2012 Financials

More than 4 decades ago, a group of committed parents of children with intellectual disabilities (I/DD), advocates and community leaders in the Lawrence area had a dream to start an organiza-tion that would provide community based services for adults with I/DD. Cottonwood opened its doors at our location on 31st Street in June, 1972 with thirty persons receiving supports. Since then we have grown to serving nearly 600 individuals daily through a broad array of services and supports. We honor those founding families and community leaders for their vision and determination so that today it is very natural to see persons with intellectual disabilities work-ing in the community and living in your neighborhood. Thank you to the citizens of Douglas and Jefferson counties for your support and your generosity in this our 40th anniversary year.

Support Services

301 persons received Case Management Services from Cottonwood in 2012. Overall 100% of parents and guardians expressed that they were very satisfied or satisfied with case management services from Cottonwood. Overall 98% of persons served said that they were happy or very happy with their case management services. Health Supports continued to provide an amazing amount of support for individuals served by scheduling 6,960 appointments to medical pro-viders in 2012 to help maintain optimum health for persons served.

Residential

In the past year, 112 persons were served in 51 different living arrangements. Number of pets served by Residential is 13! (We believe that helping people support their pets is a valued service to individuals in our Residential program.)

JobLink Employers of the Month for 2012

In 2012, persons who participated in Work Services ex-pressed 96% satisfaction with their jobs and the work provided. 96% of businesses who used Work Services to complete a job expressed satisfaction with the completed project.

97% of consumers who receive JobLink services had an average score on the Personal Satisfaction Survey of 2.5 or better on a 3 point scale. 313 persons are actively being supported by JobLink. The overall average wage for persons with jobs in the community is $8.25.

Life Enrichment Services

The persons supported in Life Enrichment Services participate in Work Enrichment and/or in Retirement. 37 persons partici-pated in Work Enrichment and 26 persons were in the Retirement program. The av-erage overall satisfaction score of partici-pants in Life Enrichment Services was 89%.

Community Developmental Disability Organization (CDDO)Cottonwood CDDO is the single point of entry for application, eligibility deter-mination & referral for developmental disability services in Douglas and Jef-ferson Counties. In 2012, thirty-seven individuals applied for DD services. Of those applicants, 19 individuals were determined eligible.BASIS (Basic Assessment and Services Information System) screenings are com-pleted by the CDDO Specialists. The year 2012 began with 724 individuals in BASIS and that number decreased to 634 individuals by the end of December, 2012.

Cottonwood Classies

Cottonwood Classic Golf Tournament & MinimastersThe golf tournament was again held at Alvamar Public Golf Club May 18th. 22 teams participated and all had a wonderful time!The MiniMasters, our annual miniature golf tournament for persons served, families, staff and community was held at The Lodge in Baldwin City on May 24th. Over 200 persons served, staff and families enjoyed the evening of fun.

The 11th annual Cottonwood Classies was held on April 30, 2012 at Free State High School Auditorium. The 2012 Classies celebrated the 40th anniversary of Cot-tonwood, Inc. Over 500 persons enjoyed honoring the award recipients and cel-ebrating those who have made Cottonwood such an integral part of our community.

Salute! A Festival of Wine & FoodThe 14th annual Salute took place on July 12-14. This 3-day event was again a wonderfully successful fundraiser for Cottonwood. Over $114,00 was raised to benefit the mission of Cottonwood.
