Welcome to the PACAH Spring Conference at Seven Springs Mountain Resort in beautiful Somerset County. The Education Committee along with Kelly and Kim have put together an exciting conference agenda and itinerary addressing many current issues, many thanks to Kelly, Kim and the Education

Committee. We also have left time for a little relaxing during the conference with the golf outing and winery trip on Wednesday and the vendor show and conference 80’s themed party on Thursday. We hope everyone enjoys the educational programs and other activities this week.

One of the goals for this conference is to provide the latest information available on the impact that MLTSS/CHC (Managed Long Term Services and Supports/Community Health Choices) will have on the operation of long-term care facilities moving into the future. The Department of Human Services (DHS) has moved forward with implementing a Managed Care program for Medical Assistance in Pennsylvania to the point that an RFP (Request for Proposals) is in process. Managed Care is scheduled to begin in Southwest PA 1/1/2017, Southeast PA 1/1/2018 and the remainder of the state 1/1/2019. The Managed Care Organizations (MCO’s), approximately twelve (12) known at this point, will have the ability to bid on providing services in each of the five (5) regions in the state. DHS has stated that each region will have two (2) to five (5) MCO’s. Friday this week will feature our MCO Meet and Greet session from 9 a.m. to noon; MCO’s will be available during this time.

PACAH has continued along the path of growth and change started a few years ago with all facility members having equal voting rights. The PACAH Board’s goal is to serve all members of our association while adhering to PACAH’s mission.

Many thanks to those members, facilities and businesses, who are active on the committees supporting the association, and I encourage everyone to become involved is some way as we continue to grow and serve all members.

Dear PACAH members and conference attendees –

Welcome to the 2016 spring conference! I believe it is apparent to our members that the landscape of long-term care is changing significantly. Starting in January 2017, Community Health Choices (CHC) will be implemented in the western region of PA. CHC is the state’s managed long-term services and supports program which will impact all nursing home eligible and dually eligible individuals as well as anyone receiving long-term care services in PA. The program will be fully implemented throughout the commonwealth by January 2019. CHC in Pennsylvania will significantly change the way all of our members are providing care.

This year also brought an increased focus in PA on nursing home quality. This summer, the PA Attorney General filed a lawsuit against the owner of multiple nursing facilities alleging sub-par care, falsified records and deception of state surveyors. Philadelphia Community Legal Services also issued a report accusing the Department of Health (DOH) of failing to properly investigate complaints against nursing homes. On the heels of this, DOH began again accepting anonymous complaints, and the Secretary announced the formation of the Nursing Home Quality Improvement Task Force to determine what additional measures can be taken to improve quality in nursing homes. While we agree that nursing home quality should be at the forefront of the industry’s focus, this has been another significant change in the landscape of long-term care.

These are just some of the major changes that will impact our members’ ability to provide care in the near future, and they could significantly alter how they do business. I believe that change is inevitable, and those who educate themselves and prepare will be best suited to succeed in the future. For this reason, while PACAH remains highly involved in all policy discussions and continues to educate state leaders on how to best implement changes, we also want to provide our members with the best educational opportunities to prepare for impending changes. Our spring conference provides this opportunity, offering attendees the opportunity to meet and network with the MCOs involved in CHC as well as educational opportunities on issues such as CJR, surviving surveys, updates from DOH and OLTL, managed care in a long-term care setting, and more. We will continue to provide opportunities for education and preparation, and it is important that members take the initiative to participate in these opportunities, read our emails and newsletters, and learn what to do to adapt to future changes.

We have also continued to make improvements and achieve many of our goals since the last conference. We have a new President, Terry Brennan, from Berks-Heim Nursing and Rehabilitation, who continues to lead the organization in a positive direction. We have instituted a New Member and Attendee Happy Hour at the conference to welcome new faces. We are bringing back the PACAH awards and are also offering some new educational credits at our conferences, including those for Assisted Living (RCAL) and Personal Care. We have also continued to work on our legislative and policy goals, including meeting with the MCOs on CHC, moving HB 1062 (to eliminate the county share), advocating for improvements in nursing home rates, and working on the Intergovernmental Transfer.

