The Preston district includes a social and urban rural mix that is demographically representative of the UK generally. Community mental health resources are located in the town and in nearby rural areas.Being a university town, Preston has a thriving nightlife. Accommodation will be available within easy travelling distance of the Academic Centre. Transport for students from the Academic Centre to community facilities are provided by the Trust.

6. IT & Library Facilities The Guild Academic Centre has excellent Library facilities: 40 current health and psychiatry journals, and a book-stock of over 2,500 catering mainly for multi-disciplinary and community mental health staff.It is equipped with eight computers with word-processing facilities, an Internet connection and CD-ROM facilities to search MEDLINE, etc.

Specific Information about Module

7. Short Title Community Psychiatry. Assessment and management of wide range of mental disorders in community. Visits to community resources. Special interests ranging from old age psychiatry to forensic in prison mental health service.

8. Aims and Objectives To provide students with clinical experience through participation in the assessment and management of acute and long-term mental illness in the context of a multidisciplinary community psychiatric service.

In addition there will be opportunity to explore:- team work and individual roles, - the interface of the psychiatric service with the community,-a special aspect in depth.

9. Programme & Methods 9a. General Description

Students will be encouraged to become part of the team providing care in the community by following-up patients after their initial assessment or discharge from hospital.This will be supplemented by a series of visits to community resources, each of which can be extended as a Special Interest according to the individual student’s needs

8. Aims and Objectives To give students clinical & organisational experience of all aspects of a community paediatric service in a large city. In particular to: (1) explore the process of diagnosis & management of children with disability.

(2) Understand the difficulties and concerns of families with such children. (3) gain insight into preventative services in child health. (4) experience multi-agency management of complex cases.

9. Programme & Methods 9a. General Description

Students will see children & families alongside doctors, HVs, school nurses, etc. in the clinic, school & home settings. They are encouraged to follow children through referrals to hospital & see them at home.Each of the consultants has special interests and students will have time to look in more depth at one area of work: health surveillance, educational medicine, disability. Opportunities to take histories & examine children under supervision.

8. Aims and Objectives This module is based on the resources available within PCT.

Aim is to provide practical clinical experiences to see patients in a variety of settings & to offer opportunities within the community. The Programme will be learner centred in approach.

8. Aims and Objectives This module gives a broad clinical experience with opportunity to study areas in depth.

The development of consultation skills, holistic care of patients. Putting evidence into practice, the use of clinical guidelines and audit. Multidisciplinary primary care team working

Students will be encouraged to develop own list of patients and see them when they attend in surgery and on visits. During supervised surgeries students will be taught on consulting skills, in particular focussed history-taking, formulation of diagnoses.Particular emphasis is put on chronic disease management, audit and public health through in-depth study. Timetables flexible and can be adapted to student needs with ample personal study time.

8. Aims and Objectives The main emphasis of this module is to provide clinical experience of attending patients in a wide range of community settings.

Students will have the opportunity to see patients in a GP setting and to follow patients through their multi-agency contacts. To explore and participate in patients health problems and their management.

9. Programme & Methods 9a. General Description

Danebridge Medical Centre is a large fully equipped general practice. Northwich Infirmary is a large Community Hospital with no resident staff.In the Hospital there is a Minor Injuries Unit, full outpatient services, inpatient rehabilitation beds, full rehabilitation services, investigative facilities and it is the base for all the community Services and Voluntary Services within the area.

Opportunities for shadowing GPs & GP registrars F2 and other health professionals. Also students will be able to see inpatients in Nursing Homes etc.

8. Aims and Objectives Opportunity for students to gain experience of all aspects of primary care. Serving a rural community we provide a wide variety of in-house services, some of which students may wish to benefit from; physiotherapy, chiropody, anti-coagulant clinic, minor surgery, family planning, child health surveillance.

Special interests in the practice include women's health & family planning, dermatology, cardiology, minor surgery, general medicine (endocrinology, rheumatology, gastroenterology).

9b. Arrangements for Shadowing AND Out of Hours Work

The main programme is supervised mini-surgeries (4-6 patients, 30 min appointments), booked in advance where you will see patients with full support and supervision from one of our GP partners. Also undertake home visits, again with supervision and support. Family planning and baby clinics, minor surgery, out of hours sessions as wish/need. We have friendly patients, fantastic staff, a long track record in education and super scenery. What more could you want from a placement!

Students will be invited to consult patients for initial assessment early on in their attachment and present each case to the GP supervisor.They will be encouraged to formulate a management plan, discuss treatment and make appropriate referrals as required. Students will visit patients at home for initial assessment, including the elderly population, nursing home residents etc.

8. Aims and Objectives We recognise there is a vast pool of teaching/learning opportunities in the practice. We cover a wide range of disease. We are enthusiastic doctors & would like to show students how much responsibility we take & what an important job we do.

We want to stimulate students to consider General Practice as a career option.

The student would observe surgeries & accompany visiting Doctors for the first week or so. When they felt ready they would encourage to see patients on their own followed by a discussion with the Doctors who would confirm the students action was appropriateIn addition, the student could have sessions with each member of the Primary Health Care Team & attend various clinics. Student could attend meetings held at surgery. Will be regular review of progress.

All 6 doctors are prepared to be involved in teaching so each student would appreciate a spectrum of views & opinions. Surgery uses Chorley Out of Hours Medical Service. There would be ample opportunity for the student to attend appropriate no. of sessions.

7. Short Title Primary care in Sandbach a Cheshire Market Town.

8. Aims and Objectives To experience a full range of Community medicine in a state of the art new Primary Care Centre opened on 1st March 2004. Full participation in practice based activities, e.g. chronic disease management, antenatal care, minor surgery, child development clinics and Nurse Practitioner Clinics. Participate in community based activities, e.g. care of the housebound, home and hostel visits with full range of primary care team members. Draw on the specialist GPs' interests, such as diabetes and dermatology.

9. Programme & Methods 9a. General Description

Students will see & examine as many patients as practicable, have introduction programme to enable an overview of the whole care team & demographic aspects of practice area.Own supervised surgeries with routine & emergency appointments, observe & where appropriate assist in additional clinics e.g. minor surgery, asthma, diabetes, drug abuse, experience home residential care & Bail Hostel visits. Our extended role physiotherapists, gym and hydrotherapy complex will give the student an appreciation of how primary care is developing its services within the community it serves. We have Vasectomy, ENT, Urology and Ophthalmology clinics on site.

9b. Arrangements for Shadowing AND Out of Hours Work

Each student will have named GP who they will shadow when not attending other commitments. On-call is provided by the PCT led N.E.W. out-of-hours service, based at Leighton Hospital. This covers the local population from 6:30 pm to 8:00 am Monday to Friday and over the weekends. Separate arrangements with the N.E.W. service can be made on request.

8. Aims and Objectives Aim: To gain a good understanding of General Practice and the role of the practice team. Objectives: Learn consultation skills, diagnosis and management in General Practice. Be aware of the roles of all members of the Primary Health Care team. Understand the role of significant event audit and clinical audit in General Practice. Participate in Practice clinical meetings. Understand the need for home visiting. Experience out of hours care.

9. Programme & Methods 9a. General Description

Attachments to all members of the Primary Health Care team. Carry out supervised patient consultations. Shadow the duty Dr and see patients under supervision. Accompany doctors on home visits. Experience out of hours care. Take part in a significant event audit or clinical audit.

9b. Arrangements for Shadowing AND Out of Hours Work

Arrangements will be made for the student to shadow when out of hours. This could be a maximum of 1 session a week. However the student may only wish to do 2 sessions in total.
