Overview of the B-Line System

The Ruth L. Gottesman Clinical Skills Center utilizes a digital recording and online assessment program called B-Line for use in courses like Introduction to the Patient for both formative and summative patient encounters. All students at Einstein are provided with a unique username & password for access to B-Line. On November 6th, you will be participating in your first Standardized Patient encounter. The recordings will be available to you for review and self-assessment. You will meet with your small group faculty on either Dec 4th or Dec 11th to receive feedback from them on your patient encounter.

Overview of the B-Line System

This self-guided slide presentation has been created for you to review prior participating in the November 6th session. Please review all parts thoroughly. At the end of the slide presentation, there will be instructions for you to assess your personal computer’s software for viewing B-Line recordings. You MUST following those instructions as indicated. Failure to do so will delay your ability to complete the required self- assessment of your patient encounter. Thank you

Standardized Patient Encounter November 6th, 2013

Assigned a specific DATE and REPORTING TIME – check schedule provided on eMED.

Dress appropriately (white coats and professional attire). You will be seeing one (1) Standardized Patient You will have 7 minutes for the interview

There will be a 2 minute warning at the 5 minute mark. After the interview, you will be done for the afternoon.

You will be emailed when the recordings and SP feedback are available for you to review.

Total time at the CSC will be approximately 30-45 minutes.

REMINDERS

- The schedule will be posted on the eMED page. - You MUST be on time. - The interviews are scheduled in groups of 10. If you are late, it will delay all other 9 students and then every student scheduled after your time slot. - Do NOT discuss details of your patient case with other

students. - The remaining slides will provide a glimpse of the exam

process

You will be given a map of the CSC with your name, room assignments and your assigned username/password for using B-Line.

Using B-Line requires students to utilize two exam rooms. We have a Login Room and an Encounter Room. Your Orientation sheet will indicate the assigned Login Room and Encounter Room.

Your group will then be released to your assigned rooms on either A Wing or B Wing of the CSC.

Login Room ↔ Encounter Room

Students begin each recorded session by entering the assigned Login Room and then entering their username and password into the Computer. Once logged in, B-Line establishes a link between your user profile and the session we are about to record. After you have successfully logged in, accept the Honor Code and then go stand outside the Encounter Room (where the patient is waiting) and await the announcement to begin.

RECAP: A student sits at the Computer Terminal and enter his username and password. If you have difficulty or it is not accepted – immediately alert a proctor in the hallway to assist you.

What am I supposed to do????

Scenario: You are a 1st year medical student who has been asked by your preceptor to meet with an adult patient at an outpatient clinic. You have 7 minutes to introduce yourself, elicit the chief complaint, and History of Present Illness. You may also explore relevant Past Medical/Surgical History, Family History, Social History, and Review of medications/allergies. Throughout the interview you are expected to demonstrate professionalism, empathy and opportunities to establish a good rapport and relationship with the patient.

Login ↔ Patient Interview (Encounter) … Continued

You’ll have seven minutes. You may leave before time is up, but once you leave you can not go back in the room. You must remain in the Login room until time has expired. At the end of your interview or when time is up (whichever comes first); you MUST return to the Login Room and click STOP ENCOUNTER.

The Control Room will make an announcement when time has ended. Once the announcement is made, you MUST leave the Encounter room and return to the Login room...

You will see on the Login room’s Computer your name and the STOP ENCOUNTER button – Click on it and then return to the lobby.

When you return to the lobby; remember to drop off your clipboard and green pen before leaving the Clinical Skills Center. On your way out, please do not discuss anything related to the patient seen with students who have not yet completed their interviews.

What happens when I am done?

• When you have finished, you’ll be told to return the

lobby and turn in your clipboard and pen. You may take with you your Orientation sheet.

• You are NOT permitted to discuss the patient cases with other students who have yet to complete their interview

• A class email will be sent when the recordings and SP’s feedback are available for your review.

PREPARING FOR THE INTERVIEW

• Review all the session notes on eMED • Ask your preceptor in the ICM: Clinical Experience

course to permit you to practice interviewing on your own next Monday the 4th (if you have not already).

• Time yourself to see how much information you are able to elicit in five-seven minutes.

• Refer to the email you were sent regarding the VIDEO DEMO – follow the instructions provided.
