Q. I have seen a lot of unknown abbrevIatIons when doIng medIcal record revIews that often looks lIke “text” language. can we have a lIst of approved abbrevIatIons to use In medIcal records? I know we have a “do not use abbrevIatIon” polIcy, but thIs would be dIfferent and useful.

A. This was pointed out during the Joint Commission survey and addressed during a Patient Safety meeting this September. A task force has been formed and will use a well-referenced resource book to determine approved abbreviations for the hospital. More details will follow as the task force gets to work. Updates will be shared via the Patient Safety Newsletter, during shift huddles or department meetings. MARK’S MESSAGEOn behalf of the Board of Trustees and senior management, I want to express our gratitude for the support that followed the hospital’s difficult decision about how to absorb an unexpected $3 million loss of revenue by the Center for Medicare Services in FY17. This plan involves using prior year reserves to offset $1 million, establishing a break-even budget to cover another $1 million, and benefit reductions primarily in Earned Time accrual and pension plans to cover the remaining $1 million.

As I said in my letter to staff, nothing about this is easy or fair. But our plan to mitigate this one-time revenue loss has been based on what we believe is in the best interest of the hospital, our 1,200+ employees, and the communities we serve by maintaining current programs and services, current staffing levels (avoiding a potential layoff of 25-35 jobs), and current salaries and annual increases for all employees.

If some employees were angry about the cuts we’ve made, I would understand. I am angry. Someone else’s math error is directly impacting our staff, and that’s just not right. But instead of anger, many employees have expressed their support, understanding, and appreciation for our approach of putting services, jobs, and salaries first. I personally want to thank every employee and community member who has stopped me in the hallway, sent emails, and shared their feedback during Town Hall Meetings. Thank you for rallying behind the hospital and the tough decisions we’ve made under these unique circumstances.

As FY16 comes to a close and we embark on FY17, we know that there are challenges ahead but I’d like to reflect on the incredible year we’ve had in FY16. Our momentum will only continue to strengthen as we execute our strategic plan to grow volume and revenue. Anna Jaques has partnered with Shields Health Care Group to offer Positron Emission Tomography/Computed Tomography (PET/CT) scans to give patients the most advanced medical imaging technique available today. This offers our community a powerful tool for diagnosing and staging cancer, evaluating coronary artery disease, and differentiating Alzheimer’s disease from other forms of dementia.

PET/CT scans enable physicians to detect the presence of disease earlier and pinpoint its location with much greater accuracy than PET or CT scans alone.

The PET/CT technology is a mobile system that will be on-site at the hospital every Tuesday following a survey by the Department of Public Health on October 5th.

MARK GOLDSTEIN APPOINTED BY GOVERNOR BAKER TO SPECIAL COMMISSION TO ADDRESS PRICE DISPARITY

On September 15th, physicians and hospital leaders gathered for the annual Medical Staff Meeting and honored retiring physicians Joe Hull, MD, and H. Thompson Mann, MD. Both longtime physicians were named honorary medical staff members for their years of dedicated service to Anna Jaques and our community.

Dr. Hull has 30+ years of experience practicing emergency medicine at Anna Jaques following ten years in internal medicine and nephrology. He served in many leadership roles including President of Greater Newburyport Emergency Physicians, Inc., former President of the AJH Medical Staff, acted as a hospital representative to the Board of Trustees, served as chairman of the Credentials Committee, and was a member of the Trauma Executive and Bylaws

Committees.

Dr. Mann is a board-certified internist who joined Anna Jaques in 2003 as a Hospitalist. Dr. Mann has been a leader in the Hospitalist program providing quality patient care and collaborating with the hospital, medical and nursing staff. Dr. Mann is an Olympic swimmer and former world record-holder and competed at the 1964 Summer Olympics in Tokyo, Japan, where he received a gold medal.

The meeting also welcomed Kenneth Sands, MD, MPH, Chief Quality Officer and Senior Vice President, Health Care Quality at Beth Israel Deaconess Medical Center, who presented the CME, “High Reliability and Humanism: Can they Coexist?”

Governor Charlie Baker’s Administration has formed a 23-member Special Commission to Review Variation in Prices among Providers which is charged with reviewing the various factors contributing to price variation in physician, hospital, diagnostic testing and ancillary services. This commission is comprised of health and human service leaders, Attorney General Maura Healey, legislative appointees, insurers, health economists, lawmakers, state officials and hospital representatives including AJH President & CEO Mark Goldstein.

The commission, created as part of a law passed this summer to avoid a ballot question fight over pricing, has until March 2017 to file its recommendations and any draft legislation, which members described as a tight timeline to reach consensus on a complex issue of Health Care Price Variation Measure.

“I am honored to be nominated by the Governor and be part of the Price Variation Commission representing community hospitals. The commission’s mission and purpose is to make recommendations to the legislature to address price variation. I’m hopeful the commission members will put a realistic plan together that will help stabilize commercial rates that all hospitals get paid. If this is done properly it will help balance the playing field and stabilize the community hospital network across the Commonwealth,” said Mark Goldstein.

The commission is scheduled to recommend steps to reduce price variations and issue a report by March 2017.

Following a comprehensive review of the Anna Jaques Laboratory, the hospital has once again received accreditation by the College of American Pathologists (CAP) Laboratory Accreditation Program which surveys the entire spectrum of laboratory test disciplines with the most scientifically rigorous customized checklist requirements.

The Laboratory Accreditation Program inspects a variety of laboratory settings from complex university medical centers to physician office laboratories, and covers a complete array of disciplines and testing procedures. The surveyors were impressed with the hospital’s program. The few findings identified by the team were not critical in nature and fixable. The surveyors also shared kudos with senior management, noting the exceptional personnel records and manual documentation control system.

