current stage of change Use a readiness to change questionnaire
Health Risk Appraisal In general moderate levels of exercise and physical activity
to not provoke cardiovascular or musculoskeletal, but if client is unhealthy or have pre-existing disease they are at risk
Purpose pf pre-participation screening Identify presence or absence of CV, pulmonary or

medical exam Questionnaires should be client led with little input from
personal trainer PAR-Q is a good example as a minimal, yet safe pre-exercise
screening measure
PAR-Q

treated differently than someone with several risks Categorized as low, moderate or high
Identify CAD risk factorsPerform risk stratification based on CAD risk factorsDetermine need for medical exam clearance
1. Using the statistics from your client use table 6-1 to tally their risk #
2. Using figure 6-4 Risk stratification determine if they are low, moderate or high risk
3. Circle that risk, Explain to your client what they need to do
Evaluation forms: Informed consent: “assumption of risk”
Client acknowledges the risks associated with activity
Limitations: not a liability waiver, doesn’t provide legal immunity
Used to communicate dangers of the exercise program or test procedures, verbally review it with client
