Filling out Form TD1 Fill out this form only if:

• you have a new employer or payer and you will receive salary, wages, commissions, pensions, employment insurance benefits, or any other remuneration;

• you want to change amounts you previously claimed (for example, the number of your eligible dependants has changed);

• you want to claim the deduction for living in a prescribed zone; or• you want to increase the amount of tax deducted at source.

Sign and date it, and give it to your employer or payer.

If you do not fill out Form TD1, your employer or payer will deduct taxes after allowing the basic personal amount only.

Provincial or territorial personal tax credits returnIf your claim amount on line 13 is more than $11,809, you also have to fill out a provincial or territorial TD1 form. If you are an employee, use the Form TD1 for your province or territory of employment. If you are a pensioner, use the Form TD1 for your province or territory of residence. Your employer or payer will use both this federal form and your most recent provincial or territorial Form TD1 to determine the amount of your tax deductions.

If you are claiming the basic personal amount only (your claim amount on line 13 is $11,809), your employer or payer will deduct provincial or territorial taxes after allowing the provincial or territorial basic personal amount.

Note: If you are a Saskatchewan resident supporting children under 18 at any time during 2018, you may be able to claim the child amount on Form TD1SK, 2018 Saskatchewan Personal Tax Credits Return. Therefore, you may want to fill out Form TD1SK even if you are only claiming the basic personal amount on this form.

Deduction for living in a prescribed zoneIf you live in the Northwest Territories, Nunavut, Yukon, or another prescribed northern zone for more than six months in a row beginning or ending in 2018, you can claim:

• $11.00 for each day that you live in the prescribed northern zone; or

• $22.00 for each day that you live in the prescribed northern zone if, during that time, you live in a dwelling that you maintain, and you are the only person living in that dwelling who is claiming this deduction.

$Employees living in a prescribed intermediate zone can claim 50% of the total of the above amounts.For more information, go to canada.ca/taxes-northern-residents.

Additional tax to be deductedYou may want to have more tax deducted from each payment, especially if you receive other income, including non-employment income such as CPP or QPP benefits, or old age security pension. By doing this, you may not have to pay as much tax when you file your income tax return. To choose this option, state the amount of additional tax you want to have deducted from each payment. To change this deduction later, fill out a new Form TD1.

Reduction in tax deductionsYou can ask to have less tax deducted on your income tax return if you are eligible for deductions or non-refundable tax credits that are not listed on this form (for example, periodic contributions to a registered retirement savings plan (RRSP), child care or employment expenses, charitable donations, and tuition and education amounts carried forward from the previous year). To make this request, fill out Form T1213, Request to Reduce Tax Deductions at Source, to get a letter of authority from your tax services office. Give the letter of authority to your employer or payer. You do not need a letter of authority if your employer deducts RRSP contributions from your salary.

Thank you for your interest in applying to Buffalo Trail Public Schools to be on the substitute teacher list. You must submit your completed application to our office. When it is approved, we will submit your name to the schools you have selected. You will be contacted at your email address when approval has been given. RESUME AND REFERENCES A package will not be considered until all pieces required have been submitted. This includes a current resume with three current professional references complete with contact information including email addresses (letters may be included). Please note that the Criminal Record Check cannot be more than six months old. Included in this package, we have listed the required documents. SUB BOOKING Each school is responsible to call the individual sub. In some cases, teachers book their own subs and in some cases, secretaries or principals are in charge of the process. The school office staff is also responsible for ensuring the substitute teacher timesheets are completed correctly, signed by the appropriate people and submitted to central office for payment. PAYROLL PROCESS Each month, you will need to complete a BTPS substitute teacher timesheet at each school you sub at. The school staff will forward the forms to central office for payment at month end. The direct deposit will be done on the 10th of the following month. The rate for subbing can be found in Article 8 of the ATA Collective Agreement on our website. We will upload your pay information to our Employee Self Service Web Based Program. Paper copies will no longer be mailed unless you are unable to access your data electronically. If you have provided an email address on your application, you will be accessing the information electronically. In February, your T4 will be uploaded to the same site. It is extremely important that you notify payroll in writing if your name, phone number, address, or banking information etc. changes. Our preferred method of communication is via email to [email protected] If you accept a teaching position with another Board or for whatever reason will be unavailable to sub, you must notify us so that we can have you removed from the school listings and issue your ROE.

Human Resources, requesting authorization to substitute teach in our division.

2. Current resume which includes a chronological listing of previous teaching

experience (grade/subject) and IFX and AFX placements.

3. Three professional references with current contact information including email

addresses listed in the resume (letters may be included). If you just completed

your IFX or AFX placement in a BTPS, we will accept a letter from the principal

and /or co-operating teacher instead of the professional references.

4. Signed and witnessed Conditions of Employment document (the witness cannot

be a spouse or relative).

5. New Employee Form with a VOID personal cheque attached.

6. A copy of your Alberta Teaching Certificate.

7. Original Criminal Records Check (the original will be returned to you at your

request).

8. Original TQS statement (the original will be returned to you at your request). If

you have applied for your TQS and it hasn’t been issued, you will have received a

letter. Please include a copy of the letter. You may be approved to sub without

a TQS Statement but it may impact payment at grid.

1. EMPLOYMENT STATUS: SUBSTITUTE TEACHING ON A CALL-IN

BASIS

2. ACCEPTANCE: In order to be placed on the Buffalo Trail Public Schools approved

substitute teacher list, the Teacher must:

a) complete the “Acceptance” section on page 2, and return this signed form to the

Human Resources Department at Central Services.

3. PROVISION OF INFORMATION: The teacher must:

a) before the date of commencement of employment, satisfy the Board that the

Teacher is a holder of a certificate of qualification as a teacher issued under the

School Act, and

b) comply with such further direction as the Board may give to the Teacher.

4. RULES AND REGULATIONS: The Teacher agrees to be bound by and comply

with all policies, regulations, rules and operational guidelines of the Board in force

from time to time.

5. NO UNTRUTHS: the Teacher expressly promises that the information provided by

the Teacher will be truthful in all respects, and the Teacher will not intentionally fail

to provide the Board with any pertinent information.

6. CRIMINAL RECORDS: The teacher expressly promises that the Teacher has never

been convicted under the Criminal Code of Canada, the Controlled Drugs and

Substances Act or similar legislation of any jurisdiction, of an offence, for which

he/she has not received a pardon.

7. DUTY OF FIDELITY: The Teacher agrees that by accepting employment with the

Board, the Teacher assumes a responsibility to at all times display conduct that is

appropriate to the role of a teacher, that does not harm in any way the reputation of the

Board, and that serves as an appropriate role model for students.

8. CONFIDENTIALITY: It is understood and agreed that all information pertaining to

the Employer’s business to which you become privy through your employment will

be maintained strictly confidential and will not, in any circumstances, be disclosed or

used by you without the express consent of the Employer. This information includes,

but is not limited to, student information, employee information, school and/or

jurisdiction financial information. This confidentiality requirement applies during

and after the course of your employment.

9. EFFECT OF NON-COMPLIANCE: This Offer of Employment is conditional. If

the Teacher does not strictly comply with all terms and conditions, this Offer is null

and void and the Teacher’s acceptance is of no force or effect.
