F ounding principlesAll treatment at LRBT should be totally free for thepoor so that no man, woman or child becomes blindjust because they cannot afford the treatment.

There should be no discrimination due to gender, caste, ethnicity, language, religion or sect.

Patients should be treated with compassion and dignity.

A series of catastrophes reduced Mubasher’s family tovirtual starvation. His father was diagnosed with kidneyproblems and lost his job due to his deterioratinghealth; his mother was asthmatic and often fell sick. There was suddenly no bread earner. His grandfatherwho had retired started supporting their family by

running a scrap material cart. But it was not enough for afamily of 6. Being the eldest Mubasher felt duty bound to

help augment the family income so he switched to theafternoon shift in his charity run vocational institute and took up a job as atrainer in a small driving school in the locality in the morning.

Unfortunately his troubles had not ended: his vision became hazy. On histeacher’s advice he consulted an ophthalmologist who told him that hewas suffering from Keratoconus in both eyes and needed corneaimplants. The cost was Rs 300,000. He was devastated to hear the newsas this was beyond his ability to raise. He had to share this with his familyas he was the only hope for their future and the key earner for his ailingparents and young siblings. All their hopes had suddenly shattered. Hedropped out from the institute and started getting serious warnings fromhis workplace. Mubasher shared his medical condition with his employerand requested assistance. The employer took him & his grandfather toLRBT where the head of the cornea subspeciality confirmed thediagnosis but advised that due to dearth of corneas in Pakistan and non-availability of local donors he would have to wait until cornea tissues werearranged. After an agonizing wait & fervent prayers they heard the goodnews that APPNA, the Association of Physicians of Pakistani descent inAmerica were donating corneas. His parents couldn’t believe their ears aswith passing time their distress and poverty had increased.

Mubasher had a successful corneal transplant in the right eye and will bereturning in future for the same operation in his left eye. Hope was onceagain in the air. The clouds of despair had lifted thanks to LRBT and itsfree services for the poor as there is no other institution in the countrywhich could have given them such complicated and expensive medicaltreatment free.

Meherman a 6 year old girl from Nushki a village nearQuetta had been facing eye issues since she was 4.Youngest of the 4 siblings she was the most pamperedand the apple of her parent’s eyes. Her father KaramKhan a daily wage earner and the only bread winnerfor a family of 7 members was very concerned abouthis daughter’s eye ailment but could not gather enough

funds to take her to a specialized facility. Randomhakeem’s, neighborhood doctors and their medications

resulted in exhausting their meagre saving as well as the loanthey took. Seeing his daughter’s pain, Karam Khan was extremelydistressed and felt totally helpless for not being able to get his daughtertreated. He was on the verge of nervous breakdown when his and thefamily’s prayers were answered. His uncle visiting from Quetta seeingMeherman’s condition offered to take them along and get Mehermantreated at LRBT Quetta. Upon hearing the good news, her poor mother’sface started glowing with happiness. Meherman had almost lost vision inher right eye due to a retinal problem. After initial examination the doctorreferred her to LRBT’s Tertiary hospital in Karachi specialized in Retinaland other eye diseases where poor patients were treated free of cost.

Her parents sold the only asset they had; her mother’s weddingornaments so they could travel to Karachi. After preliminary examinationshe was diagnosed as a case of perforation with severe eye infectionwhich was likely to result in total loss of eye. She was immediatelyadmitted as an emergency case and her retinal surgery (MIVS) wascarried out under general anesthesia. Thanks to the doctor’s expertise notonly the integrity of the eye ball was salvaged and at the same time partialvisual recovery was achieved. If this timely surgery had not taken placeshe could have faced complications such as glaucoma. Her eye wouldhave started to shrink too and would be disfigured for life.

Meherman with her eye saved has gone back to her village. Her father hadtears in his eyes while leaving LRBT, giving heartfelt thanks to everyone forsaving his daughter's vision for free.

8 Years old Kehkashan, a student of 2nd grade, is theyounger of the two female siblings in a family of 6. Her father is blind having lost his vision due to bilateralglaucoma a few years ago. Her mother, the solebreadwinner, works in a garment factory in Landhi andresides in a rented house.

