AudienceThe audience for this Response to Request for Proposal (hereinafter called the “RRFP”)

includes: Capstone Medical which encompasses: Doctors, Nurses, Sales Associates,

interns, Management, Human Relations, etc. (hereinafter called “the Client” or

“Client”) and Geeks-4-U LLC which encompasses: Project Management, System

Analysts, and System Designers. Namely: Daniel Stinebaugh, Patrick Tyler, Evan Diaz,

Hugh Connell, and Sean Nouhi. (hereinafter called “the Contractor” or “Contractor”.)

IntroductionOn behalf of the skilled team at Geeks-4-U LLC, I am pleased to offer this Response to

Request for Proposal titled Capstone Medical Project. This Response to Request for

Proposal (hereinafter called the “RRFP”) is issued pursuant to the Consultant Services

Master Agreement between the client and effective (the

“Agreement”). This RRFP is subject to the terms and conditions contained in the

Agreement between the parties and is made a part thereof. Any term not otherwise

defined herein shall have the meaning specified in the Agreement. In the event of any

conflict or inconsistency between the terms of this RRFP and the terms of this

Agreement, the terms of this RRFP shall govern and prevail.

This RRFP # , effective as of , is entered into by and between

Contractor and Client, and is subject to the terms and conditions specified below. The

Exhibit(s) to this RRFP, if any, shall be deemed to be a part hereof. In the event of any

inconsistencies between the terms of the body of this RRFP and the terms of the

Exhibit(s) hereto, the terms of the body of this RRFP shall prevail.

Scope of Work

The Contractor shall provide the Services and Deliverable(s) as follows:

1 Each remote office will connect back to the central office and will meet all stated

requirements for integrity, storage, and movement of related data and the

applications that process such data to comply with the current HIPAA

regulations. By providing you with a MSLP & multi WAN architecture utilizing

multiple ISP’s for redundancy and to achieve the requested SLA.

2 A design where the local area network (LAN) with gigabit backbone speeds and

initial performance testing to minimize bottlenecks, providing a highly available

physical network.

3 The doctors from each office will receive both a laptop and a tablet computer

with docking stations to be selected by the contractor

4 The network and communications infrastructure that supports such applications

covering all desired functions in security, redundancy, operability and

expandability utilizing a Cloud based solution eClinical Works option 2.

5 The network will have normal operating hours and during off-hour times will

only be accessible by the Manager and IT staff.

6 New printers and workstations for each office will be purchased as deemed

necessary by the contractor and will provide at least 1 generation’s worth of

upgradeability

7 New servers will be configured with group policies to allow ease of management

and to provide a high level of permission based control over who can access

resources on the network and the internet.

8 Web-based applications for all HR, Accounting & Medical Management by

utilizing a cloud based EMR system eClinical Works option 2 with an SLA

provided by the 3rd party and agreed to upon licensing this option.

9 Backups will consist of a local and offsite backups using local SAN or NAS

solutions and Amazon Glacier for offsite backup.

10 Email will be handled by utilizing a cloud based solution for email consisting of

office365 with an SLA provided by the 3rd party and agreed to upon licensing this

option.

11 Initial training on how to access and utilize the new applications (ie: Office365,

eClinical Works, Microsoft Windows 8)

12 Phone service will be upgraded by using a locally hosted PBX setup from xxxxx

on it’s own subnet of the local network.

Customer Change Rules and Regulations

The following process will be followed if a change to this RRFP is required:

1. A Project Change Request (PCR) will be the vehicle for communicating change. The

PCR must describe the change, the rationale for the change, and the effect the

change will have on the project.

2. The designated Project Manager of the requesting party (Contractor or Client) will

review the proposed change and determine whether to submit the request to the

other party.

3. The client’s point of contact will review the proposed change and approve it for

further investigation or reject it. Contractor and Client will mutually agree upon any

charges for such investigation, if any. If the investigation is authorized, the Client

Project Managers will sign the PCR, which will constitute approval for the

investigation charges. Contractor will invoice Client for any such charges. The

investigation will determine the effect that the implementation of the PCR will have

on RRFP price, schedule and other terms and conditions of the Agreement.

4. Upon completion of the investigation, both parties will review the impact of the

proposed change and, if mutually agreed, a Change Authorization will be executed.

5. A written Change Authorization and/or PCR must be signed by both parties to

authorize implementation of the investigated changes.
