STEP 2: Completing the K-12 Form IS NOW ELECTRONIC! & Electronically forward it to Ms. Morales from the Fill out
You are required to complete and turn in a new K12 form every semester/term. This form must be signed and
approved by you (the student); your parent/legal guardian; and your counselor for con-current enrollment or Ms.

NEW - Dynamic Forms - complete this form every semester. The form must list the class that you will be taking during that semester. Complete all questions on Part I: K-12 Student Information. Make sure to put down your LACC Student ID Number (not your High School ID). Your parent or legal guardian will need to create an account with Dynamic Forms to approve and electronically sign your form. Once the form is approved by your parent, your School Counselor / Official will receive an email for approval. Please watch one of these videos.

STEP 3: AFTER YOU APPLY & Received your LACC Student ID number - Access your Student Portal and Canvas Before your class starts, log into your Student Portal. From there you can check:
Your LACC e-mail account (your professors’ and other communications are sent to this address)
Your Canvas account. You can find the classes you are registered for, attend your Zoom meetings, submit assignments, participate in your class, and contact your professor. Think of it as your Schoology to attend, participate, and complete the class.

Complete College Search The basic college search tool is found by going to Colleges > Find My Fit >
Advanced College Search. Enter any college preferences you have currently, then review the list of colleges that meet those preferences. When you find a college you are interested in, click the name of the college to visit the college profile. Once on the college profile, you can click the heart icon to Add to
Colleges I’m Thinking About.
Complete Career Cluster Finder The Career Cluster Finder is found by going to Careers > Explore Careers and Interests > Career Cluster Finder. Read the introduction then click Start to
begin the assessment. Make selections and continue to click Next to complete the assessment. The Career Cluster Finder results will include your top three career clusters and a ranking of all the career clusters with a percentage match for each.
Explore the Roadtrip Nation Interview Archive The Roadtrip Nation Interview Archive is found by going to Careers > Roadtrip Nation Interview Archive. Using the database of over 5,000 interviews with various leaders, you can learn about career pathways of interest. Once you’ve
watched a video, navigate to My Planner > Tasks > Explore the Roadtrip Nation Interview Archive. Click add a comment to answer the reflection question.
Complete the Learning Style Inventory Learning Styles Inventory is found by going to About Me > Learning Style Inventory. The Learning Style Inventory assessment allows you to boost academic potential by recognizing their natural learning style, discover better learning strategies, and gain career development skills. Click Take

The StrengthsExplorer® results will display your top three strengths and related careers pathways that correlate with your strengths. Research the career pathways, and when you find one you like, add the pathway to your favorites list.

Complete a SuperMatch® College Search The SuperMatch® college search tool is found in Colleges > Find Your Fit
> SuperMatch. Select Fit criteria to find colleges or universities that are a good fit and see how closely they match your criteria. When you find a college you are interested in, click Favorite to Add to your Colleges I’m Thinking About list.
Add At Least One College to My List As you complete your college research and find schools of interest, you can add them to the Colleges I’m Thinking About list in Naviance Student. You must add 2 colleges to your favorites list for this task to be marked complete.
Build a Resume Understand how to market your skills, strengths, and experiences by creating a resume. To find the Resume Builder go to About Me > My Stuff > Resume. You can continue to return to the resume builder to add or edit entries. In order to mark the task complete, you must create a printer-friendly version of your resume.
Complete the Do What You Are® 2.0 Assessment The Do What You Are® 2.0 Assessment is if found my going to About Me > My
Assessments > Do What You Are. Read the introduction and click Take Assessment. Select Start then answer all questions to complete the assessment. The Do What You Are® assessment uses personality types to help you identify
your natural tendencies and will suggest careers and clusters you might be a good match for.

Complete a SuperMatch® College Search The SuperMatch® college search tool is found in Colleges > Find Your Fit > SuperMatch. Select Fit criteria to find colleges or universities that are a good fit
and see how closely they match your criteria. When you find a college you are interested in, click Favorite to Add to your Colleges I’m Thinking About list.
Add At Least One College to My List As you complete your college research and find schools of interest, you can add them to the Colleges I’m Thinking About list in Naviance Student. You must add 2 colleges to your favorites list for this task to be marked complete.

Build a Resume Understand how to market your skills, strengths, and experiences by creating
a resume. To find the Resume Builder go to About Me > My Stuff > Resume. You can continue to return to the resume builder to add or edit entries. In order to mark the task complete, you must create a printer-friendly version of your resume.
Create a Post-Secondary Goal By going to My Planner > Goals, you will create a Post-Secondary Goal helping you to identify future goals. The SMART goal template will ask you to provide information
on specific steps to take and how measurable, achievable, relevant, and timely your goal is.
Complete the Do What You Are® 2.0 Assessment The Do What You Are® 2.0 Assessment is if found my going to About Me > My Assessments > Do What You Are. Read the introduction and click Take Assessment. Select Start then answer all questions to complete the assessment. The Do What You Are® assessment uses personality types to help you identify your natural tendencies and will suggest careers and clusters you might be a good match for.

Update Your Applications Status on Colleges I’m Applying to – End of April 2021
After receiving your admissions decisions (March 2021) it is important to update your application status for each university.
Please add all colleges you applied to and report your data to help
future senior classes to understand their admissions possibilities. To search and add colleges Click Colleges I’m Applying To, search and click the Now in Colleges I’m Applying to, select the colleges you are applying to and click Move To Application list > click you have submitted your application and edit the
date the application was due.
Requesting Your Transcript
College> Apply to College > Manage Transcripts click the pink dot with a cross > college
application transcript select what type of transcript you need. DO NOT SELECT SAT OR ACT SCORES. STUDENTS ARE RESPONSIBLE TO SEND OFFICIAL SCORES FROM COLLEGE BOARD OR ACT. SINCE SCORES ARE OPTIONAL FHS WILL SEND OFFICIAL TRANSCRIPTS WITHOUT SCORES.
Click Request and Finish.
Complete the Senior Exit Survey & Final Transcript In this survey you will answer questions that will help your school
assess your post-secondary success, including participation in college fairs and visits, your post-secondary plans, and financial aid awarded. To access the survey go to My Planner > Tasks. Locate the task to Complete the Senior Exit Survey and Click on take this survey to open the survey. You can save your responses and
return to the survey to update responses then submit when finished.
