    Medical Group. This PCP will still coordinate yourcare, but you may seek needed care from anyHAP Senior Plus-Henry Ford affiliated specialist orhospital.

    Henry Ford Medical Group specialty physiciansare available to all HAP Senior Plus-Henry Fordmembers. HFMG physicians are listed in a chapterwith tinted pages when their cities fall withinthe county(s) this directory serves. They are listedagain in the general personal and specialty carephysician listings.

    When your choice is HAP Senior Plus-Expanded Network

    The HAP Senior Plus-Expanded Network bringstogether thousands of doctors and specialists inthe plans nine-county service area. You choose aPersonal Care Physician (PCP) who will coordinateyour care and arrange for you, as necessary, tosee other affiliated doctors and hospitals. The PCPhelps ensure that care you receive from differentspecialists is coordinated and works well together.Though your PCP may recommend specialtyservices from a particular specialist or facility,you can choose to receive needed care from anyHAP Senior Plus-Expanded Network contractedspecialist, and you may use any HAP Senior Plus-Expanded Network contracted hospital for in-network care.

    Transferring to another PCPThere may be times when you need to changeyour PCP. You may do so by first finding a new PCPwithin this directory who is affiliated with your HAPSenior Plus plan. You may logon at www.hap.orgto make your PCP selection or call a Client ServicesSpecialist at the number listed on the inside frontcover of this directory.

    Specialty CareThe PCP you choose coordinates all the servicesand care you need through the select hospitals,physicians and specialists affiliated with your HAPSenior Plus network and within plan guidelines.If you need certain types of covered services orsupplies, you may need approval in advance fromyour PCP. In some cases, your PCP will need toget prior authorization (prior approval) from HAP.Covered services that need approval in advance aremarked in the Medical Benefits Chart within yourEvidence of Coverage. To arrange your specialtyservices, your PCP will submit a request to HAP oryour medical centers Managed Care departmentfor authorization. There are some specialists thatdo not require referrals from your PCP. Please alsorefer to the detailed information in our sectiontitled Selecting a PCP.

    OB/GYN CareWomen may receive routine OB/GYN care (pap tests,breast exams, pelvic exams, and mammograms asnecessary) from any GYN affiliated with HAP SeniorPlus without a referral from your PCP.

    Non-Routine OB/GYN services (gynecologicalsurgery, hospitalization) must be obtained froma HAP Senior Plus physician within your plansguidelines for specialty care services.

    OptometryYou may visit any optical center/optometristaffiliated with HAP Senior Plus without a referral.Simply call the optical center/optometrist to makeyour appointment. Optometrists are listed by namein our alphabetical and area of practice sections.They can also be found by the city in which theypractice. Affiliated optical centers are listed at theback of the directory.

    HAP SENIOR PLUS PLAN INFORMATION As a member of HAP Senior Plus (hmo or hmopos), you will have to choose one of the network providers who are listed in this directory to be your Personal Care Physician (PCP). You may choose among the PCPs affiliated with your HAP Senior Plus network. PCPs routinely specialize in family or internal medicine. The term PCP will be used throughout this directory. Generally, you must get your health care from your PCP. See also, the next section, Selecting a PCP.

    Your PCP is the physician who coordinates your medical care and makes arrangements for you, as necessary, to see other affiliated physicians and hospitals. Coordinating your care includes checking or consulting with other plan providers about your care and how it is going. For members with multiple or chronic conditions, your PCP helps ensure that care you receive across different specialties is coordinated and works well together. Your PCP keeps your medical history and is familiar with your health care needs. Think of your PCP as your partner in health care the person you turn to when you have questions about your health and how to maintain it. If you need certain types of covered services or supplies, you may need approval in advance from your PCP (such as giving you a referral to see a specialist). In some cases, your PCP will need to get prior authorization (prior approval) from us. Covered services that need approval in advance are marked in the Medical Benefits Chart within your Evidence of Coverage.

    With limited exceptions, while you are a member of HAP Senior Plus you must use your plans affiliated providers to get your medical care and services. The exceptions include emergency and urgent care services, for which you are covered whenever you travel worldwide. You are also covered for out-of-area renal dialysis within the United States. Otherwise, if you obtain routine care from out-of

    network providers neither Medicare nor Health Alliance Plan of Michigan will be responsible for the costs. For members of HAP Senior Plus (hmopos) there is an additional benefit for some out-ofnetwork routine services. For these out-of-network services your costs may be higher. Details about this exception can be found in the Medical Benefits Chart in your Evidence of Coverage.

    Selecting a PCP No matter which PCP you choose, it is important to know which providers are affiliated with the HAP Senior Plus network you select, whether it is HAP Senior Plus-Henry Ford or HAP Senior Plus-Expanded Network.

    If you have not already done so, please select your PCP as soon as possible so that you can begin enjoying the full benefits of your HAP Senior Plus membership. Go to the sections for physicians listed by area of practice or by city to find physicians designated as PCPs for your HAP Senior Plus plan. They will be listed under the area of practice label, Personal Care Physicians. Select your PCP while logged on to www.hap.org or by calling a Client Services Specialist at the phone number listed on the inside front cover of this directory. Remember, each HAP Senior Plus member must choose a PCP affiliated with his or her HAP Senior Plus network. Family members do not need to select the same PCP or visit the same office location.

    When your choice is HAP Senior Plus-Henry Ford
