation methods.

NOTE If you upgrade Secure Messaging from a version earlier than 4.11, you must also upgrade <smd>.

Automatic Installation of Prerequisites If you install the <smd> by running the SMD.exe file and going through the installation wizard, the application installs prerequisite software as needed. This is in contrast to installing the <smd> remotely using the SBC<version number>.msi file. In that latter method, an administrator must ensure that prerequisites are installed before the installation of the <smd>.

The SMD.exe installer ensures that the proper version of Internet Explorer (IE) is installed. Windows XP and Vista installations that already have IE 7 are left as is. If the browser version is earlier than IE 7, then the browser is upgraded to IE 8. Newer versions of the Windows OS are updated to IE 8 if necessary.

In addition, the installation adds the following: Updated .Net framework. Adobe Acrobat Reader 9 (if no Reader version is installed). Microsoft Windows Installer MSI (if it is not already installed).

By default, the installation program downloads Microsoft prerequisites from the Internet. This reduces network traffic between the Secure Messaging server and local workstations, which could be significant if numerous installations occurred at the same time. If network rules prohibit a workstation from accessing the internet, there is an alternative download method in which all prerequisites come from the Secure Messaging server. To use that method, open the Secure Messaging Patient Portal Configuration Utility on the Secure Messaging Web server and then open the Secure Messaging - Web Server page. On that page, select the Host Prerequisites Locally check box.

Install the Secure Messaging Desktop To install the Secure Messaging Desktop directly at a local workstation:

1 Open the Secure Messaging Desktop Download Center page at: https://<FQDN of Secure Messaging Web server>/sbcupdate/versioncheck.aspx

NOTE FQDN stands for fully qualified domain name. The FQDN might be something like secure.yourorganization.org.

2 Click Install this version.

3 A file download window appears. Save the file to a location of your choosing on your workstation.

4 Open the downloaded installation file and proceed through the installation wizard until you get to a Settings page.

5 On the Settings page, type the fully qualified domain name used by Secure Messaging (for example, secure.yourorganization.org).

6 Consider whether to select the Display Secure Messaging Desktop in stand-alone window (for Citrix configurations) check box. The check box label is misleading because this setting is not exclusive to Citrix

configurations. A label change is forthcoming in the next release. Selecting this check box causes the minimize button in the Secure Messaging Desktop

window to be disabled. The only reason to select the check box is if Windows is configured to prevent access

to the Windows notification area (often called the system tray) in the lower-right corner of the Windows Desktop. In this Windows configuration, a user should not have the option to minimize <smd> to an inaccessible place.

7 Click Next.

A Destination Folder page appears.

8 Confirm that the destination folder is correct, and then click Next through subsequent installation pages. Click Finish on the final page.

9 Look for a briefcase icon ( ) in the system tray. This icon indicates that the Secure Messaging Desktop is running.

If the Secure Messaging Desktop is not running, click Start > Programs > Secure Messaging Desktop > Secure Messaging Desktop to start it.

10 Repeat the installation for each workstation that needs the Secure Messaging Desktop.

NOTE Always return to the Secure Messaging Desktop Download Center by using the address listed above. The page links to the most recent update of the Secure Messaging Desktop, so if you forward or save the destination address you might download an out-of-date version.

Install Remotely to a Workstation Administrators may want to deploy the SMD without physically going to each workstation. Please note that with this type of installation, an administrator must ensure that the workstation has necessary prerequisites.

To remotely install the Secure Messaging Desktop to a workstation:

1 Establish a remote connection to the workstation that needs the Secure Messaging Desktop.

2 Open the Secure Messaging Desktop Download Center page at: https://<FQDN used by Secure Messaging>/sbcupdate/versioncheck.aspx

NOTE FQDN stands for fully qualified domain name. The FQDN might be something like secure.yourorganization.org.