Kehkashan, since childhood had bilateral developmentalcataracts. She was operated in a local clinic early in her life

but it was unsuccessful: loss of vision in her left eye and partial recoveryin her right eye. When she was 6 years old she suffered a second setback as the vision in her precious only eye deteriorated further. Hermother was desperate to find a cure for her young daughter. She hadnobody to turn too but luckily she saw a ray of light when a visitingmedical team of LRBT who used to come for annual eye checkup of thefactory employees informed her that LRBT Korangi Hospital is a freestate of the art hospital and will be able to treat her daughter.

Kehkashan visited LRBT Korangi with her mother where she wasdiagnosed as a case of retinal detachment and was operated on anemergency basis. 6 months later silicone oil was removed and there wascomplete visual recovery. Presently she is under regular monitoring andtreatment through anti-glaucoma medications free of cost for thesecondary glaucoma which had developed after her cataract surgery inearly childhood.

Kehkashan is happy to be back at school as she is now able to see withher right eye. Her mother was all praises for the LRBT team who salvagedand saved the eyesight of her young daughter, completely free of cost.

Razia, 45, is wife of a rickshaw driver. Her life has neverbeen a bed of roses. Struggles and challenges havealways been her lot. Her husband’s rickshaw got stolenand since that was their only means of livelihood, thefamily became dependent on loans. Her husbandstarted driving a friend’s rickshaw at night to survive.

But the few hours at night could not raise enough moneyfor the family to afford basic needs. He barely earned

enough to pay the rent and put food on the table for the family.

They had no children so Razia thought of ways to help her husband andimprove their financial condition. Razia tried her hands on many things butas she was not educated and had no skills, it was tough. One of the manythings she tried was making envelopes from old newspapers which got apositive response and she started selling them to a stationery store in thenearby market. Happy days had just started when she started feeling thather vision was failing, it became a concern for the entire family as not onlyhouse chores were her responsibility but her small contribution also madea huge difference in the living condition of the household. Her work alsohad picked up and she got orders for newspaper bags as well, but due toher eyes she was unable to concentrate and deliver on time.

As always for the poor in our country cost of treatment is the challenge. After finding about LRBT and its free services she visited with herhusband and was diagnosed with cataract. She got her sight back as hercataracts were treated a month apart and her vision was restored free ofcost. This gift has given her and her family a new lease on life.

Thank you, LRBT!

Munir, aged 20 lives in Surjani Town, a suburb of Karachi. His father drives a rented rickshaw which is the mainsource of income for the family. Seeing his fatherstruggle hard and stressing himself with overwork at hisage, Munir dropped out of vocational school and startedworking. He could not bear to see his mother struggleon a daily basis to measure the food while giving to each

child and many a times there was no food left for hisparents. Munir started working as a junior technician at a

generator sales and repair shop. He was an honest andhardworking boy and soon got the hang of his work. He would work doubleshifts to earn more and could see the difference his earnings brought tohis family. They were sad to see that he had to give up his education but atthe same time were happy to live better days.

One day while attending to a complaint an electric spark went into his eyeand he went blank for a few seconds. The eye was burning and to cool ithe washed it with cold water. Despite the pain he left it unattended. Hismother pointed out that the redness was becoming worse and he mustsee a doctor. But he ignored it knowing well he could not afford it. One daywhile returning from a call near North Karachi he passed by LRBT’s NorthKarachi Free Eye Hospital and he could hear his mother’s plea ringing inhis ears. He felt an unseen force making him enter the hospital.

In OPD the doctor examined him and advised him immediate cataractsurgery. He was given some pre-surgery eye drops and a date of surgery. He was also advised complete rest in the meantime as his nature of workcould worsen the situation. Lying at home he couldn’t bear the thought ofthe old days returning where there was dearth of food and his parentshaving sleepless nights due to poverty. He prayed to God to make him wellagain. He fought with himself to keep away from work during this time as itwould have adverse effects in the long run.

After two weeks of complete rest and free medications from LRBT his eyewas ready to get operated. His successful surgery and post-operative caremade his eye healthy again and he was finally able to resume work. Hisfamily’s ordeal was over and Munir is back at work with even more energy. As he owes his happiness to LRBT, he ensures to put Rs 50 or Rs.100 inLRBT donation box whenever he gets a call and passes from Korangi orNorth Karachi hospitals.

LRBT Annual Review

2018

We are now addressing new challenges which have appeared in eye health in

Pakistan. With diabetes assuming frightening proportion: 17 % of the population and

as 30 % of people who have uncontrolled diabetics will develop Diabetic Retinopathy

an irreversible blinding disease, we are taking measures to provide treatment as usual

free of cost for the poor. 10 of our hospitals have Medical retina clinics and 16

hospitals provide laser treatment.

