WhoisEligibleforAssistedLiving orMemoryCare?
Typically,assistedlivingcommunitiesserveseniorsintheirretirementyears.Individualsmayenteranassistedlivingcommunityfromaprivateresidence,hospital,rehabilitationcenter,ornursingcommunity.Theadmissionsprocessisthesameregardlessoftheperson’slocationpriortoadmission.
Eligibilityisbasedonanindividual’srequiredlevelofcare(acuity)andtheirabilitytopayforservices.Alsocalledresidentialcare,assistedlivingisatypeoflivingarrangementinwhichpersonalcareservicessuchasmeals,housekeeping,transportation,andassistancewithactivitiesofdailylivingareavailableasneeded.Alsoanimportantaspectoftheassistedlivingmodelistoprovidesecurity,comfortandmeaningfulactivitiesforresidents.Butunlikenursinghomes,residentsinassistedlivingremainindependent,livingontheirowninaresidentialsetting.Assistancewithactivitiesofdailylivingmayincludehelpwithbathing,dressing,toileting,diapering,medicationmanagement,helpingwithdailylivingdecisionsandmovingfromoneplacetoanother.
Assistedlivingisdesignedforpeoplewhoneedhelpwiththedailyactivitiesdescribedabove.Itisnotappropriateforpeoplewhoneeddailyskillednursingservices.Ifanindividual’sneedscannotbemanagedinanassistedlivingcommunity,anursinghomemaybeamoresuitablesetting.
Choosingtherightcommunityisanimportantprocesssincethefacilitymustprovidetheappropriatelevelofcare,meettheapplicant’sneeds,beaffordableandhaveanopeningwhenyouneedit.

WhenisAssistedLiving/ MemoryCareappropriate?
Around70%ofadultsovertheageof65willneedlong-termcareatsomepointintheirlives,accordingtotheU.S.DepartmentofHealthandHumanServices.Whenourlovedonescannolongerliveindependently,that’swhencaregiversarefacedwiththedifficultdecisionofwhattypeofcaretoselect—becausenotalllong-termcareoptionsarecreatedequal.
Ifyourlovedoneisstillactiveandvital,butcannolongerliveindependentlyandneedsassistancewitheverydayactivitiessuchasbathing,dressingoreating,oneoftheoptionsavailabletohimorherisassistedliving.
AccordingtotheAssistedLivingFederationofAmerica(knownasArgentum),assistedlivingisdefinedas“along-termcareoptionthatcombineshousing,supportservicesandhealthcare,asneeded.”Personalcareservicesinassistedlivingoftenalsoincludemedicationmanagementandtransportation,andcareisavailablearoundtheclock.Seniorsinassistedlivingusuallyhavetheoptionofaprivateroomorsharedspaceinanapartment,dependingontheirbudgetandpreferences.
Asyourlovedoneages,theymaysimplyneedalittleextrahelpwitheverydaytasks,ortheymayrequiremorespecializednursingcareiftheyhaveage-relatedissueswithmobility,orconditionssuchasdementia.Assistedlivingcommunitiesvaryintermsofwhatservicestheyoffer,butsomeofthemdoprovidememorycareservices,usuallyinadementiaspecialcareunit(SCU).
AssistedLivingvs.MemoryCare
Althoughassistedlivingcommunitiesmayhavememorycareunitsonthepremises,thetwotypesofcarearenotsynonymous.
Memorycareisadistinctformoflong-termcarethatspecificallycaterstopatientswithAlzheimer’sdisease,dementiaandothertypesofmemoryproblems.Alsocalledspecialcareunits(SCUs),memorycareunitsusuallyprovide24-hoursupervisedcarewithinaseparatewingorfloorofaresidentialfacility.
Aswithassistedliving,ifyourlovedoneisnolongerabletocareforhimorherselfduetoprogressiveimpairment,memorycareoffersaresidentialsolution.

SignsthatYourLovedOneMayNeedMemoryCareServices
Morethan16millionAmericansdevotetimeandenergytocaringforalovedonewithAlzheimer’sdiseaseorotherdementia,accordingtothe Alzheimer’sAssociation1,butsometimesthecostofcaregivingbecomestoohigh.Caregiversoftenfindthemselvesunabletobeartheburdenofprovidinghomehealthcarewithoutsufferingfromillnessthemselves.
Thisiswhenitmaybetimetoconsiderwhethertomovealovedoneintoassistedlivingiftheirhealthneedsbecometoomuchtohandleathome.
Movingafamilymemberisneveraneasydecision.Thereare,however,sometelltalesignsthatcaregiverscanlookforinordertorecognizewhenit’stherighttimeforassisted living:
Aggression Physical,sexualorviolentaggressionfrequentlyhappeninthosewithdementia,andcaregiversorotherfamilymembersmaysufferorbegintofeelresentful.
Caregiver Stress Caregiversymptomslikeincreasedstresscanbejustastellingasignasthedementiabehaviorsdescribedabove.