Congratulations and thank you to all Laboratory staff. To host a successful survey requires organization, comprehensive policies and procedures, and a commitment to providing a quality product and exceptional customer service. A special thank you to Barbara Fernald, Jennifer Castle, and Marianne Green for their help in providing the CAP survey team with whatever they needed relative to Microbiology, Blood Bank, and the Donor Room.

LABORATORY RECEIVES ACCREDITATION FROM THE COLLEGE OF AMERICAN PATHOLOGISTS

OncoEMR, an electronic health record specifically tailored to the needs of oncology clinicians and patients, went live on September 20th at the Anna Jaques Cancer Center affiliated with Beth Israel Deaconess Medical Center (BIDMC). OncoEMR implementation will benefit the Oncology department and other hospital departments in several ways, including:•Improvedefficiencyforclinicalandadministrativeteammembers•Electronicallyaccessiblearchiveofpatienthealthinformationversus the current paper state•Automatedbillingprocessesforbothourfacilityandourproviders

Oncology department leadership and several key stakeholders within the hospital conducted an in-depth software selection process over the course of several months to determine that OncoEMR was the best fit for our hospital and clinic.

This project, which started in March and has been led by Information Technology (IT), was a full Electronic Medical Record implementation that involved a hospital-wide effort from: Nursing, the medical staff, Laboratory, Pharmacy, Health Information Management (HIM), Diagnostic Imaging, Registration, and Patient Access. Congratulations to all involved on a successful and uneventful launch!

OncoEMR WENT LIVE SEPTEMBER 20TH

Anna Jaques welcomed board-certified family medicine physician Hayley Moak-Blest, DO, to the medical staff as she joined Newburyport Family Practice led by Kevin Lanphear, DO. Dr. Moak-Blest practices full-spectrum family medicine and enjoys taking care of multigenerational families. Her special interests include: pediatrics, women’s health, office procedures, and Osteopathic Manipulation. Dr. Moak-Blest received her Bachelor of Science at the University of Massachusetts Amherst Commonwealth Honors College. She obtained her medical degree from Nova Southeastern University College of Osteopathic Medicine in Fort Lauderdale, FL, and completed her residency at University of Massachusetts Memorial Medical Center in Worcester.

Born and raised in Newburyport, she is excited to be living and working in her hometown.

SPOTLIGHT ON BREAST HEALTH DURING OCTOBER

• We are once again calling on local businesses to incorporate pink and the spirit of raising awareness with Pink Up the Port – a friendly photo contest with the winning submission (as voted on by the GBCC) featured on the front page of the Newburyport Daily News.• Anna Jaques will feature pink ribbon coffee sleeves in local coffee shops, bakeries and our own Cafeteria to spread the word about the importance of early detection and to schedule your annual 3D Mammogram.• Hospital employees are encouraged to participate in “Pink Tie Tuesdays” and “Pink Frill Fridays” throughout the month of October. Pink-up your outfit with ties, accessories, pink blouses, and more. • Newburyport Daily News will publish a special Breast Cancer Awareness Month insert featuring articles on the Gerrish Breast Care Center and its patients to hit stands in mid-October.• AJH, the Institution for Savings, and the Greater Newburyport Chamber of Commerce have teamed up to host a luncheon on October 28th featuring Kelley Tuthill, former WCVB reporter and breast cancer survivor, to share her story of survival and life after cancer with the community. This October, Pulmonary Rehabilitation will celebrate its one-year anniversary of treating patients who suffer from a variety of chronic lung diseases. Since starting the program last October, close to 90 patients have graduated from the individualized exercise program to improve their quality of life through educational sessions, continued treatment and tools to better their health. Pulmonary Rehabilitation has also seen nearly 300 patients in the hospital for consults with many joining the program or counseling at the Pulmonary Rehabilitation Clinic.

The program has witnessed many success stories in the past year. One of the program’s first patients suffered from chronic illness and was admitted to the hospital six times in six months before joining Pulmonary Rehabilitation. Since graduating from the program in January 2016, she has only been to the hospital once in nine months and counting. She goes to the gym regularly with a friend she made in the program.

Driving reminDerRecently, a hospital neighbor voiced concern over the dangerous speeding that she witnesses on One Wallace Bashaw Jr. Way on a regular basis. The neighbor noted that the most common times for speeding occur at 7:00am, 3:00pm, and 11:00pm (right around shift changes). Please be a mindful driver and slow down – this is a busy road with patients from the hospital, Newburyport Medical Center, Avita, as well as neighbors walking on the road.

The hospital has successfully completed the Joint Commission (JC) review but our dedication to quality, patient safety and improvement is ongoing. An area of improvement that the JC surveyors highlighted was the use of “Do Not Use Abbreviations” in medical records. “Do Not Use Abbreviations” should never be used in medication orders because they can easily be mistaken for other abbreviations which could result in medication errors that may cause harm to the patient.

Reminders:• “DoNotUse”listappliestotheentiremedicalrecord,includingpaperandelectronicdocumentationforms, paper and electronic nursing, respiratory, and other provider notes, progress notes and test reports. JOINT COMMISSION REVIEW IS OVER…NOW WHAT?

Amesbury High School golf team members James Tagliente and Lowell Deschenes recently accepted the 2016 “Byron Bowl” from Former Mayor Byron Matthews, along with a check for $500 to the Amesbury High School Athletic Department. Tagliente and Deschenes won the “High School Challenge” sponsored by SPS New England, Inc. during the 26th annual Anna Jaques Hospital “Fore Your Health” Golf Tournament on August 15th at Renaissance Golf Club.

Amesbury competed against players from Ipswich, Hamilton-Wenham, Masconomet, Newburyport, and Triton.