3 Write down the SMD version number listed on this page.

4 Modify the URL in your browser to the following: https://<FQDN used by Secure Messaging>/sbcupdate/sbc<version number such as 5.0.4.3>.msi

5 A file download window appears. Save the file to the desktop of the workstation.

6 Open a command prompt, and then change the directory to the Windows desktop.

7 Run one of the following commands: If you want the installation to show a "success" window when it is complete, type: msiexec.exe /i SMD.msi /qn+ If you do not want the installation to show a "success" window when it is complete,

type: msiexec.exe /i SMD.msi /qn

About Secure Messaging Desktop The Secure Messaging Desktop provides another way to access Secure Messaging. All of the regular functionality of Secure Messaging is available when you log in to the Secure Messaging Desktop, and it notifies you when you receive a new secure message.

If you are using Secure Messaging embedded within your EMR system, you can print to the secure "briefcase" while using the EMR without logging in separately to the Secure Messaging Desktop.

The "briefcase" is a special folder that stores all of the documents that are "printed" by the Secure Messaging Desktop so that you can attach them to secure messages.

NOTE When we say "print" to the briefcase, we use that term because the data is sent through a printer driver. However, you may be more comfortable thinking of it as "saving" a PDF to the briefcase.

You can also print (save) files to the secure briefcase from any application that includes a print function. In order to use the printer driver to print files to the secure briefcase from any application other than your EMR system, the Secure Messaging Desktop application must be running.

The files that you print to the secure briefcase can be accessed from the Manage Briefcase tab of the Web-based message center or from the same tab of the Secure Messaging Desktop.

There are several ways to send secure messages with a file or multiple files attached: Print any file to your briefcase for later use. Print a file and automatically open a new message with the file attached. Print a file and automatically save a new message to the Drafts folder, so you can send

it later.

The Secure Messaging Desktop has two Help files: This Help file (displayed when you click Help > Help Topics in the menu bar) includes

information about Secure Messaging Desktop settings, how to print to the secure briefcase, and troubleshooting problems with the printer driver. In the upper-right corner of the Secure Messaging Desktop window, click the question

mark icon ( ) to display Secure Messaging Help, which describes all the features and functions Secure Messaging.

Print a File to the Secure Briefcase Printing a file to the secure briefcase saves the file in the briefcase folder so that you can send it in a secure message at a later time. We use the term "printing" because a PDF file is created and stored using a special PDF printer driver. However, you could think of it as "saving" the file to the briefcase in PDF format.

To print a file to the secure briefcase:

1 Start the Secure Messaging Desktop application and log in, using your secure messaging user ID and password.

2 Open a file that you want to send securely.

3 Click File > Print.

The Print window appears.

4 In the Print window, make sure the Secure Messaging PDF printer is selected in the Name field, then click OK.

After a slight pause, the Select Your Action window appears.

NOTE If you are attaching a Medical Manager file, select the Secure Messaging Text printer driver instead.

5 Select Store file for later attachment in the Message Center, then click OK. The file is printed and stored in your briefcase.

If you want to rename the file, in the Select Your Action window, click Rename, and then type a new file name.

a You can also select one of the action options to automatically attach the resulting .pdf file to a secure message or automatically create a new draft message and save it with the file attached.

b If you think that you will always choose this action, select the Save this as my default and do not prompt me again check box.

c Click OK.

The file is added to the list of files displayed on the Manage Briefcase tab.

NOTE If the Manage Briefcase tab is displayed when you print a new file to the Secure Messaging Desktop, click Refresh to display the updated list.

Add an Existing File to the Briefcase To add an existing file to the secure briefcase:

1 Right-click the SMD icon ( ) in the system tray and click Attach or, in the menu bar, click File > Attach.

A browser window appears.

2 Browse to the file that you want to add to the briefcase.

3 Click Open.

The file is saved in the briefcase.