Home Safety Consideryourseniorfamilymember’shealthandyourownabilitiestocareforthem.Isthepersonwithdementiaunsafeintheircurrenthome?
Sundowning “Sundownerssyndrome”—veryagitatedbehaviorthatbecomesmorepronouncedlaterintheday—isacommoncharacteristicofthosewithAlzheimer’s.Thiscantakeaheavytolloncaregivers,andwhenitbeginstoseverelydisruptfamilyroutines,thismaybeasignthatthecaregivingburdenistoohardtohandle.
Wandering Inlaterstagesofdementia,theriskposedbywanderingbecomesmuchgreaterandtheprobabilityoffallsandinjuriesincreases.

RitaVasquezattributesthesesymptomsnotonlytothepressuresofcaringforsomeonewithdementia,butalsotothedisruptionstonormaleatingandsleepingpatternsthatresultwhenoneisspendingsomuchtimeoncaregiving.“Whenthebrainisalwaysonalert,manythingsaregoingtohappen—you’renotgoingtoeatwell,yournutritionisgoingtogodown,andphysicalhealthsuffers.”
Theemotional,mentalandphysicaltollofcaregivingcanbeparticularlypronouncedforspousesofthosewhoneedcare.InoneofthefamiliesVasquezworkswith,thewifeandprimarycaregiveris80yearsold.“She’stakingcareofher85-year-oldhusbandandit’sdrainingher,”Vasquezsays.“Whenhefellrecently,shecouldn’tpickhimupandhadtocalltheparamedics.”Incaseslikethis,itmightbeclearimmediatelywhenthedemandsofcarebecometoogreat.Inothercases,itmightnot be so obvious.
Ifyouarefeelingisolatedandalone,orifyoubegintofeelresentfulofyourlovedone,itmightbetimetoexaminethesourceofthosefeelings,saysVasquez.
“Anger,resentment,sleepdeprivation–allthosethingswillbecomepartofwhathappenstoacaregiver,”shesays.“Ofcourse,theguilt,whenyouthink,‘I’mnotdoingenough.’”Whenthathappens,itisimportanttorecognizehowmuchyou’vebeengivingtoyourlovedone,andperhapstellyourself,“Okay,I’mnotlivingalifeformyselfanymore,I’mlivingforthatperson.”

Asyou’reweighingseniorcareoptionsforyourlovedone,costislikelyatopconsideration.Dependingonwhatcareyourlovedoneneeds,assistedlivingcanbemuchmoreaffordablethannursinghomecareorlong-termin-homecare.Assistedlivingrentsvary,butyoucangenerallyexpecttopay$3,000to$5,000permonth(comparedto$5,000to$10,000andupfornursinghomes).Ifyourlovedonedoesn’tneedclosemedicalsupervision,assistedlivingmightbeyourbestbet,financiallyspeaking.
But how will you pay for assisted living? Here are nine creative ways to afford assisted living that you may not have thought of yet.
Important note: MedicarewillNOTpayfor assistedlivingbeyondshort-termrehabilitationservices.
1 PayforAssistedLivingwithVeteransBenefits
Ifyourlovedone(oryourlovedone’sspouse)wasaveteran,you’reinluckwhenitcomestoresidentialcare.Veteransbenefitscanbeusedtopayforresidentialcareinavarietyofsituations.Onesetofbenefitsisavailabletothosewithservice-relatedinjuriesordisabilities;anothersetofbenefits,knownasAidandAttendance,isavailabletoanyveteranorsurvivingspousewho’sdisabledandwhoseincomeisbelowacertainlimit.Toqualifyforandaccessthesebenefits,you’llneedtogothroughtheVeteransAdministration,whichcanbeatrickyandtime-consumingprocess.It’sextremelyhelpfultoworkwitha
geriatricplannerwhoknowstheinsandoutsofthesystem.Manyseniorlivingcommunitiesofferafinancialconciergeservicethatcanincludeguidingyouthroughtheprocessofqualifyingforbenefits.
AnotheroptionistoworkdirectlywithservicessuchasElderlifeFinancial,whichworkswithassistedlivingandcontinuouscareretirementcommunities(CCRCs)toprovidethisconciergeservice.ElderlifeFinancialconnectsyouwithitsnetworkofveteransbenefitsexperts,whocanhelpobtainthemaximumbenefitsyourloved one is entitled to.

