    EXECUTIVE SUMMARY

    MAJOR ACCOMPLISHMENTS

    63rd

    Street Station

    Completed traction elevator rail inspection and elevator cab mock up.

    UPS System was commissioned.

    Completed installation of Mechanical equipment & HVAC duct work at Fan Plants.

    Tunnel exhaust fan start-up was completed.

    Platform lighting was installed and track wall finishes are continuing.

    Structural steel installation at Entrance 1 will be completed by the end of April 2015.

    72nd

    Street Station

    Concrete for the basement level slab at Ancillary 1 (NW corner of 69th Street and 2nd Avenue) was completed.

    Concrete for the street level slab at Ancillary 2 (NW corner of 72nd Street & 2nd Avenue) was completed.

    Platform concrete was completed.

    CMU wall, door frames, floor topping, painting and ductwork was completed for rooms located at the North and South ends of the mezzanine and platform.

    Conduit and MC cable installation is complete in the G3/G4 tunnels.

    Started installation of facility power equipment at Ancillary 2 and installation of property line box for Con Edison electric service.

    86th

    Street Station

    Concrete mezzanine slabs were completed.

    Twelve of eighteen first upper mezzanine slabs were completed.

    Seven of nine platform slabs were completed.

    Work in the tunnels between 72nd and 83rd Street Stations was completed providing shared access to Systems contractor.

    96th

    Street Station

    Concrete pours for the second floor walls and third floor slab at Ancillary 2 were completed.

    Surface utility work was completed on the east side of Second Avenue between 91st to 93

    rd Streets and 96

    th to 99th Streets.

    The Contractor is 50% complete with rebar installation for the 4th floor slab for Ancillary 1 (NE corner 93rd St) as of April 1, 2015.

    Milestones 6, 7, and 8 (Full access Communications Rooms, Signal Rooms and Traction Power Rooms) – Access to 17 rooms out of 19 rooms was provided.

    Systems

    A total of 8,400 LF out of 22,000 LF of track was installed as of April 30, 2015.

    Continue installation of signal and communication equipment at 72nd Street Station and 96th Street Station.

    Continue installation of signal equipment at the 63rd nd 72nd Street Station signal rooms. Conduit and cable pulling in the right-of-way is ongoing. Commenced breakdown

    testing in the Central Instrument Room (CIR 147) at 63rd

    Street.

    CRITICAL ISSUES

    Construction of Entrance #1 (at 69th Street) of 72nd Street (C-26011) - The Project

    needs to complete Entrance 1 work by September 2016 to maintain Project RSD.

    Contractor is implementing an agreed mitigation plan to recover previously lost time with

    the Entrance #1 work. However, in the course of this implementation, Contractor

    encountered delay with removal of the rock at the top of escalator incline; this has

    required additional mitigation.

    Mitigation: The Project is working with the contractor to mitigate delays so that the September 2016

    date for completion of Entrance 1 can be maintained. The project is closely monitoring

    the progress of rock excavation, concrete lining installation and street work. Mitigation

    includes working second shift, extended hours and weekend work and exploring

    opportunities to increase work hours on certain upcoming activities within the building.

    Incline rock evacavation is scheduled for completion on 05/15/15. Incline transition to be

    poured by 05/08/015. Meeting these dates will allow the Contractor to meet the

    September 2016 date. Street entrance excavation was completed on 04/14/15. The

    balance of street work is on schedule.

    T&C Requirements - The Project recognizes that T&C resource requirements, if not

    properly planned for, represents a major risk to the schedule. As a result, the Project has

    been focused over the last several months in developing and implementing a risk

    mitigation strategy to reduce the level of this risk.

    An experienced person has been assigned to lead the Testing and Commissioning work.

    The Project is verifying that the Contractor's Testing & Commissioning Plans are written

    and approved and is focused on obtaining early NYCT agreement of the plans. The

    Project has developed a document that provides NYCT resource requirements to conduct

    the T&C for the Project. The Project conducts bi-weekly meetings with NYCT, Contract

    CM‟s and schedulers to review of the upcoming activities and NYCT resources

    requirements.

    Facility Power - There is the risk that facility power eneregization at all new stations

    may not be available in time to support planned Testing and Commissioning activities.

    Delay in the availability of permanent power will have a direct impact on project

    contingency.

    Mitigation:

    The project developed detailed schedules that shows completion of facility power rooms

    prior to December 2015 and is closely monitoring progress of the latest schedules for the

    facility power installation.

    72nd

    Street Facility Power: All equipment is fabricated and in storage except for the

    Mimic Panels which are expected in mid-June. A problem at 72nd Street Station

    regarding permanent power is the delays with constructing Ancillary 1 and 2 Sub-base

    and Base rooms. MTACC is working with Contractor to develop plans to recover the

    delays.