The growth of patient volume and the addition of new sub-specialties in our Korangi

Centre of Excellence necessitates an expansion of the Hospital. Ms Khurshid Bhaimia

one of our loyal donors who had earlier funded the construction of the 2nd Floor of the

old Building and the building of the OPD complex has agreed to fund an additional floor

on top of the OPD complex. Construction will commence next year after necessary

building approvals. We are also considering rebuilding our Mandra Hospital as well as

finding a suitable amenity plot for making a purpose built hospital in the North of Karachi,

as despite the recent expansion our current hospital is unlikely to be able to cater to the

expected growth in patient traffic in the medium term.

The major upgradation of equipment continued to ensure that our patients, despite being

treated free, get state of the art treatment and we spent Rs 186 Million on it. Our

Hospitals especially our Centres of Excellence – Korangi & Lahore Kot Lakhpat have

equipment which very few eye hospitals in UK and US possess.

State of the art equipment is only one necessary condition for high quality treatment, the

other & perhaps more important one is staff training & skills development. We lay great

stress on this with continuous medical education programmes for both doctors and

paramedics. We also run the largest post graduate training programme in Pakistan in

ophthalmology for doctors at our 2 Tertiary Hospitals in Karachi & Lahore & Certificate &

Diploma courses for paramedics at Karachi. As a consequence our surgical outcomes

continue to exceed the WHO/ICEH bench marks by 7 percentage points.

Finally I would like to thank our generous donors without whom 39 million patients that

we have treated so far in our journey of 33 years would be suffering a life of darkness

and despair. We would also like express our gratitude to our volunteers, the media and

our staff whose devotion and dedication has made LRBT the largest provider of eye

care in the world.

OPD VISITS / SURGERIESDuring the year ending June 2018, 3 million patients visited our 19 hospitals and 56Community Eye Health Centers, a growth of 4% over the previous year. During the sameperiod 273,296 major & minor surgeries were performed. Growth in major surgeries was 2%over last year. Cataract, which is the leading cause of curable blindness constituted 90% ofmajor surgeries. Other surgeries included vitreo-retinal procedures, Trabulectomy, Corneatransplants, DCR, Squint etc.

16,083 retinal procedures were performed a growth of 15 % over last year. This included14,090 patients who were treated with Avastin for Age Related Macular Degeneration andDiabetic Retinopathy. 35,677 laser procedures were also performed- a growth of 13% over theprevious year. 457 corneas were transplanted at our Lahore Tertiary, Korangi Tertiary andQuetta Hospitals - a growth of 63 % over last year.

We remain the biggest provider of Cataract, Vitreo-retinal and Corneal transplant surgeries forthe poor people in the country.

In our aim to ensure the highest quality of surgical care we regularly evaluate ourselves againstthe WHO/ICEH benchmark for good cataract surgery. This is expressed in terms of visualoutcomes. In 2017/18 we were 7 percentage points higher than the benchmark.

EXPENSESOur operating expenses during the year were Rs. 1,105 million being 13.7% above last year. Mainreasons for this increase were annual salary increases, additional employment for operational needs, growth in patient visits to OPD and major surgeries and repair and maintenance of buildings andequipment. These increases were partly offset by reduction in fund raising activities and exchange losson foreign currency bank accounts respectively. Capital Expenditure during the year was 227 million,77% higher than last year mainly on medical equipment and building due to upgradation of VR (VitreoRetinal) unit at Korangi and expansions in hospital buildings at North Karachi & Khanewal. Ouroverhead expenses remained 4.64% of operating expenses and are among the lowest in the world.

EXPANSION OF CAPACITYEXISTING

LAR (MULTAN)The last segment of the project involving construction of a surgical complex with all its alliedservices was completed in December last year. Additionally, for the comfort of the patients, awalkway was built and a lift was also installed. The entire project has been funded by a UAEbased donor who had earlier also sponsored the renovation and expansion of Gambat andKhanewal Hospitals.

KHANEWALAdditional capacity to provide for the increase in the number of patients requiring surgery at thefacility was completed as planned in the fourth quarter of last year. There are now twoOperation theatres of which one will be used exclusively for a vitreo retinal unit that is being set

up there. This project is being sponsored by the same generous donor who had financiallysupported its earlier major expansion and new ophthalmic equipment.

