ADDENDUM NO. 5 CONTRACT 587-15-C-13 (81) LOWER HOWARDS CREEK

WATER TREATMENT PLANT WINCHESTER MUNICIPAL UTILITIES

CLARK COUNTY, KENTUCKY

October 24, 2017

The attention of contractors bidding the titled contract is called to the following additions, substitutions, or deletions to the Drawings and /or Specifications.

A. SPECIFICATIONS 1. Section 01020 – Allowances

a. In section 1.01 on page 01020-1, add an article B which reads “The CONTRACTOR shall include in the base bid lump sum bid price the stipulated sum of $20,000 for top soil purchase should there not be enough top soil located on site.”

2. Section 02610 – Pressure Pipe

a. In article 2.01.B.3.b(3)(a) replace “membrane building” with “filter

building and high service pump station”. 3. Section 07261 – Self Adhering Air and Vapor Barrier

a. In article 1.01.A on page 07261-1, add a sentence which reads “The specified self-adhering air and vapor barrier shall be used at door and window openings, other openings, and any other place not covered by taped plastic foam insulation.”

4. Section 07262 – Liquid Applied Air/Vapor Barrier System

a. In article 1.01.A.1 on page 07262-1, add a sentence which reads “The specified self-adhering air and vapor barrier shall be used at door and window openings, other openings, and any other place not covered by taped plastic foam insulation.”

5. Section 09265 – Gypsum Board Assemblies on Wood Framing

a. This section applies to assemblies on metal framing too. 6. Section 09900 – Painting

a. In article 3.08.A on page 09900-34, change the following sign numbers and placements:

Danger – High Voltage: From 2 to 3 – At the electrical room in the

Chemical Building, the High Service Pump Station and the Filter building.

Confined Space – Entry By Permit Only: From 1 to 11; At each hatch into the high service pump station, the clearwell, the sludge pump station and valve vault, the decant pump station and valve vault, the high service meter vault, and the raw water meter and flow control vault. Fire extinguisher: from 12 to 26. Danger – Plant Service Water – Do Not Drink: From 5 to 14 Authorized Personnel Only: From 1 to 2, at the lab and the bacti lab Class 2 Oxidizer: One at each entrance to the potassium permanganate room. Danger Water Reactive Chemical: From 2 to 0

7. Section 11214 – Vertical Turbine Water Pump Equipment

a. In article 2.01.C on page 11214-3, add the following text: “e. Hollow shafts will be accepted provided they are designed adequately to support the load stresses to be encountered.”

8. Section 12345 – Inset Steel Laboratory Casework

a. In Part 2 on page 12345-19 and an article 2.09 Fume Hood which says:

“A. A fume hood shall be provided as shown on the drawings. The fume hood shall be a rigid self-supporting unit fully factory assembled without the need for field fabrication. The fume hood shall be factory wired and plumbed. The unit shall be Mott Manufacturing’s Pro, Bench style fume hood or equal.

B. The fume hood shall be designed to meet or exceed the

American National Standard for laboratory Ventilation and the American Industrial Hygiene Association Standard as outlined in ANSI/AIHI Z9.5-2012 and have its performance verified according to ANSI/ASHREA 110. The fume hood shall be manufactured in compliance with the latest edition of SEFA. The fume hood shall be UL 1805 classified.

C. The fume hood shall be a constant air volume or a variable

air volume unit as appropriate for labs in water treatment plants. The fume hood shall have a nominal face velocity of 80 to 100 ft/min. with a fully open sash.

D. The fume hood shall have double wall construction to

accommodate plumbing and electrical wiring. The fume hood shall be constructed of galvanized steel with a thermosetting laboratory grade powder coating. The unit shall have either a fiberglass reinforced polyester, PVC or stainless steel liner as appropriate for the chemicals and tests performed in a water treatment plant lab. The sash shall have laminated safety glass with a stainless steel aircraft quality cable system. The electrical system shall have rapid start LED lighting, a light switch and two 120 volt duplex receptacles mounted on corner posts. The fume hood shall have an alarm to indicate when the air velocity drops below a safe value. The alarm shall be both visual and audible. The fume hood shall include 19 inch high furring panels as necessary, a plastic sash stop, and a gooseneck faucet with 3 inch by 6 inch cup sink. “

9. Section 15100 – Small Plumbing Valves, Plumbing Specialties, and

Service Accessories

a. In article 2.01.E on page 15100-3, change Senate Law 5.3874 to read Senate Law S.3874.

b. On page 15100-16, add a paragraph 2.15 Water Meter and Service Accessories and the following:

2.15 WATER METER AND SERVICE ACCESSORIES

A. Water Meters

1. General

a. Where OWNER has standardized on one particular

make and model meter, and desires that they be

furnished on this project, then such standard, make

and model, namely Neptune, will govern.

