All registrations must be filled out completely and accompanied by payment in full in order to be processed unless otherwise stated in the brochure. Do not forget to fill out the waiver form as this will delay the processing of your registration(s).The Lockport Township Park District is unable to guarantee enrollment and is not responsible for lost or late mail. Additional classes may be added to accommodate enrollment. The Park District reserves the right to change or alter information printed in this publication such as policies, fees, times or locations. We try to give as much notice as possible for these changes and apologize for any inconvenience.

Reasonable Accommodation StatementThe Lockport Township Park District is committed to accessibility to all individuals with disabilities and strives to comply with the 1990 Americans with Disabilities Act (ADA) through equal and integrated participation. If a request is made less than 72 hours before the event the Lockport Township Park District will make a good faith effort to accommodate your request.

Registration PolicyPrograms will be filled on a first-come, first-serve basis. Mail-in and drop-off registrations will be processed after walk-ins. All programs have a minimum and maximum enrollment. If a class is filled, waiting lists will be formed and every attempt will be made to accommodate persons on that list. Proof of residency, tax bill or voter ID card along with current driver’s license is required for early registration and discounted price of programs. Residency must be updated once a year.

NSF ChecksA $30 fee will be charged to persons presenting NSF checks for Lockport Township Park District programs. This fee must be paid at the administrative office prior to participation in the program.

Program Refund Policy1. A full refund is granted on any program/class cancelled by the Lockport Township Park District (LTPD) due to insufficient program registration.2. A $10 administrative fee will be charged to refund requests (unless program is cancelled by LTPD).3. Refund/transfer requests may be made up to five business days prior to the start of class. Refund requests received less than 5 business days from the start of the class/program will only be honored for certified medical reasons or moving from the Park District.4. Refunds on Prairie Bluff Holiday Brunches will only be granted if the request is received at least 10 business days prior to the event or if the registrants spot can be filled by a patron on the waitlist. If this occurs the registrant will receive a full refund minus the $10 admin. fee.5. All refund requests, including online registration, must be made in-person in writing at the DPCC, 1811 Lawrence Ave., or Challenge Fitness, 2021 Lawrence Ave. in Lockport.6. Credit balances of $10 or less will be kept on account.

WAIVER, RELEASE OF ALL CLAIMS AND HOLD HARMLESS AGREEMENT FOR THE LOCKPORT TOWNSHIP PARK DISTRICTPlease read this form carefully and be aware that, in signing up and participating in Lockport Township Park District programs, you will be waiving and releasing all claims for injuries arising out of these programs that you and other named participants might sustain. The terms “I”, “me” and “my” also refer to parents or guardians as well as participants in the program. In registering for these programs, you are agreeing to the following: As a participant in these programs, I recognize and acknowledge that there are certain risks of physical injury, and I agree to assume the full risk of any injuries, damages, or loss which I may sustain as a result of participating, in any manner, in any and all activities connected with or associated with programs. I further recognize and acknowledge that all athletic activities involving strenuous exertion or potential body contact are hazardous recreation activities and involve substantial risks of injury. I agree to waive and relinquish any and all claims I may have as a result of participating in these programs against the Lockport Township Park District, any and all other partici-pating or cooperating governmental units, any and all independent contractors, officers, agents, servants, and employees of the governmental bodies and independent contractors, and any and all other persons and entities, of whatever nature, that might be directly or indirectly liable for any injuries that I might sustain while participating in these programs. (The parties described in the preceding sentence are referred to as “released parties” in the remainder of this agreement.) I do hereby fully release and discharge the Lockport Township Park District and the other released parties from any and all claims for injuries, damages and loss which I may have or which may accrue to me on account of my participation in these programs. I further agree to indemnify, hold harmless and defend the Lockport Township Park District and any and all other released parties, from any and all claims for injuries, damages and losses sustained by anyone, and arising out of, connected with, or in any way associated with my conduct and the activities of these programs.I further understand and agree that the terms such as “participation,” “programs,” and “activities” referred to in this Agreement, include all exercises and physical movements of any nature while I am participating in these programs and further include the provision of or failure to provide proper instructions or supervision, the use and adjustment of any and all machinery, equipment, and apparatus, and anything related to my use of the services, facilities or premises involved in these programs, and transportation to and from my events. I understand the nature of these programs for which I am registering, and have read and fully understand this Waiver, Release and Hold Harmless Agreement. I further understand that any advisements or warnings of the particular risks of these programs that I subsequently receive will be incorporated by reference into and become a part of this agreement. Phase 4 Update“Restore Illinois” 5 phase plan Status: Current Phase 4

Welcome to phase 4 of the Governor’s “Restore Illinois” 5 phase plan, we moved forward to Phase 4 today. In this phase more activity will take place. Gatherings are limited to 50 people. Social distancing and wearing a mask will continue in this phase. Below is a summary of what is open in phase 4: Prairie Bluff golf course • Golf course will allow 4 golfers per group with single rider carts or duel riders in the same household or wearing face coverings. Carts will be on a first come first serve basis. Tee times are in 10 minute increments • Driving range is open with proper distancing • Practice areas are open with proper distancing • The pro shop is open with a limit of 6 people