Attachment Viewer The attachment viewer lets you quickly view several types of attached documents so you can review the information in the file within Secure Messaging Desktop before you send it. Patients and other clinics can also view the attachment in Secure Messaging without having to open it in another application.

The attachment viewer can display the following types of files: CCD (XML-based Continuity of Care Document) CCR GIF

The attachment viewer appears during several different workflows within Secure Messaging Desktop, including the following: Attaching a file, CCD, or CCR from your briefcase or stored locally. Viewing a file, CCD, or CCR in your briefcase.

Attach and View a CCD Secure Messaging Desktop lets you attach a CCD in XML format and preview it before sending. For example, you may want to quickly review patient details before you send it. Before you can attach and view a CCD in Secure Messaging Desktop, you must already have the file located in your Briefcase or somewhere you can navigate to on your computer or network.

To attach and view a CCD stored in your briefcase:

1 In the Secure Messaging Desktop click New Message, then from the message toolbar, click Attach > From Briefcase.

2 In the Briefcase Attach window, select the checkbox for the CCD, which appears as an .xml file.

3 If you want to view the CCD before you send it, click View, and the CCD appears in the attachment viewer.

4 Click the Close button in the upper-right corner of the viewer to close the CCD.

5 At the bottom of the Briefcase Attach window, click OK, and the CCD is attached to your secure message.

To attach and view a CCD stored as a file on your computer or network:

1 In the Secure Messaging Desktop click New Message, then from the message toolbar, click Attach > From file system.

The Add Attachment window appears.

2 Click Browse, and the Choose File to Upload window appears.

3 Navigate to the file you want to attach, then select it and click Open.

4 At the bottom of the Add Attachment window, click OK. The CCD is attached to your secure message.

If you want to attach another file, click OK and Attach More, and repeat steps 2–3.

5 If you want to view the CCD before you send it, click View, and the CCD appears in the attachment viewer.

6 Click the Close button in the upper-right corner of the viewer to close the CCD.

Attach a Secure Briefcase File to a Message To attach a file in the secure briefcase to a message:

1 Click New Message.

2 Click Attach > From Briefcase.

The Briefcase Attach window appears.

3 Select the file or files that you want to attach.

4 Click OK.

5 The attachment appears above the formatting toolbar, along with commands to remove or view the file.

6 If you want to check the contents of the file before you send it, click View to display the

attached file in the viewer.

7 When you are done, click Close in the upper-right corner of the viewer.

8 When the message is ready, click Send.

Save All Attachments as a Single PDF You can set the Auto save function to save all attachments in a message as a single PDF file, reducing the number of files to maintain. When you enable this feature, it collapses all attachments into a PDF for all messages you receive.

You can also specify whether to save the message content only, attachments only, or both messages and content.

To enable single PDF file attachments:

1 In Secure Messaging Desktop, click Edit > Settings.

The Settings window appears.

2 Click the Automatic Message Processing tab.

3 Ensure that the Automatically process inbound secure messages checkbox is enabled.

4 Click the Auto save option, and several Auto save fields appear.

5 In the Save Option area, enable the Combine all items into a single PDF file checkbox.

6 If necessary, click the Save to browse button (...), and select the directory to which you want to save the attachments.

7 Click OK.

NOTE Starting in version 4.11, the Automatic Message Processing tab is hidden by default.

Save and Rename Attachments Secure Messaging Desktop now provides better controls for saving attachments as files.

The updated Save to File window now displays a checkbox for each attachment in the secure message. You can check the Item checkbox to save all the attachments, or you can select individual attachments to save as files.

In addition, you can now rename selected attachments all at once, with a common serialized file name. For example, all files could be renamed to WalterCaldwell.rtf, WalterCaldwell2.rtf, WalterCaldwell3.txt, and so on. File names are serialized, regardless of the file type or extension.

You can click an attachment name in the window to open the attachment in a separate application.

To rename files you are saving:

1 In Secure Messaging Desktop, open a secure message with several attachments.

2 In the toolbar, click Action > Save To File.

The Save to File window appears.