NORTH KARACHISpace constraints required that we optimize the limited available area so that all plannedactivity including sub specialty clinics and a large operation theatre could also be housed withall the required allied services. This was necessitated due to the significant increase in thenumber of people visiting this hospital. This project was completed in February this year, without having to suspend hospital work at all.

KORANGILRBT had been contemplating for some time to upgrade its Vitreo retinal unit and alliedservices and acquire the latest technology / state of the art equipment so that quality eye caremay be provided to the needy and poor. We are extremely grateful to Amreli Steels Ltd for theirmagnificent generosity in funding the entire project. As planned, civil work was undertaken atthe beginning of the Holy month as surgical activity is comparatively reduced during thisperiod. The project is presently underway and is expected to complete by mid-July followingwhich it will become fully operational.

CHINIOTA purpose built Hospital located at the outskirts of Chiniot on the Chiniot/Jhang Road to replacethe current temporary facility is fast nearing completion. Civil work is expected to complete byend March 2019. The new 50 bedded facility is expected to become operational in April.

NEW HOSPITALS

RAHIM YAR KHANThere is a gap of about 500 kilometers between our Hospitals at Lar and Gambat making itdifficult for patients who have to traverse long distances to reach our Hospitals. It had beenLRBT’s long term plan to build a Hospital at Rahim Yar Khan District to address this issue. Thisregion has high prevalence of poverty. LRBT is being financially supported by a generousdonor who is providing us land, entire construction of the Hospital and operating expenses forfive years. Groundbreaking is planned to take place in the first quarter of 2019.

SOPHISTICATED EQUIPMENT The following state of the art equipment was provided to our various Hospitals to enable ourdoctors to provide quality eye care to the poor people who visit them ensuring that freetreatment does not mean second rate treatment.

CORNEAL DISEASES Corneal diseases are the fourth biggest cause of blindness. There is therefore a high demandfor cornea tissue and demand exceeds supply. The shortage is the result in general of thereluctance of the people to donate their organs after death (only 6 people have donated theirCornea) despite there being no bar in our religion.

Renowned National and International Islamic scholars and Mufti’s agree that organ donation ispermissible in Islam so long as the donation will not lead to the death or harm the donor:

LOCAL ISLAMIC AUTHORITIES ON ORGAN DONATIONIn an Islamic conference held at the ‘Sheikh Zayed Islamic Centre’, University of Karachi, lastyear, all the ulema – from every school of thought – gathered to discuss deceased organdonation. All the ulema agreed, and were on one page that as per need, organ transplantationis allowed and legitimate in Islam. They also agreed that deceased organ donation was entirelyin accordance with Islamic principles.

MUFTI MUNIB UR REHMAN“As per human needs organ transplantation is allowed and legitimate in Islam.”

“The best human being is the one who serves the most humanity and saving lives throughtransplantation is great service to humanity”

DR ABUL KHAIR MOHAMMAD ZUBAIR, PRESIDENT – JAMIAT-I-ULEMA PAKISTAN-NOORANI“Charity was the deed of prophets and organ transplantation is the greatest charity to savehuman lives. Saving lives can never be against Islam”

ALLAMA BABAR ZAIDI“The matter is need of the hour and organ transplants need to be used widespread in order tosave lives”

MUFTI MOHAMMAD IBRAHIM, A CII MEMBER FROM SUKKUR“Islam promoted conveniences and Shariah allowed transplantation when one needed that forsurvival.”

INTERNATIONAL AUTHORITIES ON ORGAN DONATION

HIGHEST COUNCIL OF SCHOLARS, RIYADH“It is permissible to transfer whole or part of organs from a deceased to another person, if theneed for such a transplant is critical. Live organ donation is also allowed if the recipient is inurgent need of the organ.”

FATWA OF MUFTI OF EGYPT, DR MUHAMMAD SYED TANTAWI“Live organ donations are permissible by consensus of jurists, whether the recipient be hisrelative or a stranger, as long as such a donation is considered to be of benefit by a trusteddoctor…[Taking organs from the body of a dead person to transplant to someone who needs it,whether to save his life or to cure him, is permissible, and this ruling is [also] based onconsensus of jurists.”