As always, I think we should all be proud of PACAH and the association it has become. We continue to fulfill the goals as outlined in our mission statement by advancing excellence and improving the quality of care and life through legislative and regulatory advocacy, innovation, education and professional networking, provided in a close-knit and affordable setting. This could not have happened without the leadership of our president and the executive board, the assistance of our Meeting and Marketing Manager, Kim Deline, and participation of all of our members. Nomination Process:Both full-voting and business members are eligible to nominate PACAH members for awards. Please complete the nomination form - which will be published on the PACAH website soon - and return to PACAH by August 1, 2016. The nomination form will also be distributed to all members via email. The Executive Board will review the award nominations and select winners for all awards. Awards will be presented at the PACAH 2016 Fall Conference Luncheon.

AWARDS CATEGORIES

Administrator or CEO of the Year: This award recognizes administrators and/or CEOs from member facilities who have shown innovation, dedication and commitment to their facilities, residents and staff, as well as the advancement of long-term care in general. Recipients should also have demonstrated leadership through innovative programs, communication, and education. Special projects or success in dealing with challenges will be taken into consideration, as will time spent furthering and shaping long-term care in Pennsylvania through advocacy, involvement and leadership with PACAH.

CFO or Fiscal Director of the Year:This award recognizes fiscal directors or CFOs of member facilities who have shown innovation, dedication and commitment to their facilities, residents and staff as, well as the advancement of long-term care in general. Recipients should also have demonstrated leadership through innovative programs, communication, and education. Special projects or success in dealing with challenges will be taken into consideration, as will time spent furthering and shaping long-term care in Pennsylvania through advocacy, involvement and leadership in PACAH.

Innovation Award:This award will recognize a member facility or business for innovative work done to improve the quality of resident care that can be used as an example for other PACAH members who may be interested in replicating the improvements. Innovations can be made in the field of technology, provision of care, programing, etc. The unique and creative nature of the project will be considered as well as the time and effort needed to implement the project and any measurable improvements that have been obtained as part of the project. Utilization of special funding or grants should also be noted.

Business Member of the Year Award: The recipient has demonstrated a continued commitment to the improvement of long-term care and has assisted PACAH members in the performance of their duties, through leadership, education, and advocacy with the association and involvement in association activities above and beyond the scope of normal levels of assistance. Involvement in PACAH committees, conferences, advocacy and educational opportunities as well as commitment to furthering member benefits for the association will be considered.

Friend of PACAH Award: The recipient has demonstrated a continued commitment to the improvement of long-term care and has assisted PACAH members in the performance of their duties, often, above and beyond the scope of normal levels of assistance, enabling member facilities to provide excellent service. The recipient has shown continued commitment to PACAH and its goals and positions. This is the highest honor awarded to a nonmember and may be made to an individual or company/agency.

Welcome to Seven Springs!

Seven Springs is a borough in Somerset and Fayette Counties. The borough and resort are part of both the Pittsburgh metropolitan area and the Johnstown Metropolitan Statistical

Area (Fayette County is part of Pittsburgh metro; Somerset County is part of Johnstown metro). The population was 127 at the 2000 census. The 2010 census reported a huge percentage decline down to only 26 inhabitants, of whom 15 were in Fayette County, and 11 in Somerset County.

It is most well known for Seven Springs Mountain Resort in Somerset County, although some areas still list the resort’s business address as Champion, an unincorporated village located across the border in Fayette County.

BREAKOUT SESSION - “CJR – PROVIDING THE FIRST GLIMPSE INTO PAC REIMBURSEMENT FOR THE FUTURE”9:15 – 10:15 a .m .SnowflakeNHA – 1RN – 1CPE – 1

CMS has finalized the Comprehensive Care for Joint Replacement (CJR) model, with implementation set for April 1, 2016. CJR will hold hospitals accountable for the quality of care they deliver to Medicare fee-for-service beneficiaries for hip and knee replacement procedures from surgery through recovery, and cause the hospitals to now become the gatekeeper for both the “Payer and the Provider”.

The CJR is likely the first of a new wave of mandated bundles from CMS, and may pave the way for similar moves by other payers. Hospitals, skilled nursing facilities, and post-acute care providers should view the joint program as a template to build upon in a value-based future rather than an aberration of fee-for-service to be minimized or ignored. Non-traditional players, such as home care, meal services, and other senior housing providers, should view the CJR as a new market opportunity.

