in the same city, this
year the event was
held at Seruni Hotel
close to Taman
Safari, a
conservation center
and national
tourism park. The
event was held for
two days, on 12 – 13
September 2014.
The first day was quite serious. People were preparing themselves to present their team’s KPI in front of BOD. The event started on 10:00 and opened by CEO’s speech for then continued by CFO, explaining about company’s business performance for January – June 2014. The agenda was continued after lunch by sharing and presenting each department’s KPI starting from HR & GA department, Control department, continued by Finance department and the last was the Secretary department. Each team was given 25 minutes to present and answer the questions from BOD members.

During the break time, the committee asked all employees to play some games that require them to work and solve the problem together. There were three groups who played two games. BOD members were the judge to award scores to each team.
Finishing all the KPI presentation gave a relief for everybody. After cleaning up, all employees and BOD members couldn’t wait to enjoy their meal. It’s a dinner time!
Dinner was held at Bumi Aki, an Indonesia Restaurants at the top of mountain. The temperature
was cold, however this did not prevent everybody to enjoy the dinner and to have chat each oth-
ers. Not only the meal the people were enjoying but also the announcement of the winners of the
previous games held in the afternoon. Prize had been provided to the winners.
The second day was more relaxing. On this day, the outing participants were ready for Taman
Safari tracking and outbound! Unlike the last year’s tracking that passed through garden and wa-
terfall, this time we had a chance to explore forest with its fresh air. And of course, the out-
bound !!

EMERGENCY DRILL COOLING WATER SYSTEM FAILURE

Once more, the operation team has performed the emergency drill with different format, not as competition anymore.
Emergency drill has become a needed and routine activity for the operation team. With this event all members of oper-
ation team are confident to face any worst situation that might cause the plant to shut down or cause fire case in PA-
TRA SK plant. The emergency drill is one way to improve the capability and awareness among all manpower to face
emergency situation.
Emergency drill’s scenario held on 25 September 2014 was “cooling water system failure”. The process of cooling wa-
ter is used to remove heat from various sources such as machinery or heated process material and also to maintain
vacuum condition on vacuum column (VD-V0101 & CD-V0252).
Detail emergency drill started with failure of cooling water supply turbine pump (Y-P0315 A) at the same cooling wa-
ter motor pump (Y-P0315 B) as a stand by pump still in overhaul condition, after unsuccessful effort to start turbine
pump, we decided to shut down VDU and CDW unit. The most important thing on emergency drill is how to shut down
the unit smoothly and without any accident.
After finishing Emergency drill, we did some brain storming to improve PATRA SK team work and there were some
corrective action plans for this event :
Improve communication during the emergency, due to lack of frequency available. Communication during emer-
gency time must be effective and clear.
Review and analyze response to an actual emergency to identify opportunities for improvement and track your
progress on necessary changes
Improve problem impact analysis.
Next action plan, operation team will arrange worst scenario to prevent it happen in PATRA SK.

HAZOP (HAZARD & OPERABILITY) STUDIES REVALIDATION
HAZOP (Hazard and Operability) Study is a structured and systematic examination of a planned or existing process or operation, for identify-ing, evaluating and controlling the hazards and potential operability problems of processes involving highly hazardous chemicals. HAZOP Study is one of method in Process Hazard Analysis (PHA) and it is part of implementation of Process Safety Management elements in PATRA SK. PHA is applied in case of new plant installation, re-analysis for existing plant, Management of Change (MOC) requirement and special PHA refers to Procedure no.OP-004/TS-PSM.
What’s HAZOPS ?
PATRA SK HAZOPS was already made prior to the construction of LBO Plant in 2006 and since ini-tial Start-up on 2008 we have conducted some modifications (MOC) to improve the operation con-dition. Considering any safety risks arises since initial start up and to ensure the current process still appropriate with operating and design concept, PATRA SK Management committed to conduct Re-PHA (HAZOPS Revalidation) after 5 year operations.

HAZOPS is carried out by a multi-disciplinary team with expertise in design of installation, commissioning, production and process operation, maintenance of facilities and ongoing requirements for health and safety. Two (2) mandatory people involved in HAZOPS are :
1. Must be an expert in Process 2. Must be an expert in HAZOPS Method
In organizing a HAZOPS Revalidation we appoint HAZOPS Leader, HAZOPS Scribe and HAZOPS Team Member. For the revalidation workshop, PATRA SK hires a vendor (PT. Lebah Solusi Indonesia) as HAZOPS Leader and HAZOPS Scribe. PT PATRA SK HAZOPS Team members consist of Process Engineers from Technical Support, Operation Team, Opera-tion Supervisor, Process Safety Engineer, Instrument Engineer, Rotating Engineer and Stationary Engineer. While the core team of PATRA SK HAZOPS Team members consist Process Engineer from each unit in PATRA SK (VDU, CDW and UTOS), Operation Supervisor, Operation Engineer and Process Safety Engineer. The other discipline Engineer partici-pated as supported Team Member during the revalidation workshop. One of the success factors in HAZOP Study is to use Team Member who is familiar/trained on HAZOP. On 2nd – 4th Sep-tember 2014, PT PATRA SK conducted HAZOPS in-house Training to all team members which was facilitated by PT. Lebah Solusi Indonesia. The Training participants were very enthusiastic, the classroom training was fun, relax but the serious atmosphere makes the session effective and enjoyful.
