WHEREAS, Bids and Awards Gommittee (BAC) Resolution No. 21-04-01dated 05 April 2021 declared the bid offered by Toyota Otis, lnc. as the LowestCalculated Bid (LCB) for the Procurement of Supply and Delivery of Thirty-Seven (37)Multi-purpose Vehicle for Philippine Statistics Authority (PSA) Field Offices(Rebidding);

WHEREAS, on 13 April 2021, Toyota Otis, lnc. was subjected to post-qualification;

WHEREAS, on 16 April 2A21, the result of the post-qualification evaluationwas presented to the BAC (see attached post-qualification report);

WHEREAS, the bid offered by Toyota Otis, lnc. was determined to becompliant and responsive to the legal, technical and financial requirements of thesubject procurement;

WHEREAS, after review and verification of the submitted documents, theBAC found the same to be valid and sufficient;

NOW, THEREFORE, the BAC RESOLVES AS lT lS HEREBY RESOLVED,a. To declare the bid of Toyota Otis, lnc. as the Lowest Calculated and

Responsive Bid (LCRB) for the Procurement of Supply and Delivery ofThirty-Seven (37) Multi-purpose Vehicle for PSA Field Offices (Rebidding);and

b. To recommend the award of contract to Toyota Otis, lnc. for the above-stated procurement.

You are therefore required within ten (10) days from receipt of this Notice of Award toformally enter into contract with the Philippine Statistics Authority (PSA) and to submitthe Performance Security Bond in the form stipulated in the agreement. Failure to enterinto said contract or provide Performance Security constitutes sufficient ground forcancellation of this award.

NOW THIS AGREEMENT WITNESSETH AS FOLLOWS:

1. In this Agreement words and expressions shall have the same meanings as arerespectively assigned to them in the Conditions of Contract referred to.

2. The following documents as required by the 2016 revised Implementing Rulesand Regulations of Republic Act No. 9184 shall be deemed to form and be readand construed as integral paft of this Agreement, viz.:

Philippine Bidding Documents (PBDs);i. Schedule of Requirements;ii. Technical Specifications;iii. General and Special Conditions of Contract; andiv. Supplemental or Bid Bulletins, if any

. Winning bidder's bid, including the Eligibility requirements, Technical andFinancial Proposals, and all other documents or statements submitted;

Bid form, including all the documents/statements contained in theBidde/s bidding envelopes, as annexes, and all other documentssubmitted (e.9., Bidderb response to request for clarifications on thebid), including conections to the bid, if any, resulting from the ProcuringEntity's bid evaluation;

iii. Pefformance Security;

iv. Notice of Award of Contract; and the Bidder's conforme thereto; and
