To receive the Verizon New 2 Year Line of Service Offer, the
following requirements must be met:
1. Purchase and activate any new Verizon Wireless Smartphone on
a New 2-year agreement in a Costco Warehouse and/or Costco.com.

2. Purchase Order and Activation must be made between the dates

If you qualify for this rebate, your $150 Costco Cash Card will
be mailed to the address provided above approximately 12-14 weeks
after Stuart Lee has received your rebate request. Other terms and
conditions: Offer valid on any new Verizon devices purchased
between 03/01/2015 03/31/2015. New 2- year agreement required
(Upgrades on existing Verizon Mobile Numbers are not eligible.).
Rebate is only valid if handset is purchased at Costco Warehouse
and/or Costco.com. All rebate forms must be postmarked no later
than the 30th day from the date you signed your qualifying Verizon
service agreement. Verizon service must be active at time of
redemption to qualify for rebate. If you qualify for this rebate,
your Costco Cash Card will be mailed to the address provided above.