    86th Street Facility Power: Equipment delivery has slipped slightly due to fabrication

    changes to low voltage equipment. Equipment delivery is still forecast to be on time

    (Low Voltage Gear by 06/25/15; all other equipment to be delivered prior to the end of

    the May). The main concern now is that contractor needs to complete rooms and room-to

    room conduit runs in order to accommodate equipment delivery. The Contractor has

    commenced acceleration of Ancillary 1 roof structure and has been directed to accelerate

    completion of the north facility power rooms so that rooms will be ready to accept

    equipment delivery.

    96th Street Facility Power: All main permanent power equipment, except for low voltage

    gear and Mimic Panels have been delivered. Leaks at this station are generating concerns

    that equipment installation might be delayed until leak issue is resolved. The Project has

    employed the services of an experienced grouting firm who has been successful in

    resolving leaks to the point that the schedule will not be delayed.

    Track Installation Delay – Track installation is delayed due to the slow progress in

    submissions and installation. Contractor has experienced problems achieving timely

    approval of submittals due to conflicting survey data. Installation of Track-work is on the

    SAS Primary Critical Path and has had a direct impact on both Program Contingency and

    the SAS Revenue Service Date.

    Mitigation:

    MTACC/Contractor has developed modified installation sequence that allows continual

    installation and concurrent access with Station Finishes Contractors. This modified

    installation sequences are expected to remove Track-work installation “off the SAS

    Program Critical Path.”

    Contractor has mitigated submittal approval issues by submitting all Profile Submissions

    early except for Zone 11; MTACC/NYCT is working with contractor to expedite

    approval of remaining Track-work alignment submittals. This is expected to correct the

    previous issue with submittal delays.

    Bi-weekly meetings are being held with contractor to further identify areas where the

    track-work installation can be expedited.

    QUARTERLY HIGHLIGHTS

    STATUS OF THE SAS PROJECT COST

    MTACC is currently managing to a Current Working Budget (Revision 10 Estimate & Post -

    Revision 10 updates) of $4.451 Billion, exclusive of $816.614 Million in financing costs. The

    MTA Board has approved Local Funds totaling $3.509 Billion. Total Federal participation is

    $1.300 Billion, of which $1.063 Billion has been obligated as of December 31, 2013. The 2012

    SAS Grant Amendment (NY-03-0408-09) was executed in the FTA TEAM system on

    September 19, 2014. The FTA placed a restriction on the 2012 grant outlining that funds could

    not be drawndown until MTA and the FTA had executed the revised SAS Phase 1 Full Funding

    Grant Agreement (FFGA). The revised SAS FFGA was approved on March 31, 2015.

    STATUS OF THE SAS PROJECT SCHEDULE

    Overview

    Schedule information contained within this report reflects schedule status of April 1, 2015.

    Through March 31, 2015, construction is 81.0% complete, representing a gain of 3.5% for the

    period of January 1, 2015 through March 31, 2015.

    Construction progress continues to support MTA‟s forecasted Revenue Service Date of

    December 30, 2016. The following milestones were achieved:

    Contract C-26006 - 63rd Street Station Reconstruction: Milestone #3d, Mezzanine 6 Interconnecting Conduits was completed February 20, 2015.

    Contract C-26012 – 86th Street Station: Entrances, Ancillaries, Station Finishes and MEP Systems: Milestone #4 - East / West Tunnels South of 86th St. Station for follow-on

    access to C-26009 Systems Contractor were completed February 27, 2015.

    Contract C-26010 – 96th Street Station: Ancillaries, Entrances, Concrete Work, Finishes (Arch, MEP): Milestone #4, Shared Site Access - East & West Trackways were achieved

    on March 2, 2015.

    SAS FFGA Schedule Contingency Management

    To be compliant with Section 6.2 FFGA Schedule Contingency Management of the “Schedule

    Management Plan,” MTACC is required to ensure schedule contingency for MTACC is above

    the minimums set forth below:

    “The FFGA RSD of February 2018 shall apply and a minimum level of schedule contingency of

    240 days (i.e., the “trigger point”) through the 3rd

    Quarter of 2016 (i.e., the “hold point”) shall be

    maintained, at which time the schedule contingency minimums will be updated as mutually

    agreed. Failure to meet this requirement shall trigger the requirement for a recovery plan.

    path(s) (namely “near critical” paths) for both projects shall be 25 calendar days.

    MTACC will manage the project to reflect a target schedule date of June 2016. Currently,

    schedule contingency of 102 calendar days has been built into the MTACC IPS and will be used

    ORGANIZATION

    During the first quarter of 2015, several staffing changes occurred. The Chief Scheduler left the

    SAS Project in January 2015. In the interim, the Program Control/Risk Management group has

    been providing scheduling services. SAS has hired a Scheduling Manager who will start in April

    2015 to perform the scheduling functions previously done by the former Chief Scheduler. The

    Project Scheduler for Contract 6 (C-26009) also left the Project in February 2015.