2 HowtoUseaLifeInsurancePolicytoPayforAssistedLiving
Ifyourlovedonehasalifeinsurancepolicy,heprobablypurchaseditlongago,thinkingtoprovidesupporttohisfamilyafterhisdeath.Butalifeinsurancepolicycanalsoprovidefinancialsupportnow,ifthat’swhenthemoneywouldbemosthelpful.Tocashoutapolicy,askyourlifeinsurancecompanyabout“accelerated”or“living”benefits.Commonly,thecompanythatoriginallyissuedthepolicybuysitbackfor50to75percentofitsfacevalue.Theamountisdecidedbasedonthepolicyamountandmonthlypremiumsaswellasthepolicyholder’sageandhealth.Differentrulesmayapplydependingonthecompanyandtypeofpolicy.Forexample,somepoliciescanonlybecashedinifthepolicyholderisterminallyill;othersaremuchmoreflexible.
Ifthecompanythatissuedthepolicywon’tcashitin,don’tworry.Yourlovedonecanalsosellthepolicytoathird-partycompanyinreturnfora“lifesettlement”or“seniorsettlement,”whichisusuallyalumpsumof50to75percentofthepolicy’sfacevalue.Afterbuyingthepolicy,thesettlementcompanypaysthepremiumsuntilthepolicyholderdies,atwhichpointthecompany,ratherthanthepolicy’soriginalbeneficiaries,receivesthebenefits.Anotheroption,knownasa“lifeassurance”benefitorlifeinsuranceconversionprogram,allowsseniorstoconvertthebenefitofalifeinsurancepolicydirectlyintolong-termcarepayments.Lifeinsuranceconversiontypicallypaysbetween15and50percentofthevalueofthepolicy—lessthanalifesettlement—butisavailableforlesser-valuepoliciesthatmightnotqualifyforlifesettlement.
3 HowtoUseLong-TermCareInsurancetoPayforAssistedLiving
Ifyouoryourlovedoneboughtcareinsurance,you’reoneoftheluckyones.Long-termcareinsurancepoliciesapplytoassistedlivingcare;allyouneedtoknowishowtocollectonit.Somelong-termcarepolicieshaveaspecificdesignatedbenefitfornursinghomecare,basedonamentalorphysicaldiagnosis,whichcanbeusedtopayforassistedliving.Orthepolicymaysetadesignatedpaymentforhomecare,whichcanbepaiddirectlytothe
assistedlivingcommunityortothebeneficiary,whothenusesittopayforassisted living.
Onemorething:Ifyourlovedonedidn’tbuylong-termcareinsurance,it’sprobablytoolatenowtoconsiderthisoption.Butthereistimetosignupforalong-termcarepolicyyourself,soyoudon’tputyourownfamilyinthesamepickleinthefuture.

4 HowtoUseanAnnuitytoPayforAssistedLiving
Ifyouhaveanesteggbutyou’reconcernedaboutoutlivingyourresources,anannuitymaybeagoodoption.Whenyoupurchaseanannuity,youpayalumpsumupfront—andreceiveregularpaymentsbackoverapromisedperiodoftime,usuallytherestofyourlife.Anannuitycanhelpyoustretchyourbudgetandbesurethatyou’llalwayshaveatleastsomemoneycominginevenifyoulivelongerthanyouexpect.
Thebigbenefitofannuitiesisthatyoucontinuetoreceivemoneyregularly,evenifyourpurchasepremiumrunsout.Ifyouliveareallylongtime,yougetmorebackthanyouputin.Theunderwritertakestheriskthatyoumightlivelongerthanthemoneylasts—andmakesanextraprofitifyoudieearly.Underwritersdon’tgointotheannuitybusinessexpectingtolosemoney,butannuitiescanstillbeabetterdealforyouthanjustconsumingyourmoneyyearbyyear.
Anotherbenefitisthatannuitiesaren’tfullycountedasassetsbyMedicaidwhenyouapplyforgovernmentassistance.Theincomefromtheannuityiscountedasa“resource,”butthemuchlargersumoriginallyusedtopurchasetheannuityisnot.
Annuitiesarecomplexfinancialtools.Therearemanyvariations.Someyoubuynowtogetfuturepayments,othersdeliverimmediatepayments;somearebasedonafixedinterestrate,othersworkoffvariablerates.You’llwanttodosomehomeworkandtalktoatrustedfinancialadviseraboutwhatannuityoptionsmightbeappropriateforyoursituation.
Beverycautiouswheninvestinginannuities.Thereareunscrupulousmarketingschemespushingphonyannuitydealsthattargetvulnerableseniorsthroughcommunitycenters,adulteducationseminars,telemarketing,andslantedadvertising.Andoutrightannuityfraudismorecommonthanmostpeoplerealize.Alwaysuseyourcommonsensefilter:Ifitsoundstoogoodtobetrue,itmightbeascam.You’llwanttochooseareputablecompanywhenyoubuyanannuity,andworkwitharepresentativewhocomeshighlyrecommended.Andmakesureyourrepresentativehelpsyouthinkthroughsomeofthetrickierdetails,likeinflation.

