ANNUAL VALIDATION OF STUDENT PERSONAL INFORMATION Schools are required to advise parents/guardians of the information which is held in their records for students and to ensure information is current. To do this, schools request families to update annually recorded information on the students currently enrolled in the school.

The validation process ensures that details are accurate. Information about addresses, family relationships, contact details and medical conditions can become out of date. As schools have a duty of care to their students, it is vital that this information is accurate.

The Validation Form also includes information about parents and students, permissions for minor excursions, and publication of student photographs and work are also covered.

Evidence of identity and immunisation: if a student’s name has been changed this must be updated on the validation form, evidence of identity or documentation supporting the name change must be provided.

Checklist:Emergency contacts (only those listed in the emergency contact section are permitted to collect students from school).Publication, printing and excursion permissions.