B. Water Service Accessories

1. Water Meters

a. The potable water flow shall be monitored by a disc

or piston type positive displacement meter with

touch read lids. The flow shall be totalized at the

meter body.

b. Characteristics are as follows:

Size: 5/8 inch

Flow Range: 3/4"-2 to 30 gpm

Maximum Pressure Loss: 5/8"-10.8 psi @ 20 gpm

Register Type: Straight reading, hermetically sealed,

magnetic drive

Registration: 5/8"-10 gal. or 1 cu. ft.

Capacity: 5/8"-1,000,000j gal. or 100,000 cu. ft.

Max. Oper. Pressure: 150 psi

Connection: As shown on the Drawings

Maincase: Bronze

Measuring Chamber: Bronze

Piston: Hard rubber

Strainer: Stainless steel or copper

Trim/Bolts: Stainless steel

Accuracy: Within 1.5 percent of through-put

Max. Cont. Flow Rate: 5/8"-10 gpm

c. The OWNER has standardized on one particular make

and model of water meter and desires that the

meters furnished under this project be such

standard. That make and model water meter, namely

Neptune Radio Read, 5/8-inch x ¾-inch T-10 ED2B31

RWG1SG59, Register E-Coder, R900i outside,

standard register gallons, will govern.

2. Meter Coppersetters

a. Meter setters shall be iron meter yoke, riser type,

vertical inlet and outlet for polyethylene pipe. They

shall be 5/8" x 3/4" Ford Model No. TVBHC72-9W-66-

33G. A master lock No. 1, Model #2055 shall be

provided with each setter. The key shall match

OWNER’s existing key.

3. Meter Box and Cover/Lid

a. Meter box shall be a polyethylene box Model

DFW37C-14 (top) and DFW37C-18 (bottom) by DFW

Plastics.

b. The meter box cover/lid shall be a predrilled Model

DFW37C-1BA-Lid w/AMR Hole by DFW Plastics.

4. Pressure reducing valves shall be Wilkins 600 LUSC.

10. Section 16440 – Motor Control

a. In article 2.02.C.1.a on page 16440-9, add the following text: “IEC-rated starters/contactors will only be accepted for use with soft-starters (RVSS) or non-motor loads such as capacitor banks or resistive loads. NEMA starters/contactors shall be provided for FVNR motor loads.”

11. Section 16444 – Solid State Reduced Voltage Motor Control

a. In article 2.03.A on page 16444-2, change maximum ambient from 50°C to 40°C.

12. Section 16483 – Adjustable Frequency Drives

a.. In article 2.02.A.10.h on page 16483-3, change maximum ambient from 50°C to 40°C.

b. On page 16483-8, delete paragraph 3.01.A.2 and 3.01.A.3. A burn-in

test will not be required. c. In article 2.02.A.8 on page 16483-3, add the following text: “A DC

link choke that provides equivalent harmonic reduction will be accepted in lieu of the line reactor.”

13. Section 16496 – Automatic Transfer Switch

a. On page 16496-4, delete paragraph 2.02-I. A 4th pole in the ATS will

not be required. 14. Section 16711 – Network Switching/Routing

a. In article 1.09.A on page 16711-2, delete the requirement for a spare 10-port fiber switch.

b. In article 2.01.A on page 16711-2, add “Sophos” as an acceptable

manufacturer. c. In article 2.02.A on page 16711-3, change “10/100” to

“10/100/1000” d. In article 2.02.E on page 16711-3, change “SF300-24PP” to “2960-

XR” e. In article 2.03 on page 16711-3, delete this paragraph. A fiber-only

switch will not be required. f. In article 2.04.A on page 16711-3, add “Sophos” as an acceptable

alternate to the Cisco router/firewall. g. In article 2.05.A on page 16711-3, change “Cisco 500 series” to

“Sophos AP55/AP55c” and add the requirement: “IEEE 802.11ac shall also be supported.”

15. Section 17100 – Process Instrumentation

a. In article 2.02.I.1 on page 17100-7, add the following text: “The Hach Model TU5300 turbidimeter will be accepted as an alternate.”

16. Section 17250 – SCADA Computer Hardware

a. In article 2.02.C on page 17250-3, add the following text: “11. Rack-mount UPS systems are required to be supplied for IDF-Admin, IDF-FB, IDF-HS, and IDF-CHEM (quantity 4).”