5 HowtoUseaReverseMortgagetoPayforAssistedLiving
Ifyourlovedonesowntheirhomeoutrightorhaveonlyasmallmortgageonit,areversemortgagemightbejustthesolutionyou’relookingfor.
Areversemortgageallowsyoutocashoutthevalueofyourhomeequity,eitherinalumpsumorinaseriesofmonthlypayments.Thebankdecidesonavaluebasedonwhatthehomeisworth,interestrates,theapplicant’sage,andotherfactors,andtheloanbalancegraduallyincreasesovertime.(Ifabankholdsamortgageonthehouse,ithastobepaidbackbeforeyoucanbeginreceivingpayments.)Theborrowercanstayinthehomeuntildeath,eveniftheloanbalanceexceedsthehome’sworth.Upondeath,theloanbalancemustberepaid,whichusuallymeansselling thehome.
Reversemortgageswereoriginallydevelopedtohelpwidowsremainintheirhomesafterthebreadwinnerpassedaway.Todaytheyworkbestwhenoneparentneedsassistedliving
buttheothercanremaininthehome.Toapplyforareversemortgage,onehomeownermustbeovertheageof62,andonepersonmustcontinuetoliveinthehome.
Besuretodoyourhomeworkabouttheprosandconsofreversemortgages—theyaren’tforeveryone.Forexample,it’sprobablynotagreatchoiceforabelovedpropertythatyouwanttokeepinthefamily.
Finally,areversemortgageisabigcommitment,soit’simportanttoworkwithareputablecompany.Makesureyouunderstandthetermsandreadthefineprint,astherearemanyrulesabouthomeowners’insuranceandmortgageinsuranceandkeepingthepropertywellmaintained.Theremayalsobehighfeesinvolved,orclausesthatmakeiteasytolosethehome.TheConsumerFinancialProtectionBureaurecentlyreportedthatreversemortgagescamsandforeclosuresareontherise.
6 RentoutYourHometoMakeAssistedLivingMoreAffordable
Ifonlyoneparentisstillliving,orifbothparentsneedassistancewithdailyliving,thefamilyhomecanbeanimportantresource.Sellingisanoption,ofcourse.Butinmanyfamilies,MomandDad’shouseischerishedandfamilymembersaren’treadytomakethis
decision.Inthiscase,considerrentingoutthehouseandusingtherentalincometopayforassistedliving.Theideaofbeingalandlordmightseemscary,butforapercentagefeeyoucanhireaservicetomanagethepropertyforyou.