    RISK MITIGATION AND PROJECT EXECUTION STRATEGY

    The SAS Project continues to employ a variety of risk management techniques to identify,

    quantify and manage risks that may impact the project cost or schedule. A program Wide Risk

    Register update was completed in October 2014. A risk Analysis followed the Risk Register

    update and was completed in December 2014. The project will evaluate the risk analysis results

    and issue a Final Report that provides the 80% confidence level budget and RSD date. This

    report will be issued after Project senior management review and approval. The top risks

    identified by the risk analysis are:

    A total of fifty-five (55) CNCRs have been issued (29 CNCRs are concrete related).

    During this period, twenty- five (25) CNCRs were issued and ten (10) closed. Seven

    (7) were closed during this period. Contractor to submit concrete statistical analysis

    by end of April 2015.

    Daily Inspection Reports are on schedule submitted in EDMS on 3/15/2015.

    QA/QC meeting were held on 1/15/2015, 2/19/2015, 3/12/2015 & 3/26/2015. PMOC

    stated that the QA/QC meeting should be held every two weeks until quality issues

    are resolved. QA Manager prepared a matrix of outstanding issues showing

    commitment dates from Contractor so that the issues can be discussed at the next

    QA/QCmeetings.

    A meeting was held on 2/15/2015 between CM Office and MTACC Quality to

    discuss Quality Issues (PPMs not held for ongoing activities, QWPs not approved,

    Shop drawings not approved, QM needs additional help, NCRs not timely issued) by

    the Contractor. A letter was sent to the Contractor on 2/25/2015 outlining quality

    issues and outstanding NCRs requesting action plan form Contractor. Contractor

    responses to quality issues were received on 3/26/2015.

    Report for QQO#4 was conducted on 3/19/2015 and issued on 3/26/2015. There were

    ten (10) findings and one (1) observation. Contractor received a rating of 82%.

    To date, twenty- nine (29) Quality Work Plans (QWPs) have been reviewed for this

    contract. Six (6) QWPs were reviewed during this period.

    To date, sixteen (16) Preparatory Phase Meetings (PPMs) were held for this contract.

    One hundred six (106) NCRs were issued up to 3/31/2015 with nineteen (19) closed and eighty-seven (87) open (101 NCRs are concrete related). During this period,

    twenty-seven (27) NCRs were issued and eleven (11) NCRs were closed.

    Daily Inspection Reports were submitted to EDMS on 03/27/2015.

    QA/QC meetings were held on 1/15/2015, 2/12/2015 and 3/12/2015.

    To date, one hundred-nineteen (119) Quality Work Plans (QWPs) have been

    submitted and one hundred-one (101) have been approved. Thirteen (13) QWPs were

    approved during this period.

    To date, forty-four (44) Preparatory Phase Meetings (PPMs) were held for this

    contract. Three (3) PPMs were held during this period.

    Mock-ups of C2A ceiling panels, glass fins light fixture and Ancillary 2 Façade

    concrete were performed.

    QQO#5 was performed on 2/27/2015. Report was issued on 03/06/2015. Five (5)

    findings were noted. Judlau received a rating of 93%.

    ACE evaluation was conducted for the period 10/1/2014 through 3/31/2015. Judlau

    received a satisfactory quality rating.

    Daily Inspection Reports are on schedule. Latest DIR submittal entry stored in

    EDMS from 3/9/2015 thru 3/13/2015 was dated 3/16/2015.

    QA/QC meetings were held on 1/20/2015, 2/17/2015 and 3/31/2015 as per schedule.

    However, 3/31/2015 meeting was cancelled. GC QAM was unable to attend.

    To date, one hundred fifty-eight (158) Quality Work Plans (QWPs) have been

    reviewed. No QWP revision was reviewed during this reporting period.

    Quarterly Quality Oversight #15 was performed dated 1/22/2015. Three findings and

    one observation were found. Judlau received a rating of ninety-six percent (96%).

    System Safety Certification (SSC) has started on SAS. Four (4) SSC meetings were

    held during this reporting period.

    Special Inspection Audit of CM office and Judlau utilizing MTACC Procedure

    QA.10 – Special Inspection was held on 2/6/2015. The audit report was issued on

    3/23/2015.

    contract. Three (3) QWPs were reviewed and comments provided.

    To date, Eleven (11) Preparatory Phase Meetings (PPMs) were held.

    Quarterly Quality Oversight #8 was performed on 2/20/2015. Report was issued on

    3/26/2015. There were four (4) findings and one (1) observation. CSJV received a

    rating of 96%.

    Five (5) Factory Acceptance Tests (FATs) were conducted by CSJV/NYCT during

    this period. Results of the FAT were satisfactory.

