AMENDMENTS • A photocopy ofl the Single Largest Completed Contract must be submitted i.e. similar contract completed within the last five (5) years prior to December 8, 2014, whose value must be at least fifty percent (50%) of the ABC supported by the Constructors P~rformance Evaluation System (CPES) rating sheet which must have a satisfactorY rating or a certificate of satisfactory cOl1lpletion of the project. • Tax Clearance Certificate from the Bureau of Internal I Revenue shall also be submitted as parf of the eligibility and technical documents contained in the first envelope. I • The List of Contractor's personnel to be assigned to th~ Contract must be supported by thelr respective updated resumes, photodopy of Professional Regulations Corrimission (PRC) valid license, DPWH Accreditation and Occupational and Safety Certificate, if applicable. 1.
