Mission & Values- To educate our cardiovascular disease (CVD) patients about

their medications and disease states.- To reduce overall rate of deaths caused by CVD. - To provide ultimate convenience by having on-site and home

visit counseling. - Home visits will assure proper storage conditions of medications, along with home safety and accessibility issues.- To uphold patient privacy by adhering to all HIPAA rules and regulations.

StatisticsAccording to The Heart Foundation:

- Heart disease is the number one cause of death for both men and women in the United States, claiming approximately 1 million lives annually.

- Every 33 seconds someone in the United States dies from cardiovascular disease (CVD)

- By 2020, heart disease will be the leading cause of death throughout the world.

Statistics cont...- An estimated 80 million Americans have one or more types of heart disease.

- Currently about 7.9 million Americans are alive who have had a heart attack.

- Locally and statewide, African Americans, especially males, have the highest rate of death from CVDs.

- About 64.34% of the Baltimore population is comprised of African Americans.

- African Americans are more prone to CVD than other races
