Are you interested in supporting people with learning disabilities and or autism whilst developing new and exciting ways of working, sharing skills within the acute settings? An exciting opportunity has arisen to join Royal Cornwall Hospital to improve the quality of care received by individuals with a learning disability and or on the autism spectrum. You will be responsible for managing the interface between mainstream general hospital services, primary care services and community specialist learning disability services in supporting people with learning disabilities when requiring hospital services. This is a challenging, stimulating and influential opportunity to make significant impact on the delivery of patient care in acute settings. You should have already built up a strong base of clinical experience and you will need to have skills to make clinical decisions and to promote self-management. We are looking for an individual who can develop effective and collaborative working with healthcare and multi-agency colleagues and includes identifying and meeting the training needs of staff as well as providing specialist and nursing advice. 1. Act as a resource for the acute hospital regarding the complex health needs of people with a learning disability and or on the autism spectrum.

services and community specialist disability services.

3. Challenge professional and organisational boundaries and barriers, identifying areas for skill/knowledge development to ensure that people with learning disabilities and or autism spectrum conditions have equity of access to healthcare services.

colleagues, including identifying and meeting the training needs and providing specialist and nursing advice.

3. Work with the Referral Management System (RMS), continue to lead and if required to further develop the current accessible Choose and Book system, with senior colleagues internally and externally.

4. Represent the Trust at both the Learning Disability and Autism Partnership Boards, disseminating current initiatives, acting and leading on them, in liaison with senior colleagues.

5. To act as a clinical expert with advice and support for staff in the practical application of the Mental Capacity Act 2005.

6. Manage the interface between mainstream general hospital services, primary care

services and community specialist support services, recognising and leading upon practice development /change where identified.

1. Raise the profile of the health care needs of people with a learning disability and autism across secondary care provision, bridging the gap between acute clinical care areas to enable better communications and access to healthcare.

2. To actively promote the Acute Liaison Nurse role to service users, families and carers

and other professionals and developing and maintaining networks with partners in primary, community and learning disability services.

3. To contribute to the development of healthcare information and resources in

accessible formats for service users and their families / carers.

4. Develop constructive relationships by attending and contributing specialist knowledge to case conferences / clinical meetings and discharge planning meetings as appropriate, and to make referrals direct to social services / other agencies as required.

5. Liaise with voluntary services within the Hospital and Trust.

6. To assist clinical nurse managers and others in providing information so that the

health needs of people with learning disabilities are reflected within RCHT priorities, ensuring that the delivery on National Service Frameworks, local implementation strategies, local care pathways are inclusive of their needs.

7. To work with RCHT IT managers to ensure that people with learning disabilities and

people with ASD are consistently identified within their acute care pathway. This identification will inform operational decisions, prompt the involvement of the Acute Liaison service and assist with audit and incident management when necessary.

8. To collate and manage key information (activity, reasonable adjustments, outcomes

etc) and produce a bi-annual report that will inform contract monitoring and commissioning decisions by NHS Kernow.

Medical Examination The appointment is subject to a satisfactory medical examination. This job description represents the current expected role of the Learning Disability Liaison Nurse. It may be modified through discussion and negotiation as the organisation develops. The job description will be reviewed twelve months after appointment and thereafter, not less than every three years.