You can click any attachment to view it in another application.

3 Enable the Item checkbox to select all attachments, or select the checkboxes for the individual attachments you want to rename and save.

4 Click Rename multiple, and a text field appears.

5 Type the name you want to use for the saved files, such as the patient name, or other identifying title. For example, type WalterCaldwell.

6 Click Apply, and the files are renamed. The new name appears in the Rename column.

7 When the files are named to your preferred names, click OK.

The files are saved with the new names.

Open, Rename, and Delete Files In the left menu pane, under User, click Manage Briefcase. This window lets you download, view, rename, and delete files that have been printed to the secure briefcase. To rename a file, in the Action column for the file, click Rename. In the window that

appears, type a new file name, and then click OK. To delete one or more files, select the check box to the left of the files to be deleted,

and then click Delete. When a prompt appears to confirm the deletion, click OK. To view the contents of a file, click View. The file contents appear in the attachment

viewer. Click Close when you are finished viewing the file. To download a file, click Download. The File Download window appears. Click Open

to view the file in an appropriate application. If you click Open for an XML file, a window appears and displays the XML and HTML versions of the file. Click

If you do not see a file that you expect to be in the briefcase, click Refresh to refresh the list of displayed items.

NOTE To automatically delete files after they are attached to a secure message, on the Settings tab, click Preferences, and select the Remove items from briefcase after attaching check box.

Set Secure Messaging Desktop Settings The Secure Messaging Desktop Settings window enables you to change default Desktop settings. There are two ways to open the Settings window: If the Secure Messaging Desktop is open, click Edit > Settings. If the Secure Messaging Desktop icon ( ) is in the Windows system tray, right-click

the icon and then click Settings.

NOTE The Settings command in the left pane, under User, allows you to modify your message center settings, and is described in the message center Help. You can view message center Help by clicking the Help icon ( ) in the upper-right corner of SMD.

Changes that you make on any of these tabs are saved when you close the window by clicking OK.

NOTE Starting in version 4.11, the Automatic Message Processing tab is hidden by default. If you previously enabled Automatic Message Processing features such as Auto Print or Auto Save, when you upgrade to version 4.11, the Automatic Message Processing tab remains visible, but the fields are disabled. To re-enable the features, check the Automatically process inbound secure messages field at the top of the tab, and the features in the tab are activated. If you did not previously use Automatic Message Processing, and you need to use this feature, contact Customer Support for information on how to re-enable the tab.

General Tab On the General tab of the Settings window, you can: Identify the message center web server linked with the Secure Messaging Desktop.

Before changing the value in this field, contact your Network Administrator.

To change this field, click the Edit button, and then type the fully qualified domain name of the message center web server. Select an attachment option.

In the Attachment options area, select the action that should occur when you print to the secure briefcase.

Prompt user to choose an action – A prompt window is displayed so that you can rename the file if necessary, and then either attach the file to a secure message or save the file so that it can be attached later.

Automatically create new message with file attached – You are not prompted, and the file is attached to a newly created secure message.

Store file for later attachment in the Message Center – You are not prompted, and the file is automatically saved in the temporary storage location that is displayed in the Temporary storage location for printed files area. (You cannot change the storage location.)

Change the amount of time that the Desktop can remain idle before you are logged out. To change the period of inactive time that must pass before an automatic logout occurs, type a number of minutes in the field. Specify whether you want to see notification popup messages.

By default, Secure Messaging Desktop displays notification messages from the Secure Messaging Desktop's briefcase icon in the system tray. To disable these messages, select the Disable popup messages check box.

Automatic Message Processing Tab If you want the Secure Messaging Desktop to process incoming messages automatically, enable the feature by selecting the Automatically process inbound secure messages check box on the Automatic Message Processing tab of the Settings window. Then, with the Auto print or Auto save option selected, choose setting options to indicate how your messages should be processed.

