Mission Pathways Health Centre for Children leads in

the provision of a continuum of coordinated

family-centred services for children and young

adults in Lambton County who have physical,

developmental and communication needs.

What is the Goal of the Service?

The ultimate goal of this service is to help

parents choose the proper Early Childhood

Education services for their child’s needs and

therefore having a successful transition into

school.

Who can refer?

Anyone can make a referral – parents,

guardians, childcare providers, family

physicians, paediatricians and other

professionals. Parents must be aware of and

support the referral.

How do you refer a child?

To start the referral process call Pathways

Health Centre For Children at 519-542-3471 or

print the Request for Service form from our

web site and drop it off at Pathways. You will

be asked to sign consent for release of

information form. We will then set up a time

for screening and observation for your child, in

a comfortable environment.

What are the requirements?

Your child must be registered for placement in

a child care or licensed home day care within

Lambton County.

Who is eligible for the service?

Children who are eligible will be fully assessed for

ECE Resource Support, and their families will be

invited to a conference to develop the Service

Plan.

What is the Service Plan?

The Service Plan is a document that lays out the

type and amount of resource support planned

for the child, and the setting in which it will be

offered. This plan is based on the family’s

choices & priorities, a combination of

classroom observation, resource and clinical

assessments that provides a comprehensive

picture of the child’s overall general

development. Parents sign off on this plan

once they are fully informed about it and in

agreement with it. This Plan is reviewed at

least once every six months.

Transition to School

Planning for a child’s successful transition

to school is a very important part of the

Service Plan. Early Learning Resource

Teachers work with families, clinical staff

at Pathways Health Centre for Children

and the school board to ensure that the

supports are in place when the child starts

school (JK, SK, or Grade 1).
