1. That I was born on June 8, 1982 in Laoag City;

2. That I established my permanent residence in #25 Rimando Road, Baguio City 2600;

3. That on August 20, 2015, I applied for a Record Clearance from the National Bureau of Investigation;

4. That I received an information from the National Bureau of Investigation on August 20, 2015 that my namesake, a certain Maria Makiling, has a pending criminal case for Attempted Homicide docketed as Criminal Case No. 1234 pending before Branch 67 of the Metropolitan Trial Court of Quezon City;

5. That I hereby unequivocally deny involvement in the aforementioned case;

6. That I also declare under oath that I am not in any way involved in any case proceeding as either plaintiff/complainant or defendant/accused in the Philippines and abroad, specifically with regard to the aforementioned case;

7. That I have been employed in Hong Kong for five (5) years to this date;

8. That I am executing this Affidavit to attest to the foregoing facts and to deny absolutely and entirely any involvement in the case/s aforementioned and for all legal intents and purposes.

1. That I am a holder of Philippine Passport No. EC123456 issued on January 15, 2015

by the Department of Foreign Affairs in Baguio City;

2. That on February 25, 2015, my bag, containing my passport was snatched. After

two (2) days, my bag, together with my passport was recovered by the Police

Officers; however, the portion of my passport bearing my photograph was torn and

could no longer be located.

3. That I reported the above-mentioned incident to the Police and a certified true copy

of the police blotter is hereto attached;

4. That the damage on the said passport has rendered it unusable for travel purposes

and therefore I request the Philippine Embassy/ Consulate to issue a replacement

passport in my favor; and,

4. That I attest to the veracity of the foregoing information.