7 HowtoUseMedicaidorSSItoPayforAssistedLiving
Ifyoudon’thavemuchinthewayofsavingsorotherfinancialassetsandyourincomeislow,youmayqualifyforgovernmentassistancetopayforassisted living.
StartwithMedicaid,whichisrunasapartnershipbetweenthestatesandthefederalgovernment.Inmanystatestheprogramsgobyanothername,solookupthenameofyourstate’sprogramonlineorinthegovernmentpagesofyourphonebook(example:ArizonaLong-TermCareSystem).Medicaideligibilityisdifferentfromstatetostate,buttypicallyyoumusthavelessthan$2,000inassets,inadditiontoyourhomeandyourcar,inordertoqualify.
OnlysomeassistedlivingcommunitieswillacceptMedicaid,andMedicaidbedsareusuallylimited.Tofindlong-termresidentialcareoptionsnearyou,checkwithyourlocalAreaAgencyonAging.Tohelpyounavigatethemazeofsigningupforpublicbenefits,youcanalsocallforafreeconsultationfromaGovernmentHealthInsuranceCounselor.
Importantnote:BewareoftryingtoqualifyforMedicaidby“gifting”moneyandotherassetstoadultchildrenorotherfamilymembers,alsoknownas“Medicaidspend-down.”Thisonce-
popularstrategyisn’taseasyasitsoundsandcanbackfirebadly.ThegovernmenthasbecomeincreasinglystrictaboutMedicaidqualificationandhastherighttodoa“look-back,”goingoveryourfinancialtransactionsforthepastfiveyears.Anygiftsofmoneyorassetsmadeduringthistimearecountedasresources,includingassetsputintoanirrevocabletrust.Ifyou’recaughttryingtospenddownyourresourcestoqualifyforMedicaid,thepenaltiesaresteep—includingdisqualificationfromreceivingMedicaidforalengthyperiodoftime.Consultinganelderlawattorneyearlyintheprocessishighlyrecommended.
Ifyouhaveadisability,anotheroptionissupplementalSocialSecurityincome(SSI).Alsoadministeredbythestate,SSIispartofthegovernmentalsafetynetforthosewhoareimpoverishedandpartiallyortotallydisabledbyillnessorinjury.SSIcomesintheformofmonthlypayments,whichyoucouldusetopayfornursinghomecareorassistedliving.ToqualifyforSSI,contactyourstatedisabilitydepartment.You’llneedtodocumentyourfinancialstatusandyou’llalsoneedadoctortocertifythatyoucan’tworkbecauseofamedicaldisability.

8 HowtoPoolFamilyResourcestoPayforAssistedLiving
Ifyou’reworriedaboutMomorDadlivingalone,otherfamilymembersmaybeworried,too.Gettingeveryonetogethertotalkaboutitsometimesmakesitpossibletofindasolution,suchaspoolingassetsandtradingmoneyfortime.Forexample,ifoneortwosiblingsorfamilymembershandlethebruntofdailycare,suchasdrivingtomedicalappointments,otherswithlessflexibleworkschedulesmightcontributemoneyinstead.Orifthere’safamilyhomethatnoonewantstosellyet,siblingswithavailablefundsmightpayforassistedlivingwiththepromiseofrepaymentwhenthehouseissold.
Theresearchandpaperworkassociatedwithfindingandchoosingamongassistedlivingcommunitiesand
qualifyingforfinancialsupportisabigjob.Sometimesfamiliesgetstuckbecausenoonefeelsqualifiedtotakeonthetask.Itcanbeahugerelieftoworkwithageriatriccaremanagerorseniormovemanagerwhoknowstheresourcesinyourarea.Acaremanagercanworkwiththeentirefamilytopresentoptions,resolveroadblocks,andhelpyoufindtheperfectsituationforyourlovedone.
Moneymatterscanalsobringupfamilytensions.Ifyou’rehavingtroublecommunicatingaboutthischallengingtopic,speaktoaseniorlivingconsultantorgeriatriccaremanagerabouthowtohandlefamilyconflicts.Youmightalsoenlistthehelpofaprofessionalmediator.

9 PayforAssistedLivingWithaBridgeLoan
Ifyourlovedonedoesn’thavealotoffreecashorfinancialassetsthatareeasilyliquidated,theanswermightbeabridgeloan,anoptiondevelopedbyElderlifeFinancial.Bridgeloansareshort-termloansofupto$50,000designedspecificallytoprovidethefundsforamovetoassistedlivingoracontinuingcareretirementcommunity(CCRC).Theycomeintwotypes.Thefirsttypeisanunsecured(nocollateralrequired)lineofcreditintendedtofinancethefirstmonthsoflivingexpenseswhileseniorsselltheirhome,obtainveteransbenefits,ortakeotheractionstofreeupfunds.
Interestratesfortheselinesofcreditrangefrom8.25to12.5percent,sothisoptionisbestusedwhenthetimetopaybackisrelativelyshort.(Borrowershaveuptofiveyearstorepay,butmostrepaytheloanswithinayear,accordingtothecompany.)
Thesecondtype,calledtheCapitalAccessProgram,isalower-interest,lump-sumloansecuredbyrealestateoranotherassetthatthecompanyrecentlyintroduced.It’sdesignedtohelpseniorscomeupwiththelargeup-frontentrancefeetypicallyrequiredforaCCRC.
