The Practice Nurses Our Practice Nurses can see patients for a number of reasons including;

New patient health check for over 16 yr olds

General health MOT’s

Blood pressure monitoring

Asthma checks

Cervical Screening (Smear Tests)

Family planning

Breathing tests, ECG’s and blood tests

Flu & Pneumonia Vaccinations

Travel vaccinations

Immunisations

Chlamydia screening

Dressing removal/changing

Diabetic, COPD, Emphysema and Heart disease checks

Patients with a long-term illness and conditions will be monitored by our Nurses as they have been specially trained to do this. Our practice nurses are able to refer to other colleagues in the practice team, such as the podiatrist (chiropodist), health visitors, district nurses, community psychiatric nurses, social services and the dietician. Practice Nurses can be seen by appointment.

Community Psychiatric Nurse (CPN) The practice has a Community Psychiatric Nurse who can assess and treat patients for a range of mental health issues. Our reception staff may advise an appointment with our CPN if appropriate or a GP may refer you.

Health Care Assistants (HCA) Our Health Care Assistants deal with Home Visits for our elderly patients who cannot attend the surgery to check on their Medical Problems. They hold phlebotomy clinics and surgeries for New Patient Health Checks, Spirometry, ECG, Blood Pressure, Ear Irrigation, alcohol screening etc. They may also assist with other GP or Nurse clinics. A Health Care Assistant can be seen by appointment.

Triage and Minor Illness Nurses Our team of Minor Illness Nurses can provide;

Telephone triage assessment for same day appointments and for general medical advice

Minor illness clinics e.g. ear, nose and throat problems, cystitis, etc.

Travel vaccine information This triage centre now forms the base for Trinity Care Wakefield.

Pharmacists The practice has in-house pharmacists who will handle acute medication requests, repeat medication reviews and can discuss any queries relating to medication. Patients may be asked to have an appointment with a pharmacist for a review.

Substance Misuse Clinics We have a number of Spectrum staff who hold clinics at Trinity Medical Centre for our patients who are struggling with alcohol and drug use. A GP will need to refer you to be seen by one of these support workers.

Midwife The district midwife provided by Mid Yorkshire Hospital Trust attends both surgeries to hold antenatal clinics. They work alongside the hospital specialists to ensure that pregnant women receive care throughout their pregnancy and after their baby as been born. Appointments to see the midwife can be made by contacting the surgery.

Administration and Management Carol Beedham is the Practice Manager and Fern Whitelam is the Business Manager. If patients have any concerns about the services offered by the practice that cannot be resolved at reception they can contact management staff in confidence, either by phone or in writing at the surgery postal address or email address: [email protected]

Secretaries Our Medical Secretary Team and can help with queries about referral letters and hospital appointments.

Reception We have a team of receptionists who work across both Trinity Medical Centre and Sandal surgeries. They will be more than happy to help with any queries about our services and can direct you to the most appropriate health professional.

Medical Students & Trainee’s We are a registered training practice and regularly have Medical Students from Leeds University sitting in sessions with our clinical staff. Patients should always let someone know if they are not comfortable having a student present. Registrars and FY2’s are trained Doctors working with us for six month periods to complete specialist training in General Practice. These trainee GP’s will hold their own clinics under close observation by our regular GP’s.

Trinity Care Trinity Care is a service offered by a team of nurses in addition to our own Triage service. The service is run from Trinity Medical Centre and is available for patients registered at the following surgeries;

Trinity Care offers a 365 day service for patients registered at the above surgeries who feel they need same-day medical attention. A fully trained nurse will offer medical advice over the phone or navigate patients to the most appropriate services. This includes’ same day GP appointments where necessary. GP Care Wakefield GP Care Wakefield is a new service offering same day and routine access to GP and Nurse Appointments outside of usual surgery hours. The service is ran from Trinity Medical Centre and Pontefract General Hospital and is available to patients registered at any of the Wakefield District GP practices. The service can be accessed by calling your own GP who will divert any calls out of hours, or be able to book any routine appointments at a convenient time. The service is open;

Monday – Friday; 6pm – 8pm (for routine appointments) 6pm – 10pm (for same-day/urgent appointments)

Weekends/Bank Holidays; 9am – 1pm (for routine appointments) 9am – 3pm (for same-day/urgent appointments)

Health Checks If patients are aged between 16 and 75 years and have not been to the surgery for three years or more, they can make an appointment to see one of our nurses for a health check. If patients are over 75 years old and have not been to the surgery for 12 months or more, they can make an appointment to see one of our nurses for a health check. We would encourage our new patients to complete the New Patient Health Questionnaire and to make an appointment for a new patient health check.