This session will review alternate payment models, CMS goals, highlight the Top 10 things to know about the CJR, and identify necessary SNF value-propositions, as well as provide an overview and outcomes of a two year pilot which designed a concept for a more coordinated surgical care model by optimizing perioperative care for orthopedic patients.

Today, health care organizations are under constant attack from cyber threats. In this session, we will review the top 10 most common IT problems and ways they can reduce those risks. Many individuals have been drawn to watching the “Hoarding Show” on the cable network! Hoarding behaviors have become a fascination! Unfortunately hoarding is a social, mental health and health concern. Though difficult to understand individuals who hoard have difficulty discarding belongings due to the emotional attachment and belief they may need the items at some other point in their life. They end up collecting so much that their actual “living space” may be confined to a small room or even a small area in their home. Though we usually see problems with hoarding behaviors in older adults, the actual “collecting” may begin at an earlier age, such as in adolescence. Hoarding effects between 2-5 percent of the population, but due to stigma it does not surface until a crisis occurs. BREAKOUT SESSION - “CJR – PROVIDING THE FIRST GLIMPSE INTO PAC REIMBURSEMENT FOR THE FUTURE”9:15 – 10:15 a .m .SnowflakeNHA – 1RN – 1CPE – 1

CMS has finalized the Comprehensive Care for Joint Replacement (CJR) model, with implementation set for April 1, 2016. CJR will hold hospitals accountable for the quality of care they deliver to Medicare fee-for-service beneficiaries for hip and knee replacement procedures from surgery through recovery, and cause the hospitals to now become the gatekeeper for both the “Payer and the Provider”.

The CJR is likely the first of a new wave of mandated bundles from CMS, and may pave the way for similar moves by other payers. Hospitals, skilled nursing facilities, and post-acute care providers should view the joint program as a template to build upon in a value-based future rather than an aberration of fee-for-service to be minimized or ignored. Non-traditional players, such as home care, meal services, and other senior housing providers, should view the CJR as a new market opportunity.

This session will review alternate payment models, CMS goals, highlight the Top 10 things to know about the CJR, and identify necessary SNF value-propositions, as well as provide an overview and outcomes of a two year pilot which designed a concept for a more coordinated surgical care model by optimizing perioperative care for orthopedic patients.

The Department of Human Services (DHS) deputy secretary for the office of long term living will report on current initiatives undertaken by her office. This will include updates on DHS’s plans on implementing Community Health Choices (CHC) in Pennsylvania, the FY 2016-2017 budget proposal, and other new initiatives and innovative projects being undertaken by the Department of Human Services, in particular, the Office of Long Term Living.

Despite convincing evidence since the 1840s that improved hand hygiene reduces infection rates, healthcare worker compliance with hand hygiene is consistently suboptimal in many healthcare settings. Healthcare facilities may fall short of goals to improve hand hygiene compliance if that improvement is dependent solely on the availability of alcohol-based hand rubs (ABHR) stations, the deployment of current monitoring methods, and compliance with regulatory and professional standards and guidelines. Implementation of a credible hand hygiene program can be enhanced by using systems that target healthcare delivery workflow with strategies that influence healthcare worker behaviors and integrate hand washing into patient care activities. The presenter will demonstrate application of a new model to map specific hand hygiene improvement interventions to internal and external motivators of behavior to close the gap on healthcare worker hand hygiene performance. This session introduces a multi-modal framework of system and behavioral strategies that are vital to investigate, understand, and mitigate gaps in hand hygiene compliance, remove obstacles to hand hygiene performance, and convince healthcare workers that hand hygiene compliance is valued, expected, and important.

Medication management in long-term care is so much more than just taking a pill. Those entrusted to care for the elderly must strive to maximize the benefits of medications while minimizing the often significant risks associated with them. Failures in this process can have dire consequences for residents and threaten the “regulatory health” of facilities. And often overlooked in this complex equation is the resident’s right to be actively involved in all decisions related to their medications. This presentation, given by a Doctor of Pharmacy and the former Chief Nurse of Maryland’s state survey agency, will explore the rights and responsibilities of clinicians, residents, family members and regulators relative to medication management in long term care.

Survey Deficiencies, Staffing and Quality Measures will be discussed, outlining how a facility should analyze the current 5 Star Rating in each area, set reasonable targets for improvement in each area within a reasonable time frame, and figure out an action plan to achieve the target.
