Income Verification (Needed for Pre-K Registration ONLY)

School Entrance Health Form

Photo ID of Parent or Legal Guardian

Proof of Residency (2 documents listed below are required) Copy of lease or rental agreement with current address and

parent/guardian name, lease or rental agreement must contain letterhead;

Copy of deed with current address and parent/guardian name; Current

Real Estate tax receipt with current address and parent/guardian name;

Previously issued W-2 or Form 1099/ pay stub with the address of the

residence; Sussex County property tax statement for the current year

which shows resident’s name and address; Voter registration

documentation from Sussex County with current address.

**ALL REQUIRED DOCUMENTS ARE NEEDED BEFORE CHILD

FOR POTENTIAL KINDERGARTEN STUDENTS: Children must be 5 years old by September 30th to be eligible to start Kindergarten this fall.

FOR POTENTIAL PRE-KINDERGARTEN STUDENTS: Children must be 4 years old by September 30th to be eligible to start Pre-K this fall.