Drug and Alcohol Workers We have two specialist workers who have weekly clinics within our practice to help people who want to deal with drug and alcohol problems. Patients need to be referred to these services by a GP.

District Nurses We no longer have District Nurses assigned to the practice. District Nurses are now regionally based and mainly work with housebound patients in their own homes. The District Nurses can be contacted on 01924 327220 between 9.00 a.m. and 4.30 p.m.

Named Accountable GP All patients will be allocated a named GP responsible for their overall care at the practice. Patients’ will be informed of this named GP either following registration or on their next interaction with the surgery. Where possible, we will accommodate written requests to change a named accountable GP for a patient if they wish. We would like to stress that this does NOT have an effect on which GP patients see when they visit the surgery. All our clinical staff can access patients’ medical records and provide the same level of care.

APPOINTMENT INFORMATION

Appointments Routine Appointments may be made by telephoning the surgery after 8.00 a.m. on 01924 784101 We aim to offer routine appointments to see any doctor within 48 hours. An appointment to see a doctor of choice may take longer than this. Pre-bookable appointments are normally available up to 6 weeks in advance. Patients may ask to be contacted by a doctor or nurse by telephone if they feel a query can be dealt with without an appointment. Our reception staff will ask you about the problem for which you are making the appointment as it may be that you could be seen by a more appropriate member of our healthcare team.

Same Day Appointments Same day appointments must be made by telephoning 01924 784100 for both surgeries. Requests for a same day appointment will be assessed by a Triage Nurse before an appointment is booked. A same day appointment could be at either the main surgery or the branch surgery.

Home visits Home Visits must be made by telephoning 01924 784100. Requests for home visits should be received before 11am. Home Visits are reserved for patients who are genuinely too ill to attend surgery or are housebound. Whenever possible patients should attend the surgery where we have the necessary range of equipment and access to patient records to ensure that a consultation is safe and appropriate.

Night, Weekend and Bank Holiday Cover If patients urgently need a doctor out of normal surgery hours please call Trinity Care. If it is outside of their operating hours, dial 111. When calling 111 patients will be assessed, given advice and directed to an appropriate service. If there is a life threatening medical emergency patients should always ring 999.

Saturday SurgeriesTrinity Medical Centre is open on Saturdays for pre-booked GP, Nurse and Health Care Assistant appointments. These are available for booking throughout the week via our website or by calling the appointments line on: 01924 784 101. These appointments are designed for our patients who struggle to get to a weekday surgery. There are limited appointments available. There are limited reception services available on Saturday.

Cancellations, Non-Attendance & Time-Keeping We ask that all patients let us know as early as possible if they cannot keep an appointment. If patients do not notify the practice within a reasonable time frame and the appointment cannot be offered to another patient then this will be marked as a missed appointment. 4 consecutive missed appointments will result in removal from the practice list. If patients are running late for an appointment they should call the practice as soon as possible so that the doctor can be notified. If an appointment time is missed by 10 minutes this will be recorded as a non-attendance.

Care Navigation Our reception staff are trained in Care Navigation so they will ask a few questions when patients request a GP appointment. This is to help navigate patients to the best person in practice to deal with the problem such as booking a patient with lower back pain in directly with a physio rather than a GP. This is often quicker and more effective for the patient.

PRACTICE INFORMATION

All our doctors work at both Trinity Medical Centre and Sandal Castle Medical Centre. Patients can always request to see a specific Doctor should they choose however we are a Group Practice and so all records are shared by all doctors. We are not a Limited partnership.

