Providence Signature Network*This national network provides the broadest selection of nearly 1million health care providers, both in Providence facilities andnon-Providence facilities. Providence Choice Network*This network allows you to choose a medical homefrom nearly 300 primary care clinics locatedthroughout select Oregon and southwest Washingtoncounties. When you select the Choice plan, yourdesignated medical home care team from theProvidence Choice Network supports and arranges allof your health care needs, including referrals tospecialists as needed.

Providence Connect Network*This Portland-area network lets you choose a medical homefrom more than 70 primary care clinics in Clackamas, HoodRiver, Multnomah, Washington and Yamhill (zip code97132 only) counties. When you select the Connect plan,your designated medical home care team from theProvidence Connect Network supports and arranges all ofyour health care needs, including providing referrals tospecialists within the Providence Connect Network, asneeded.

*Practitioners and hospitals in Providence Health Plan networks must meet Providence Health Plan credentialingrequirements. Providence Health Plan does not currently use specific quality measures, member experiencemeasures, patient safety measures, or cost-related measures to determine provider networks, including ProvidenceMarketplace networks.

Choosing a health care providerPrimary Care Providers (PCPs) perform exams, provide preventive care and treat many routine issues intheir offices. We encourage you to choose one provider who can serve as the primary provider for yourhealth care needs; however, you are not required to select a PCP nor are you required to have a referralwhen seeking the care of a specialist. In the event you see a specialist, it is recommended that you notifyyour PCP so that your medical information may be shared between providers.

Should you change providers at any time, you may wish to request that your medical records be forwardedto your new provider’s office.
