2016-17 ANNUAL REPORT

Programs and services of the Humboldt Senior Resource Center (HSRC) are vital for the health and welfare of to-

day’s aging population in Humboldt County, as well as for the projected significant growth of the next generation of users. HSRC’s mission is as vital today as it will be in the future.

HRSC’s successful history can be attributed to the efforts of its dedicated staff and tireless volunteers as they provide quali-ty services that meet the participants’ needs. This quality work could not be effectively achieved without the fantastic support and generosity by individuals, organizations, foundations, partnerships, and government agencies.

This year, the Board of Directors finalized a four-year (2017-2021) Strategic Vision that resulted in three strategic goals:

The HSRC Board of Directors

1. Support all current programs with Redwood Coast PACE’s growth having priority status; envision a licensed and operational PACE Fortuna site serving Eel River Val-ley; and seek funding partners for programs. 2. Achieve competitive salaries and benefits for employees. 3. Restore the Nutrition program to previous service levels.

I’m pleased to be able to report that HSRC is meeting the challenge and is moving rapidly forward into the future. Prog-ress is being made in the renovations on the Eureka campus to provide improved facilities for Adult Day Health; PACE is meeting its growth targets, serving more than 130 partici-pants; and we are actively proceeding with plans for a facility to expand our services in Fortuna. A working agreement with Times Standard newspaper has greatly increased the circula-tion of our informative Senior News , which is now included in the newspaper; thank you Senior News advertisers.

Enjoy reviewing this 2016-2017 Annual Report, which highlights the programs and services that are provided to our local aging population. Thank you for your support and con-tributions which have made an important difference for many of Humboldt’s seniors; you are a big part of this success story. Proper nutrition plays an increasingly important role in aging, and can affect quality of life as a person ages. Older adults with food insecurity run the risk of malnutrition, which in turn can lead to poor health or disease and then loss of independence. The meals provided by HSRC’s Senior Dining Centers and the Home Delivered Meals program not only reduce hunger and food insecurity — they enhance socialization and provide contact for homebound recipients, as well. Home delivered meals were provided to homebound elders from McKinleyville to Fortuna.

Recreation helps to improve all aspects of an aging senior’s life, including cognitive functioning, physical strength, and social interaction. Exercise can help prevent serious physical and emotional problems such as depression, chronic conditions, and memory decline. Services offered through HSRC’s Activities Program feed the soul of each participant, enhance social engagement, and encourage health and well-being.

Senior News is the area’s only monthly publication for those age 50 and better. Senior News features local columnists and community members, health information, local happenings, and area senior center activities. It is available at more than 120 locations in Humboldt & Del Norte counties.

Submissions are encouraged; advertising and subscriptions are available.

Promoting Independence

HSRC’s recreational opportunities include the ukulele group, who meet bimonthly in Eureka.

The Multipurpose Senior Services Program (MSSP) provides social and healthcare management for frail elderly clients age 65 and older who are eligible for nursing home placement but who wish to remain in the community. MSSP promotes elder independence by arranging and monitoring the use of community services to prevent or delay institutional placement of these frail clients.

ADULT DAY HEALTH & ALZHEIMER’S SERVICES

(ADH&AS) Services provided to Adult Day Health participants include:

• Nursing care

• Physical, occupational, and speech therapies

• Mental health services

• Therapeutic activities

• Social services

• Personal care

• Hot lunches & nutritional counseling

• Transportation to and from the Day Center

Adult Day Health Care (ADHC) is a licensed, community-based day health program providing services to those age 18 and up with chronic medical, cognitive, or mental health conditions and/or disabilities who are at risk of needing institutional care. The ADHC model of care stresses partnership with the participant, family and/or caregiver, primary care physician, and the community in working toward maintaining personal independence.

HSRC’s Alzheimer’s Services provides information about Alzheimer’s and other dementia-related conditions, and resources including caregiver assistance and support groups – all available to anyone in our community who needs help in living with, or helping someone, with these syndromes. The Alzheimer’s Center’s resource library is open to the public.

A Continuum of Care

Our PACE enrollment staff can provide a full list of services covered by PACE.

Redwood Coast PACE— a Program of All-inclusive Care for the Elderly—is a comprehensive health and supportive services plan that helps qualified nursing home-eligible adults age 55 and older to continue living as independently as possible in their homes and community. The PACE Interdisciplinary Team coordinates each participant’s care plan of medical, social, and home care services so that these elders no longer need to manage their complex medical care alone.

Donations from our generous community during our 2016-2017 fiscal year helped HSRC to:• Offer more than 1,325 activities enjoyed in more than 11,200 participant visits

• Publish 13,000 copies of Senior News monthly

• Produce close to 500 meals daily in our Central Kitchen for our Nutrition, Adult Day Health, and PACE program participants

• Serve an average of 150 meals every Tuesday – Friday at our three Senior Dining Centers — 30,000 meals annually

• Deliver an average of 150 meals each weekday to homebound elders — 40,000 meals annually

• Offer 72 caregiver support groups annually and provide 253 consultations to assist community members with Alzheimer’s-related information and resources

• Care for 80 Adult Day Health participants

• Arrange care and services for 104 MSSP clients

• Provide comprehensive health care and support services for 124 Redwood Coast PACE participants

• Employ 120 staff who provide these services to our community’s seniors, their families, and their caregivers to help promote elder independence