Trinity Medical Centre (the practice) was formed in April 2017 as a merged partnership of the previous Grove Surgery and Almshouse Surgery who shared the same building, Trinity Medical Centre. The practice retained the previous Almshouse branch surgery and Sandal Castle Medical Centre and continues to uphold the values and high standard of both individual practices.

Registering as a new patient New patients are requested to attend in person, complete a registration form and new patient questionnaire and provide photo identification e.g. passport and something to confirm their current address e.g. utility bill.

Repeat Prescriptions Repeat prescriptions are issued only with the doctors’ consent. Repeat prescriptions can be ordered;

- Online via our website www.tmcwakefield.co.uk - In person at either surgery. - By sending a stamped address envelope to the surgery with the request. - Phone the surgery, choose the repeat prescription option and leave a message with the request.

Please allow 24 hours for the prescription to be processed. Prescriptions not collected within 4 weeks will be destroyed and need to be re-ordered. Prescriptions may be collected from a local chemist by prior agreement with us.

SystmOnline The Practice offers patients an online system which enables the booking of appointments, ordering repeat prescriptions, viewing of summary medical information, completing questionnaires and changing contact details. To gain access to online services patients should complete an application form which is available from any reception area. Alternatively the form can be completed online on the practice website. This service can be accessed through our practice website www.tmcwakefield.co.uk

Test Results We encourage everyone who has tests arranged by the doctor or nurse to all patients to ring the Triage team after 2pm who can give patients their results and explain them if necessary.

Data Protection We need to provide patients with the best possible care, to do this we need to keep accurate and up-to-date records. We follow strict rules to ensure records are kept confidential at all times and only people involved in patient care will have access to their contents. A Privacy Notice is available on the practice Website.

Privacy at desk If a patient wished to talk to a Receptionist in private this can be arranged.

Complaints We try to give the best possible service, however if a patient has a complaint or suggestion, please let us know. We hope that most problems can be sorted out easily and quickly, preferably at the time they arise, and with the person concerned. If a problem cannot be resolved in this way, and patients wish to make a complaint, we need to know as soon as possible. We operate a practice based complaint procedure as part of the NHS system for dealing with patient complaints. We also have suggestion box at each reception, but request patients do not put complaints in here.

Family & Friends Test As part of the NHS Family & Friends scheme, patients can complete a form after visiting the practice that asks if patients would recommend us to their family and friends and gives them the option to explain their answer. The forms can be left in a box at each surgery and will remain anonymous.

Access to records Patients have the right to see their health records, subject to limitations in the law. Signed consent is required to allow anyone else to have access to information in a patients’ health records, e.g. employers, or insurance companies. Applications for access to records must be made in writing. Patients can also request access to their detailed coded record online.

Translators If English is not a patient’s first language, we will do our best to organize an interpreter. (Telephone Interpreting Service) Adequate notice is required if this service is required.

Chaperones Patients will be offered a chaperone for all intimate examinations. Patients can always ask for a chaperone for any examination that the doctor or nurse deem necessary.

NHS Zero Tolerance Policy We ask that all visitors to the practice treat the doctors and practice staff with courtesy and respect. We will not tolerate violence, abuse, or threatening or aggressive behaviour towards any of the staff. Such behaviour will result in immediate removal from the practice list and the police will be called.

Accessibility The Practice provides clearly marked and wider disabled parking bays at both surgeries. We have dipped curbs and automatic doors. We allow guide or other assistance dogs into the premises. We have a hearing loop available on reception and a portable one for use in the consultation rooms. We have a disabled access toilet at both surgeries and lift access to first floor rooms where required. Disabled patients can make appointments by email, on-line booking, or letter if telephone access is not possible. We also offer large font or electronic communications on request. All practice staff aim to cater for any additional needs a patient may have. If patients have a regular requirement for either face to face or written communication, and need special adaptations within their appointments, they should let practice staff know so that we can setup appropriate reminders on our practice systems making each interaction with the surgery as easy as possible for the patient.
