One of Marion’s other interests is knitting and we hear she has been busy working on Tea Cosies for the Community Day – come by and check out her creativity. Meet Marion Modra

Southern Cross Care Healthy &

Active Ageing Manager

Marion joined Southern Cross Care last year as

part of our Rehabilitation team. Marion is an OT

by background and has worked in aged care,

disability, community and rehabilitation settings.

Marion is surviving year 12 as her daughter

completes her high schooling – it seemed easier

Meet some of our other new staff

members

CHAN LAM- PODIATRIST (PRS, LVRS)

Chan our new Podiatrist graduated at the end of

2011 at the University of South Australia. She has

experience in Private Practice and Aged Care. Her

interests include diabetes foot care and orthotic

therapy. Outside of work she enjoys travelling,

trying new foods and has recently enrolled in a

hand building ceramics course. Chan works at

Lourdes Valley Rehab Services on Monday and is

at the Pines Rehab Services on Wednesday and

Fridays.

TOM LAM- PHYSIOTHERAPIST (PKRS)

HUY PHAM- PODIATRIST (LVRS, PKRS)

Huy is one of the new podiatrists at Southern

Cross Care and is based at the Phillip Kennedy

Centre, also traveling down to Lourdes Valley on

Wednesdays. Huy has worked in private practice,

aged care and podiatry locum services prior to

working in day therapy at Southern Cross Care.

He has interests all aspects of podiatry,

particularly in orthotic manufacture and

modification. In his spare time, Huy likes to

participate in sports and fitness and spending

time with friends.

“I have been a physiotherapist for 4 years and studied at UniSA. I have worked in residential aged care and private practice previously to starting at Southern Cross Care. I only work part-time at PKRS Day Therapy Centre as I also run my own private practice in Parafield Gardens. My interest include cricket, basketball, football (Port Power), badminton- pretty much all sports, travelling and eating. I have felt very welcomed by the team at PKRS and look very forward to working here in the future.”

Healthy ageing is important and a

right to all of us

Healthy Ageing- What is it?

Southern Cross Care (SCC) appreciates healthy

ageing as we believe it is a right for all individuals.

Ageing is not confined to the notion of “getting

older and being unable to do things for

themselves”. This historical view of ageing has

evolved quite rapidly and it is our belief that

everyone has a right to healthy ageing.

The World Health Organization (WHO) defines

health as

“a state of complete physical, mental and social

well-being and not merely the absence of disease

or infirmity.”

There are many definitions of healthy ageing and

linked terms such as active ageing and positive

ageing.

As there is no universal definition of healthy

ageing, WHO’s definition for active ageing is

“the process of optimizing opportunities for

health, participation and security in order to

enhance quality of life as people age: enabling

them to reach their potential for physical, social

and mental well-being throughout the life

course.”

In other words, healthy ageing allows people to

realise their physical, mental and social potential

regardless of age & disease processes.

My Aged Care (MAC) &

Commonwealth Home Support

Programme (CHSP) Update

Southern Cross Care is continuing to transition

our Day Therapy, Respite and Home and

Community Care Services to the new

Commonwealth Home Support Programme.

This has seen changes to our referral pathway

from the 1st July 2015, with the introduction of

My Aged Care. For new clients wanting to access

our services at Southern Cross Care, we can assist

with the referral process which can be done

online by service providers or by phone for clients

or family members.

When contacting My Aged Care to attend

services at Southern Cross Care you must specify

Southern Cross as the provider of choice.

Please speak to admin or therapy staff for

anymore information or assistance we can

provide.

A recent change is the Commonwealth Home

Support Fees Policy has been relinquished

following feedback from service providers and we

are awaiting a new Commonwealth Home

Support Fee Framework which we will then work

to advise clients and implement. With this delay

brings celebration as our Day Therapy Service cap

will remain in place and fees remain unchanged

until 1 November 2015.

For our existing clients, therapy continues as

normal and we look forward to assisting you

meet new goals and hit new targets during the

warmer spring months. Please keep a look out for

our new programs, including the commencement

of independent gym use via a membership.

World Physiotherapy Day

8 September 2015 Each year, on September 8, we celebrate World Physiotherapy Day, a global event celebrating the incredible role physiotherapists play in the community and the relationships that patients have with their physiotherapist.

