1. Key Aspects of Submitting Declaration and Information related to Assets and Liabilities

through PRISM

The Officer can file the returns as per the format prescribed under Lokpal and Lokayuktas Act,

2013.

The Officer would be able to view the previous year’s returns(would be helpful in filing return

in future)

The Officer can copy previous year’s returns (would be helpful in filing return in future)

The Officer can save the information filled in the forms.

The Officer can upload the manually filled declaration and information.

The return cannot be amended once the Officer clicks “Submit Application” button.

2. Login to PRISM

Login to the SPARROW using the existing user-ID and password as shown in Fig.1:

As a result, the following page would appear as shown in Fig.2:

3. Filling up the declaration and information forms relating to Assets and Liabilities

Click Declaration link to proceed further. Select the Year for filing the return as shown in

Fig.3.

Click ( ) button as shown in Fig.4:

Fresh Return- Upload Form (Officer has the option to Upload manually filled and scanned Form)

as shown in Fig.5:

Click Upload Form radio button

Browse the pdf file and select

Click Submit button as shown in Fig.6:

A message would appear OK for confirmation for filing the return as shown in Fig.7:

A message would pop up for Successful Submission to confirm that return has been filed as shown in Fig.8:

Note: Scan the copy and browse the pdf file only. Once submitted, the information cannot be amended. A unique Application No would get generated after successfully filing of the return.

3.1 Fresh Return-Fill Form

For online filing select ( ) as shown in Fig.9 :

Fill all the information as shown in Fig.10:

Fill all the Appendix and forms before submitting as shown in Fig.11:

After clicking Submit Application, a message would appear with OK for confirmation as shown in Fig.12:

A unique application number would get generated to confirm that return has been filed

successfully as shown in Fig. 13:

3.2 Copy from previous return

In this case user can use the values from the previous return (immediate previous year).

Here Upload Form ( ) feature is not applicable.

3.3 Nil Return

In case the officer desires, he can click Nil Return button as shown in Fig.17:

As a result of this, all the columns of the form would get pre-populated with “NIL” value. However, wherever required, the officer can fill the information in the desired columns.