FATWA COMMITTEE OF KUWAIT“If an organ to be transplanted is taken from a deceased person, the ruling is that such atransplant is permissible, no matter whether the intention for the transplant by the donor wasmade in the form of a will by the deceased or otherwise. This is based on exigency (darurah),such as saving a life, that transforms what is originally forbidden into a state of permissibility. Thus, organ transplants are permissible, as long as there is an urgent need for it.

FATWA OF DR YUSUF AL-QARADHAWI“Someone who wishes to alleviate another human’s suffering, such as kidney disease, bydonating one of his healthy kidneys, is allowed to do so in Islamic law. [I]n fact, it is consideredto be a good act and the donor will be duly rewarded. This is based on a Prophetic tradition inwhich the Prophet p.b.u.h. is reported to have said that those who show kindness on earth willreceive God’s mercy and kindness. On this, I am of the opinion that there is no prohibition fromdonating an organ or body part to someone who needs it to cure him/her, and to replacehis/her non-functioning organ, such as the kidney or heart etc. with the intent of giving charity. And this is considered to be a continual (jariah) donation, and the rewards for the donorcontinues to be accrued for as long as there are those who benefit from it.

FATWA OF THE NATIONAL FATWA COUNCIL OF MALAYSIACadaveric transplant of the eye and heart is permissible if the following conditions are observed:

In the case of extreme need and exigency, in which the life of the receiver depends on thatorgan, and there is sufficient evidence that the transplant process will be successful.

In the case of heart transplantation, the death of the donor must be determined before thetransplant can be performed. Proper action must be undertaken to ensure that there is nokilling and trading of organs involved.

CONSENSUS:Organ donation is allowed under the following conditions

1. Health of donor should not be affected/ Body should not be disfigured2. It should not be done for monetary gains3. Major health and life benefit to the organ recipient

The consequence of hesitancy on part of Pakistanis is that corneal tissue has to be importedfrom abroad mainly the USA and Sri Lanka.

these large private donations from individuals who are passionate about the work of LRBT andwant to see the organisation grow and develop further.

GLT continues to successfully increase its focus on generating interest in its cause amongstthe younger generation of volunteers, students and donors for which purpose a disco eveningwas held at Morton’s private members’ club on 15th March 2018 and raised £2,000.

Outgoings:- During this 12-month period GLT remitted £ 114,366 specifically for the purchaseof IOLs in Pakistan for use in cataract operations in all LRBT Hospitals and the purchase ofMedical eye equipment of £196,958, a total of £311,324. GLT was well able to meet itscommitment of funding the supply of intra-ocular lenses to LRBT’s hospitals in 2017/18 fromthe net income generated this year.

GLT is expecting, on a best endeavours basis, to meet LRBT’s current forecast of the cost ofIOLs of approximately £278,000 to Pakistan during the 12 months to December 2019.

Trustees Art Malik and Nusrat Mirza visited LRBT in Karachi and Lahore in October 2017. Bothtrustees were very impressed with the volumes of patients that LRBT is able to treat on a dailybasis at both of these centres, the calibre of the staff and the availability of state of the artequipment at each site. The GLT trustees had an opportunity to meet the LRBT Boardmembers and both were involved in film making projects for GLT, which were to be shared withLRBT. The trustees wish to thank the LRBT team for its hospitality and support.

Finally, the Trustees would like to record their thanks to all the many donors for their generosityand to the supporters and volunteers who have, according to their means and time available, contributed so generously to GLT and to its performance in this period.

The fundraising total for the year was $120,000. This total is the highest we have raised ina single year since we first started fundraising in the US.

The majority of the donations raised were during Ramadan and they were received viaonline, text to donate service and cheques by donors across the United States.

Last year, we developed a clear plan to deliver high-quality level awareness of theorganization and increase fundraising donations across the country from New York, toCalifornia, Texas and Washington DC. A chapter in Texas has already been established &we are hopeful we will open another one in California next year. A strong network ofdonors with over 1500 on our database has already been established and we expect it togrow significantly in 2019.

The team in the US continues to look for trustees and advisors to help support anddevelop fundraising initiatives, including building strategic partnerships with companiesacross the US.

We would like to express our gratitude and sincere thanks to all our donors for theirgenerosity to help provide free eye care in Pakistan.

LRB FOUNDATION CANADALRBF, Canada, donated a sum of CAD 20,020 for conducting 840 cataract surgeriescarried out at six Hospitals in our network.