PB Mulligan’s • Outdoor and indoor dining with tables spaced 6-ft. apart • Parties up to 10 people are allowed • Standing areas are limited to 25% capacity with proper distancing • Banquets up to 50 people are allowed with proper distancing between tables to start in mid-July • Music on the patio starts 07/2/20 and runs every Thursday and Saturday (6:30pm -8:30pm) except 07/04/20

Challenge Fitness • We will open on July 1st normal hours • Fitness equipment will be moved to 1 tennis court with all equipment spaced 6-ft. apart • Lap Swim will be available • Hot tub, steam room and the nursery room will remain closed • Indoor and outdoor tennis and lessons have already resumed• Fitness classes on tennis court 10ft spaces

Camps • Session 1 of summer camps started 6/15/20. Session 2 and session 3 resident registrations will take place on 06/27/20. Session 2 starts on July 6th • Sport camps have begun and more camps will be available in July

DPCC • DPPC will be utilized by camps and SilverSneakers through August • Besides camps and Silversneakers, DPCC will be closed to the public

Outdoor Pools • We will remain closed for the season

Playgrounds / Splash Pads / Basketball courts/pavilions/outdoor bathrooms • Playgrounds-are open with a max. of 50 people• Splash pads-are open from 10am to 8pm (Hassert Park splash pad will open on 7/3 with the same hours.)• Basketball courts-waiting on guidance from DCEO • Pavilions – planning to start to rent pavilions with a max. of 50 people starting 6/30/20 • Outdoor bathrooms – open with daily cleaning on 06/29/20 • Water Fountains – waiting on guidance from DCEO

Outdoor fields• Fields will be available for competitive play with groups of 50 or less • Adult softball leagues will be begin in the fall

Special Events • Please check our website for updates on special events • Will County Auto rebuilders car show in August is cancelled • July and August concerts are cancelled, September is tentatively still on • Drive in Movie (Ferris Bueller’s Day Off) is July 31st at Hassert Park Please visit our website at www.lockportpark.org for the latest information on Park District facility information, programs and special events. We are planning on staying in phase 4 for quite some time. We do expect guidelines to slightly change while we are in phase 4. Moving to phase 5 will require a vaccine or the elimination of new cases over a sustained period of time. We will continue to monitor these guidelines and make the best decision for our staff and patrons.

Through The Ed Brown Tree and Bench Commemorative Program, friends and family can provide a unique dedication to a loved one by purchasing a tree or bench to be placed in one of Lockport Township Park District’s beautiful parks.

A commemorative tree or bench can be purchased for many occasions including the memory of someone who has passed, in honor of a dedicated service member, to celebrate a birthday, a graduation, retirement, or any other occasion.

Commemorative TreesThe Dellwood Foundation’s commemorative trees are 2” to 3” around and a minimum of 6’ in height. The tree will be planted in a neighborhood park with the final location selected by the park services department in collaboration with the donor’s specifications.

A plaque will be mounted at the base of the tree after the tree’s growth has been established. Trees will be planted in the spring, early summer or fall. The price of each commemorative tree is $300, which includes the tree itself, delivery, installation, a commemorative plaque and maintenance.

Commemorative Park Benches The Dellwood Foundation’s commemorative park benches are 6’ long and made of recycled plastic composition. The price for each commemorative park bench is $800, which includes the bench itself, delivery, installation, a commemorative plaque and maintenance. The Dellwood Foundation will add to your contribution, thereby installing a $1,000 bench.

Although the park services department has the final decision regarding bench locations, every effort will be made to accommodate the donor’s request.

Dellwood Foundation ScholarshipsWe believe that all residents of the Lockport Township Park District, regardless of financial limitations, should have access to our programs and facilities. The Dellwood Foundation will work with the Lockport Township Park District to attempt to provide a scholarship to residents faced with a financial hardship.

Dellwood FoundationThe Dellwood Foundation is a 501(c)3 charitable organization dedicated to enhancing cultural and recreational opportunities in Lockport Township. An affiliate of the Lockport Township Park District, the Foundation works to provide cultural events, park improvements and assist with recreational programming.

Located in scenic Dellwood Park at the end of Dell Park Ave. off of State Street in Lockport, the Canyons Disc Golf Course is host to 27 baskets throughout

the park. It has been said that every “hole” is a signature basket with breathtaking natural backdrops from flowing water, rock formations,

bridges to steep terrain (slope: 0% to 12%) and so much more. It is truly one of the most beautiful disc courses in the area with challenges that even the most experienced disc golfer will find exhilarating.

The composite course features three skill levels (red, white and blue), 27 concrete tee pads (white course), six water hazards, and a practice area.

Day Camps:This season there are many changes in our camp program due to the COVID-19 crisis. For the first session, we are only offering limited spots at Adventure and Chaney Camps based on Phase 3 of the State Guidelines. Activities such as: free play, sports, games, arts & crafts, snack time, and time for homework is conducted on a daily basis by qualified staff right in the comfort of the children’s home school.