B. DRAWINGS

1. Drawing Sheet C003 – Site Yard Piping Area 2

a. Where 3 inch ductile iron pipe is called for 4 inch ductile iron pipe

may also be used. b. Where 2 inch ductile iron pipe is shown but unavailable, the pipe

shall be copper as noted in the general piping notes or it can be welded schedule 10S/schedule 40S stainless steel as noted in the general piping notes.

2. Drawing Sheet C004 – Site Yard Piping Area 3

a. Where 2 inch ductile iron pipe is shown but unavailable, the pipe shall be copper as noted in the general piping notes or it can be welded schedule 10S/schedule 40S stainless steel as noted in the general piping notes.

3. Drawing Sheet C023 – Various Waterline Changes

a. Add the following note to the sheet: “This work needs to be completed prior to the new water treatment plant being placed in service. The existing water treatment plant can operate off of lake water while this work is being completed. The existing plant can operate off the lake for approximately one month. The Contractor will need to coordinate with WMU for timing of work. Approximately 300 LF of the 24” pipe will need to be drained and approximately 5,000 LF of the 18” pipe will be drained to isolate the two pipes.”

b. See Attachments 1 and 2 for clarified drawings of the existing and

new raw water/finished water line tie-ins. 4. Drawing Sheet C701 – Chemical Feed Building, Section A

a. The pipe trench is being made deeper as shown in Attachment 3.

5. Drawing Sheet S203 – Filter Building Roof Plan

a. The notation “REV” on a section or detail flag is used to indicate that the referenced section or detail is in the reverse orientation. An example is 3 REV/S205. This calls Detail 3 on Sheet S205 and notes that it is depicted in the reversed, or mirrored orientation.

6. Drawing Sheet S400 – Equalization Basin, Structural Plan, Sections and Details

a. See the changes to the basin as shown in the partial plan,

Attachment 4. b. See Attachment 5 for changes related to the grout slope being

added to the effluent trough. 7. Drawing Sheet S704 – Chemical Feed Building Structural Details

a. See Attachment 6 for changes related to the increased depth of the pipe trench.

8. Drawing Sheet A008 – General – Finish Schedule & Details

a. In the finish schedule where it says VCT add a note which says to “Match existing”.

9. Drawing Sheet M003 – Mechanical Equipment Schedules

a. In the Unit Heater Equipment schedule, Unit heater UH-3 shall be model HLA. Delete the reference to model 5100.

10. Drawing Sheet E004 – Electrical Details

a. Add the detail in Attachment 7, drawing RE-12. b. Add the detail in Attachment 8, drawing RE-13.

11. Drawing Sheet E005 – Electrical Site Plan

a. Modify per the Attachment 9, drawing RE-14.

12. Drawing Sheet E006 – Electrical One-Line Diagram

a. Modify the generator feeder and ATS as indicated in Attachment 10, revision drawing RE-11.

b. Add the following general note: “If necessary due to space

limitations, the PFCC capacitors and contactors in MCC-HS1 and MCC-HS2 will be allowed to be supplied in a separate MCC unit that will be mounted against the wall. However Contractor must

include in the bid all interconnecting conduit/wiring and installation necessary to accomplish this.”

13. Drawing Sheet E700 – Power Plan

a. Add a 20A 120V circuit (2#12, 1#12G, ¾”C) to each of (4) overhead

door actuators from Panel LP-Chem2. The overhead doors that will have actuators are doors 700B (corridor), 705A (carbon), 701B (Fluoride), and 702B (polyphosphate.) Provide installation of three-button control station and safety sensors for each door. Cabling shall be installed in conduit except for the last whip at the safety sensors.

14. Drawing Sheet I003 – SCADA & I.T. Network Block Diagram

a. Note that the fiber switch and copper switch in IDF-Admin will be

allowed to be combined into the same ethernet switch using the Cisco 2960-XR model.

15. Drawing Sheet WL001 – Lettie Lane Water Line

a. HDPE pipe shall be DR9 (250 psi) ductile iron pipe size (DIPS) PE

4710. b. Service meter shall be 3/4 inch Neptune per SDW-24 on plan sheet

C904 and Form of Proposal, Section 2, Item No. 7. Other appurtenances shall be per attached water meter and service accessories specification.

16. Drawing Sheet WL001 – Lettie Lane Water Line and Sheet WL002 –

Lisletown Road Water Line a. Ductile iron pipe shall be pressure class 350.

b. Thrust restraint is required for all piping as shown in the details and as specified.

17. Drawing Sheet WL003 – Water Line Details

a. Flushing hydrant assembly shall be per SDW-11.