NOTE Starting in version 4.11, the Automatic Message Processing tab is hidden by default. If you previously enabled Automatic Message Processing features such as Auto Print or Auto Save, when you upgrade to version 4.11, the Automatic Message Processing tab remains visible, but the fields are disabled. To re-enable the features, check the Automatically process inbound secure messages field at the top of the tab, and the features in the tab are activated. If you did not previously use Automatic Message Processing, and you need to use this feature, contact Customer Support for information on how to re-enable the tab.

Auto print options

When Secure Messaging Desktop prints automatically, it receives the secure message, and downloads the message and attachments to a folder, then converts the items to a PDF before it sends the PDF to the printer. Message – Select this option to automatically print the message when it is received. Attachments – Select this option to automatically print attachments when you receive

a secure message with attachments. Messages and attachments – Select this option to automatically print the message and

any attachments when you receive a secure message. Printer – Choose the printer to which you want to automatically print the messages and

attachments. Required temporary folder – Set the location for the temporary folder to which

messages and attachments are saved before being printed.

Auto save options Message – Select this option to automatically print the message when it is received. Attachments – Select this option to automatically print attachments when you receive

a secure message with attachments. Messages and attachments – Select this option to automatically print the message and

any attachments when you receive a secure message. Combine all items into a single PDF – Specifies whether saved files are saved as

individual files in their original format, or saved combined into one PDF file. Save to – Type a full path or browse to the directory where messages are sent for

automatic processing. Required temporary folder – Set the location for the temporary folder to which

messages and attachments are saved before being combined into a single PDF.

Misc Tab The Misc tab contains options for specifying printer driver settings and scheduling software updates.

The SMD Text printer driver settings can be changed if you installed the Secure Messaging Desktop Text printer driver. If that is the printer driver you are using, select the Automatically name files check box if you want the Desktop to use default file names that are differentiated by a prefix that you type in the Next prefix field.

In the Software updates area, select a value from the drop-down list for the Check for updates field to indicate whether you want the Desktop to automatically check for updates at a specified interval. The options are Daily, Weekly, Monthly, and Never. If you select any option other than Never, the Secure Messaging Desktop checks for updates on the specified day. If it detects a new update, it displays a pop-up message to inform you that the update is available, and the message New updates are available appears on this tab.

The date and time when the Secure Messaging Desktop last checked for updates is displayed to the right of Last checked. If you save a new value in the Check for updates field, the appropriate date is displayed to the right of Next check the next time you display this tab.

If your message center uses a proxy server, it is identified at the bottom of this tab; otherwise, the tab displays the information Proxy: none.

To automatically log in to the Secure Messaging Desktop from the EMR's message center using the EMR secure messaging User ID, select the Enable auto login check box. For this setting to take effect, exit and restart the Secure Messaging Desktop.

WARNING The Auto Login feature that enables Secure Messaging Desktop to integrate with the EMR can open a possible security risk. To protect against this possible threat, we provide a way to disable this feature. When Secure Messaging Desktop version 4.11 (or later) is installed, if the Enable Secure Messaging Desktop Auto Login field is unchecked, then Auto Login is disabled at the web server, and the option is removed from the Misc tab.

To re-enable Auto Login:

1 In the Kryptiq Configuration tool, navigate to Secure Messaging > SM Web Server.

2 Check the Enable Secure Messaging Desktop Auto Login option.

3 Click Apply.

4 Restart Secure Messaging Desktop.

5 Navigate to Edit > Settings, then click the Misc tab.

The Auto Login checkbox is now visible.