Registration will be taken at Challenge Fitness until Oct. 1, 2020. After Oct.1st, registration will be taken at the Dellwood Park Community Center between 8am-4pm. OASIS registration is taken IN-PERSON ONLY.

The Lockport Township Park District presents the Hollywood at Hassert Park Drive in Movie this Summer!

“Ferris Bueller you’re my hero!” Come enjoy the 80’s classic Ferris Bueller’s Day Off at the Hassert Park Drive-In. There will be a maximum number of cars allotted for the movie night so register early! All cars must be pre-registered to enter. If you are not pre-registered, you will not be permitted in the event area. All cars will be staggered to follow social distancing guidelines, safety and best view. When registering please notify the registrar if you will be driving a Car, SUV/Minivan or Truck to the event. Food and snack specials will be available to order from PB Mulligan’s. 1. Day trip payments are non-refundable unless the spot can be filled. Please notify the travel supervisor as soon as possible if someone will be taking your place. A $10 administration fee will be charged.

2. Extended tour deposit/payments are refundable up to 60 days prior to departure, unless stated otherwise. Less than 60 days prior to departure deposits/payments are non-refundable unless the spot can be filled or the optional trip insurance was purchased. Please notify the travel supervisor ASAP prior to departure if someone is taking your place. A $10 administration fee will be charged.

3. Please arrive a minimum of 15 minutes prior to scheduled departure. No refunds will be issued for not arriving on time.

4. Anyone with mobility issues must be accompanied by another adult who is capable of and totally responsible for providing all assistance. LTPD Travel cannot provide special individual assistance to travelers with special needs for walking, dining or other routine activities.

5. Patrons suffering from motion sickness are responsible for taking precautions prior to tour departure. Reserved seats are not guaranteed for this condition.

6. If you have food allergies or medical issues that may affect the tour and its members, please notify the travel supervisor immediately after registering.

7. Seating requests for motor coach travel may be provided if available, on a first-come, first-served basis for handicapped individuals. To qualify for reserved seating you must own a handicapped parking permit and contact the travel supervisor with your request a minimum of four weeks prior to departure.

8. Seating on the motor coach is first-come, first served. Coach assignment is based on registration dates.

9. Trip insurance is optional but highly recommended. A separate check is required to purchase insurance. In some cases, a minimum of 10 participants is required for insurance to be purchased.

10. If the minimum registration is not met, LTPD has the right to cancel a tour. If a tour is cancelled you will be notified no later than 7am on the date of departure. A full refund will be given.

11. LTPD has the right to make changes in the itineraries. Schedules may change without notice. Return times are estimated.

12. LTPD Travel reserves the right to withhold tour membership, or require any traveler to leave a tour at anytime, when such action is determined by LTPD Travel to be in the best interests of the traveler or the tour group.

13. It is not recommended or encouraged for any patrons to drive themselves to any trips. LTPD Travel will not be held accountable and fees will not be adjusted if anyone chooses to provide their own transportation. The trip supervisor must be contacted in advance if this were to occur.

Park Board MeetingsThe Board of Commissioners meet at the Dellwood Park Community Center located at 1811 S. Lawrence Ave. in Lockport at 7pm on the fourth Monday of every month with the exception of May 2020. The public is welcome to attend.

Residency - Proof is Required ANNUALLYA Lockport Township Park District resident is described as a person whose legal domicile is within the boundaries of the Lockport Township Park District. Residency is the principle place of habitation of the individual occupying the home, apartment, condominium or estate. It shall be the principle yearly residence for the family residing at that location. The resident must be able to produce a valid driver’s license or state ID, and a current property tax bill or voter’s registration card verifying residency at that location.

Free Resident ID CardResidents are required to register for a free ID card annually. Those 17 years of age and younger must be with a parent/legal guardian when registering. Resident IDs must be presented at the front desk for admission into open gym at DPCC and to be eligible for discounted rates at the pools and Prairie Bluff Golf Club. No exceptions will be made. A $5 fee will be charged for replacing lost or stolen ID cards. Resident ID cards expire one year from the date issued.

Park PoliceThe mission of the Lockport Township Park District police is to have safe parks and facilities for the community to utilize and enjoy. The Park District Police Department has 13 sworn police officers. Regular patrols are conducted in the parks, on the trails and throughout the facilities. Uniformed officers patrol these areas in vehicles, on foot and on ATVs. At times, officers will be in plain clothes. Lockport Township Park District police officers will be going to park district programs to talk to children about safety at the park, school and home. If you see anything suspicious or any criminal activity, please notify the police immediately. If you have any concerns or need information about the Park Police, please notify John Waxweiler Jr., Chief of Police at 815-838-1183.

Mandated ReportingLockport Township Park District staff are mandated by the State of Illinois to report to the DCFS hotline if they observe a situation that provides reasonable cause to suspect child abuse or neglect. Workers in certain professions, including recreational program personnel, have the above legal mandate; including all LTPD staff. Staff are trained in signs and symptoms of abuse and neglect and the procedure for making a report.
