RATION CARD MISSING DETAILS IN DATABASE

The Ration Card is issued by the State Government. It is a very important document that serves as an identity and

nationality proof in India. It helps categories families into Above Poverty Line (APL) and Below Poverty Line (BPL) on the basis of their economic status. It also provides citizens with the right to purchase goods from rationing shops at subsidized rates.

This Service is Used for Applying Missing Details in Ration Card. Some Cases Family Having the ration Card but Ration Card Details are Not Available at the Govt. The Processing Steps and Mandatory Documents for Applying Ration

Card Missing Details in Database as shown Below.

Note: The asterisk (*) denotes mandatory requirement of documents. Apply for Ration Card Missing Details in Database: This section contains instructions for the operators of the Meeseva web application to apply for Ration Card Missing Details in Database Service.

1) In Meeseva home screen, under List of Services, select Civil Supplies Department as depicted in Figure 1.

2) Select Ration Card Missing Details in Database Service in Civil Supplies Department as depicted in Figure 2.

3) Now Ration Card Missing Details in Database Request Screen will be displayed as depicted in Figure 3.

4) Enter Ration Card no and click Verify button as depicted in Figure 4.

5) It displays Ration card details screen as depicted in Figure 5.

4) Enter Head of the House hold Details such as Card Type, UID, Name of Head of the Family, Name of Head of the Family(In local language), Mother's Name, Mother's Name(In local language), Father's Name, Father's Name(In local language), Gender, Spouse's Name, DOB, Age and EID as depicted in Figure 4.

Note:-1. Here EID/UID Any one is Mandatory

Here we can also fill Applicant details with the help of Aadhaar number.

5) Enter Professional and Gas Connection Details such as Occupation, Annual Income, Gas Connection Status, Gas Company Name, Gas Agency Name and Consumer No as depicted in Figure 5.

6) Enter Residence Address details such as Door No, Locality/Land mark, District, Mandal, Village/ward, Pincode and FP Shop No as depicted in Figure 6.

Note:- Here I am selection In Case of HOF Related Changes.

7) Enter Permanent Address details such as Door No, Locality/Land mark, District, Mandal, Village/ward and Pincode as depicted in figure 7.

8) Select Add Member details either Yes/No as depicted in Figure 8.

Note:

1. Here if Members are available then select “Yes” It will display family Members.

2. If members are not Available Then Select “No”

3. Here I am selecting Yes

9) Enter Member Details Such as Member Name, Gender, Date of Birth(DD/MM/YYYY), Mother Name, Father Name, Spouse Name, Opting to Lift Commodity, Age, EID, UID and Relation with Head of the Family as depicted in Figure 9.

10) To Add another Member details Click Add New Row Button as depicted in Figure 10.

11) Enter all Informant details such as informant Name, Relation, Mobile No, Delivery Type , Mobile no and Proof Document as depicted in Figure 11.

12) Collect the necessary supporting documents as indicated in the Ration Card Missing Details in Database request page. Scan the documents and upload into the system, as depicted in Figure 12. Note:- Here Proof documents should be uploaded based on Selection.

Note: All the mandatory fields have to be filled in; otherwise request will not be accepted by the system. 13) Click Show Payment. Another window appears requesting to Confirm Payment as depicted in figure 13.

14) Collect the amount from the Citizen and click Confirm Payment to submit the request as depicted in figure 15.

On confirmation, a receipt will be generated as depicted in the following figure. The receipt will contain the Certificate delivery date. Note: Ensure that you have sufficient balance with your service provider (SCA), else the system will not accept the request.

Note: - On submission of the request, Citizen Receives Message. Like your Request for Ration Card Missing Details in Database has

been entered vide Application Number, Transaction Id and sent to Dept.

16) On submission, the request will be sent to the work flow of the respective Department for further processing.

17) Once the request is approved ,then the citizen need to collect the Ration Card from franchisee where he/she applied. Note:- Print Ration Card Service is Used For Printing The Ration Card
