1.0 INTRODUCTION1.1 Background Information

This Proposal is compiled With the purpose of ACM Certification for assisting Poly Cable Industries

Ltd.with the execution of Quality Management System Certification. The certification will be carried outat the nominatedPoly Cable Industries Ltd..

ACM Certification BD Limited, as part of ACM Ltd (UK), has extensive previous international experience

with this method of QMS (Quality Management System) certification process.

1.2 Company Profile

2.0 GENERAL PROPOSAL:

It is proposed that ACM Certification will carry out the 3rd

Party Independent Assessment with one

qualified auditors both at nominated Site. The Work Scope and specification will be as per the informationprovided to ACM Certification from Poly Cable Industries Ltd.prior to the actual work starts.

2.1 Consultancy:

For the preparation of the management system we offer your company extensive consultation from our

consultancy wing, which is absolutely independent and separate entity and managed by highly expertise

consultants, which shall include entire system documentation and technical training for two days.Technical training shall include ISO 9001 Awareness, Standard Awareness, How to prepare yourself for

certification, awareness on conducting internal audits at your premises etc. An exam will be held at the endof the training session and each personal will be provided with training certificates from ACM

Certification.

Charges: 40,000 BDT

2.2 Document Review:

This is an obligatory service provided by the certification body in order to assist the company to take

necessary corrective action before the final audit starts in the documentation. The document review is done

in order to know to what extent and how well your company manual & procedures are addressed inconjunction to the ISO 9001:2008 standard.

2.3 Pre-Assessment (Stage-1 Audit) :

ACM Certification will, carry out a Pre-Certification Assessment of your Quality Management & FoodSafety Management Systems according to the requirements of ISO 9001:2008. Audit Reports will be

compiled as per the requirements specified and will be sent to Poly Cable Industries Ltd.for necessaryaction if required.

Charges: BDT = 30,000/-

2.4 Final Audit (Stage-2 Audit):

The principal purpose of the initial Assessment is to audit the Companys Quality Management Systems

for compliance with the ISO 9001:2008 standards. Please note that Initial Assessment is the obligatory

service.

Charges: BDT = 30,000/-

2.5 Accreditation Mark/Logo: UKAS of England

2.6 Accreditation Code: EAC 19

2.7 Scope of Certification: Manufacturer of XLPE & FRLS-PVC Cables, Wires & Conductors

2.8 Routine Surveillance Visits:

It is also an obligatory service ACM Certification will perform surveillance visit approximately after every

year i.e. 2 Surveillance Visits will be performed during the 3 years validation period of the Certificate.

Such routine surveillance visits are performed to ensure the continuous compliance of your Management

Systems to the requirements of ISO 9000 Standard.

3.0 Summary of ACM Certification Service Charges

3.1 Charges:

Initial Audit (Stage: 1 & 2) = BDT 60,000 only

3.2 Invoicing for Certification: Advance of the initial audit fee an amount of BDT 60,000/- only

(against the Stage -1 & 2 Audit Fees) will be taken at the signing of this contract via cross cheque or cash.

Invoicing for all surveillance activities will be invoiced six weeks prior to the activity starts.

4.0 Summary of ACM Certification Consultation & Training Charges

4.1 Charges:

Documentation & Training Charges = BDT 40,000 onlyTravel up to the site = Will be borne by the Client

Visit Numbers = Three Days (03) includes Two (02) days Technical Training

Note: No VAT will be charged if paid in cash

4.2 Invoicing for Consultancy:For consultancy, an amount of BDT 40,000 of the total payment will be made at time of signing the

contract agreement via cross cheque or cash

4.3 Contract:Once the terms & condition are agreed between both the parties a formal contract will be sent to you for

signatures.