6 Enable the Auto Login checkbox then click OK.

7 Restart Secure Messaging Desktop, and Auto Login is enabled.

Use the Right-Click Menu When the Secure Messaging Desktop's briefcase icon is displayed in the system tray, you can right-click it to perform various actions. You can also right-click in the gray menu bar to access the same commands: Logoff – Log off without exiting from the Secure Messaging Desktop so that you can

log in with a different user ID and password. You can also click User > Logoff. Inbox – Display the messages in your message center Inbox. Attach – Displays a browser window from which you can select a file to be added to

the secure briefcase. Browse to the file that you want to attach to a secure message and click Open. You can also click File > Attach in the menu bar. Settings – Display the Settings window. You can also click Edit > Settings in the

Secure Messaging Desktop menu bar. Help Topics – Display this Help system. You can also click Help > Help Topics in the

Secure Messaging Desktop menu bar. About – Display an informational window that includes the Secure Messaging Desktop

version number. You can also click Help > About in the Secure Messaging Desktop menu bar. Exit – Close the Secure Messaging Desktop window. You can also click File > Exit in

NOTE This topic applies only if you installed the Secure Messaging Text printer driver.

If you use the Secure Messaging Text printer driver for a nontext file such as .pdf, .rtf, or .doc file, the output becomes garbled.

Use the Secure Messaging Text printer driver for text-only applications. Use the Secure Messaging PDF printer driver for all other files.

Secure Messaging Desktop Does Not Launch From the Windows Program Menu

If you have the Secure Messaging Desktop running in your Windows system tray, the utility does not open when you attempt to start the program from the Windows Start menu.

Open the application by clicking the Secure Messaging Desktop icon ( ) in the system tray.

NOTE You may need to click the Show hidden icons button in the system tray to see the Secure Messaging Desktop icon.

Secure Briefcase Prints to a Different User NOTE This topic applies only to Centricity Physician Office and Centricity Practice

Depending on whether you log in to Centricity or the Secure Messaging Desktop first, you might be logged in as two separate users. The following rules apply: If you log in to Centricity and print to the secure briefcase, the Secure Messaging

Desktop automatically logs you in, based on the information you used to log in to Centricity. If you log in to Secure Messaging Desktop as one user, log in to Centricity as a second

user, and then print to the secure briefcase, the file is placed in the secure briefcase for the first user.

Launching SMD in a Citrix XenApp Session

Some providers may use Citrix XenApp software to run EMR and other software using a computer terminal or thin client, rather than a desktop PC. Use the information below if you want to launch SMD together with CPS or EMR in a Citrix XenApp session.

Summary The goal is to launch the Secure Messaging Desktop (SMD) client after the EMR client within the same Citrix session.

The EMR client and SMD client need to run on the same local profile on the Citrix server, so that SMD can process the files that the Secure PDF Printer sends to the user’s local temp directory.

There are two known methods to configure SMD to launch in the same Citrix session as the EMR. Batch file Session sharing

Launching with a Batch File You can use a batch file to launch SMD and the Centricity client in the same Citrix Server session. If this method does not work for your environment, you may consider using the Session sharing method.

The following batch file is an example of what typically works well for in-house testing as well as other customers:

NOTE: You can add /min to the SMD line to launch SMD as a minimized application. The only adjustment you may need to make is to adjust the paths in the batch file to match your environment. This line may vary for Centricity Practice Solution (CPS) or EMR and will likely vary based on you version of Centricity as well.

Launching with Session Sharing Session sharing provides a method for launching SMD and your Centricity product together, but it must be carefully configured, according to the following: All Citrix servers must be configured exactly the same:

The same versions of CPS, EMR, and SMD must be on all servers. This means all must be at the same patch levels.

Session sharing will work for SMD if there is an adequate number of Citrix servers, and if the load evaluator is configured properly. Load balancing takes priority over session sharing on a full server unless load

balancing is disabled (disabling load checking is often not a desirable configuration). If one or more Citrix servers goes down and increases the load on the remaining,

the load balancer may put users on different Citrix servers, breaking SMD. During peak periods, Citrix servers may become full, depending on available Citrix

server resources.

NOTE: Consult Citrix for more information on setting up session sharing on your servers.