The theme for this years World Physiotherapy Day is movement for health, fulfilling potential. Whether you’re in a rehabilitation centre, hospital or practice, join in the celebration. This global event is an opportunity to promote the crucial role physiotherapists plays in keeping populations well, mobile and independent. As the peak body representing the interests of Australian physiotherapists and their patients, we are passionate about championing rehabilitation. The APA represents more than 18,000 members who conduct over 23 million consultations each year. On World Physiotherapy Day, we promote awareness of the benefits of physiotherapy in the prevention, maintenance and treatment of injuries among Australians. Many people with long-term health conditions or disabilities lead happy and fulfilled lives. However, some do not because they do not receive the right kind of support. This waste of human potential has a cost to society, as well as individuals. Physical therapists are health professionals who have a key role in helping people with long-term conditions achieve their goals, fulfil their potential and participate fully in

society. They work with people to maximise movement and functional ability. If you have problems that affect your mobility, ask a physiotherapist, the qualified experts in movement and exercise.

Pilates

We can all benefit from any form of exercise but Pilates is a wonderful gentle programme that can:

Increase flexibility

Improve posture and prevent stooping Strengthening muscles (particularly trunk and lower back) which in turn can

Reduce back pain

Holistically benefits our mind, body and soul.

Aqua Therapy

As you may be aware, there have been changes to the scheduled aquatic pool times at Phillip Kennedy

Rehabilitation Services. There are now 3 different types of hydrotherapy sessions, which therapists have

assessed what session is best suitable for you. If you would like to commence one of these sessions, please

speak to reception about booking an assessment or contact the center on 8242 2985.

in Melrose Park. Please speak to reception about booking an assessment or contact the centre.

Chronic Pain and Relaxation Group

The group aims to promote the health and wellbeing of clients who have chronic pain by learning and practicing techniques that are proven help self-management of pain at home and in the community.

In the group, clients can expect to:

Practice strengthening and capacity building exercises with a Physiotherapist

Learn techniques to manage chronic pain with an Occupational Therapist

Understand basic mechanisms of chronic pain and how to get ahead

Private Services

The Pines Rehabilitation Service Gym Membership Clients can now access the gym within specified Gym Opening Times. You will need an assessment by a physiotherapist prior to attending the gym and a medical clearance completed by your medical practitioner to specify any health issues that may impact on exercise and advice of any particular activities that should be avoided. This is essential for all clients, however especially important with clients who have musculoskeletal injuries, arthritis, osteoporosis, diabetes, epilepsy, metastatic diseases or cardiopulmonary issues. Your physiotherapist will orientate you to the equipment and can provide you with an initial gym program. This program can be reviewed, modified and progressed by booking a follow-up apt when required. You can also take advantage of personal training services with a Rehabilitation Assistant who specialises in fitness and rehabilitation of the older adult. You can purchase a casual visit to our gym for $5, a monthly membership for $40 or a quarterly membership for $90 as per the fee schedule.

RESIDENTIAL CARE

We truly are becoming an organization that wants to provide opportunities for all to

maintain and/or improve their physical health as an important part of healthy

aging!

Early Intervention at Southern Cross Care Southern Cross Care supports active and healthy ageing for our customers. We have set up an Early Intervention Working Party (EIWP) that includes nurses, physiotherapists, lifestyle and allied health staff. The EIWP will meet once a fortnight to identify customers who could benefit from early intervention support due to being at risk of falls, frailty and social isolation. The staff will review customer’s circumstances individually and develop a person-centred recovery pathway to support them to regain their functional ability and social connections.

Fitness & Lifestyle Programs Southern Cross Care lifestyle programs will focus on building all residents physical and social abilities. Studies have shown that people of all ages and abilities can respond well to an exercise program including some resistance exercises. Southern Cross Care has made an investment in exercise equipment and staff training to offer individually-tailored exercise programs to our customers. According to the latest research too much sitting is the biggest contributor to the physical decline that many older people experience when they move into residential care. Southern Cross Care is reshaping our lifestyle programs to offer more opportunities to all our residents to move more and sit less, including through group exercise activities, walking groups and individual resistance programs using the exercise equipment.
