Currently, she leads the client services and sales teams for Phoenix Home Care & Hospice that serves four states. Most recently, Tara’s strategic initiatives have centered on ACOs, Bundled Payment, Value-Based Programs and developing a Chronic Care Management program. Tara continues to work closely with Acute Care and Post-Acute Care providers in creating successful

partnerships in the Accountable Care Environment. Throughout her career, Tara has had extensive experience developing relationships and promoting integration with acute, post-acute, and long-term care providers and third-party payers across the healthcare continuum.

Joan M. Doyle RN, MSN, MBA, Executive Director, Penn Home Care and Hospice Services, Penn Medicine, University of Pennsylvania Health System (UPHS)Joan Doyle is the Executive Director of Penn Home Care and Hospice Services. Joan has over 25 years of combined clinical leadership and executive management in the healthcare industry. Her expertise in health care extends from hospital clinical operations to startup of home infusion therapy services and development of home health care, home palliative care and hospice business lines. By offering a unique combination of clinical and operational strengths, Joan and her team

have achieved significant accomplishments and are recognized for their success in shaping and transforming the Home Care Services Entity at Penn Medicine. As Executive Director of Penn Home Care and Hospice, Joan is responsible for executive management of a home infusion company with a census of 1400, two home health agencies, two hospice agencies, and a home palliative program with a combined census of 2400 patients and two inpatient hospice units totaling 40 beds.

Katherine M. Follmer, R.N., B.S.N., M.B.A./HCM, Vice President, Care Continuum, Mercy Health (MH)Cathy Follmer has been in health care for 40 years. Presently she serves as Mercy Health Select’s (MHS) Vice President of Care Continuum. Ms. Follmer provides strategic and operation leadership for Care Transitions, Ambulatory Care Coordination and Post-Acute Services within MH, engineering innovative practice modalities and assisting in sharing best practices across regions. Ms. Follmer focus is on key initiatives within MH such as CMS Bundle Payment Initiatives; CJR and BPCI, utilization of services, decreasing length of stay, reducing avoidable hospitalizations, transitioning care,

palliative care and improving quality of services throughout the continuum. A key focus of her work is the transitions of care and alignment with physician practices, development of an integrated care coordination post-acute network.

Joyce MarkiewiczExecutive Vice President, Catholic Health, President and CEO, Catholic Health Home and Community Based CareJoyce is President and CEO of Catholic Health Home and Community Based Care in New York. Joyce has been a Health Care Professional for greater than 35 years with 25 years in Home Health Care. She currently leads over 1900 Professional and Para Professional staff, collaboration with multiple Physicians and Physician Groups ensuring the daily provision of care of over 3,000 patients each day. She is responsible to create a clear and unified vision for six distinct corporations each

with specific State and Federal regulations as well as leveraging the relationships and governance responsibilities of two joint ventures with enterprise wide revenues of over 150 million dollars.

In his current position, he provides operational leadership and strategic direction for all post-acute care services, including long-term care facilities, home health and hospice, palliative care as well as coordination of services with area health post-acute health care providers. Robert J. Sowislo, CFO,U.S. Medical Management and Visiting Physicians AssociationRobert J. Sowislo is the former Chief Financial Officer (CFO) of the Oakwood Health Care System (25 yrs.) and most recently CFO with USMM, LLC (USMM) and Visiting Physicians Association (VPA) (16 yrs.). USMM/VPA is a 240 Provider, Physician led, Primary Care Group dedicated to the delivery of Home Based medicine based in Troy, Michigan. He currently leads Government Shared Savings Program initiatives and Advocacy efforts for the USMM/VPA enterprise. VPA and its affiliate organization U.S. Medical Management, LLC serves over 60,000 housecall patients yearly in 14 states out of 42 offices across the United States.

Gary Swartz, JD, MPA, Public Policy Consultant, American Academy if Home Care MedicineMr. Swartz currently serves as Public Policy Consultant for organizations including the American Academy of Home Care Medicine, and for Chronic Care Management, LLC. In these roles, he provides public policy, regulatory and practice management advice to the Board of Directors, and to medical practices and organizations across the country. He participates in national coalitions in support of primary care services and alternative payment model development.

Patty Upham RN, BSDirector Care Transitions, FirstHealth of the CarolinasPatty has been the Director since November 1999. She serves as the overall leader and manager of Care Transitions which includes: two freestanding not-for-profit home health agencies; a complex care management program for patients not eligible for home health but who require ongoing support and management of their chronic disease; and the FirstHealth Center for Telehealth providing remote monitoring services across care settings.

Conference Cancellation/Transfer PolicyAll conference cancellations must be submitted in writing and faxed to The Remington Report at 949-715-1797. For conference cancellations received before January 12, 2018, the registration fee will be returned, less a processing fee of $150.00 per attendee. After January 12, 2018, no refunds will be made, however registration is transferable to another person from the same agency up until three weeks prior to the conference. We will NOT transfer registrations after this date under any circumstances.

After this date, the registration fee is forfeited. Cancellation requests not received by the date required will not be eligible for a refund. No shows will not be refunded or able to transfer to future conferences. There are no exceptions to our policy. Attendees may not pay for one attendee to attend the conference and bring two people to have one person attend one day and one person attend the next day. There are no substitutions. No refunds due to weather conditions. We do not accept walk-ins.

The Remington Report assumes no liability for non-refundable transportation costs, hotel accommodations or additional non-conference related expenses incurred by registrants.