    Safety Certification meeting were held on 1/15/2015, 2/19/2015 and 3/19/2015.

    Response was received from AAJV on 12/19/2014 and 1/9/2015 regarding agenda

    items 1-3 of Special Inspection meeting held 12/4/2014 with AAJV. Item 4 requires

    response from AAJV as to why overall configuration management requirement was

    not included in the quality specification 01 43 00.

    Configuration management plan, response to CSJV letter 1/5/2015 was provided on

    3/16/2015 stating that configuration management plan be in place for

    Signal/Communication and SCADA systems although there is no requirement in the

    contract specification for a comprehensive configuration management plan.

    Draft PQ Manual was sent to PMOC for review on 3/19/2015.

    Safety and Security Certification Checklist:

    The checklists for facilities and systems were distributed to all five contractors for final review

    and adoption. A meeting was held with all five contractors to explain the changes that were

    made to meet the specific requirements of the FTA Safety and Security Certification process.

    These checklists represent the project baseline for the completion of the construction/installation

    phase of the project.

    Body of Evidences (BOEs) to process Safety and Security Certification Requirements

    Specific instructions were provided to contractors on what the different Body of Evidences

    Contract Site Meetings and Visits:

    During the work in progress there were meetings held on a monthly basis by the Working Group

    at each contract field office. Also there was a field walk-through arranged to see the work in

    progress compared to all BOEs submitted by the contractor. All submitted BOEs were reviewed

    and verified for their merit. FTA PMOC Representatives and MTA Oversight Representatives

    are invited to all TWG meetings. Meeting minutes are distributed to all invitees.

    Meeting with FTA, PMOC and MTA Oversight Representatives:

    Monthly meetings are held with the above representatives to brief them on the progress of SSC

    program.

    Administrating SSC Program:

    EDMS IT staff uploaded the finalized checklist. IT staff is in the process of modifying the

    Description: Contract C-26010 is for the station finishes, installation of

    mechanical, electrical and plumbing systems in the station, installation of

    elevators and escalators, utility restoration, construction of the above ground

    structure of the entrances and ancillary facilities, remaining invert slab, street,

    sidewalk and tree restoration, and retrofit of the existing tunnel between 99th

    and 105th Streets and the base slab of the new bored tunnel from 87th to 92nd

    Streets.

    Progress, Cost and Schedule Package Milestones C-26010

    Station Area - The Contractor completed 15 out of 19 rooms for Milestone 6, 7, and 8 that were turned over

    to C6. MEP work and framing for the ceiling and wall

    panels are also ongoing in the Main Station area. CTJV delivered and installed 3 of 9 escalators in the main

    station.

    Ancillary 1 - The Contractor poured 4 of 5 slabs above ground.

    Ancillary 2 -The Contractor poured 3 of 5 slabs above ground.

    Entrance 1 -CTJV poured the one-sided walls at Entrance 1.

    Entrance 2 - The contractor completed the pour for the walls at Entrance 2 and began installing rebar for

    the street slab.

    Roof Level - The Contractor completed the traffic switch to the west side South of 93rd Street and North of 97th Street, where decking removal has started. The

    traffic switch will be completed once the utility work

    and backfilling of the roof between 93rd and 97th Streets. Work for the new water main was completed

    except for the tie-in.

    Description: Second Avenue Subway Contract C-26011 is for the

    construction of ancillary facilities, 3 entrances, the installation of station

    entrances finishes and the mechanical, electrical, and plumbing systems for

    the 72nd

    Street Station. Major construction work includes station electrical

    service, fan plant and pump room equipment, elevators and escalators.

    Progress, Cost and Schedule Package Milestones C-26011

    Platform concrete is complete. Room finishes including MEP work is ongoing. Installation of vertical supports

    for track wall has commenced.

    Conduit installation is ongoing behind mezzanine walls. Installation of steel framing for mezzanine walls is

    complete.

    Entrance 3 (SE corner of 72nd St. and 2nd Ave) concrete pours are ongoing.

    Entrance 1 garage rock excavation (NE corner of 69th St. and 2

    nd Ave) is on-going. Street excavation has

    commenced and is near completion.

    Concrete pours for the Ancillary 1 structure (NW corner of 72

    nd St. and 2

    nd Ave) are complete up to the 2

    nd Floor

    above ground.

    Concrete pours for the Ancillary 1 structure (NW corner of 69

    th St. and 2

    nd Ave) are complete up to the basement

    level.

    All milestone rooms have been inspected and turned over to Contract C-26009 (C6) with the exception of 4

    rooms.

    Escalators 1, 3, and 5 (platform to mezzanine) have been deliverted and set in place. Installation is ongoing.

    Electrical equipment (switchgear, transformers) has been delivered to the Ancillary 2 sub-basement Facility

    Power Room.
