American High School Prom 2014

Permission Slip to Attend Prom Each AHS student attending prom must complete this form prior to purchasing a Prom Bid.

A NON-AHS guest will complete an additional Guest Pass form. _______________________ has my permission to attend the Prom on the evening of May 31, 2014. (Name of AHS student) I understand that my son/daughter will be responsible for his/her own transportation to and from the Galleria in San Francisco. The dance begins at 8:00 p.m. and ends at 11:45 am.

AHS Students: Please initial below that you understand each of the following expectations:

� Behavior at school dances will be closely monitored. Possession of, or being under the influence of any controlled substance is prohibited. Guests in violation’s parents will be contacted, asked to leave the dance and the police may be contacted. A Breathalyzer may be used at the discretion of the administration.

� Doors will be closed at 9:00 P.M. (Students may be admitted after 9:00 P.M. with prior permission of the

administrator in charge).

� American High School is not responsible for any items left at the coat check or brought to Prom.

� Guests must be accompanied by a AHS student and must not be above the age of 20 years old at the time of the dance. AHS students and guests must show a current school I.D. card (photo ID for out of school guests), A guest is a NON AHS student

� There are no “in and out” privileges at dances. Once a student exits the dance he/she will not be readmitted. Students or guests wearing revealing attire or with inappropriate sayings will be sent home. The administrator in charge will determine what is revealing or inappropriate.

� Students and guests must dance in a safe and appropriate manner. Demeaning or sexually explicit dancing,

groping, fondling, bending over, lap dancing, floor break dancing, and moshing are not acceptable.

� I understand that cutting on Friday, May 30th or Monday, June 2nd will not be tolerated. If a student is absent due to illness or other emergency, THE ABSENCE MUST BE CLEARED BY A PHONE CALL FROM A PARENT/GUARDIAN BY 3:00 P.M. ON FRIDAY. STUDENTS WHOSE ABSENCES HAVE NOT BEEN CLEARED BY THIS TIME WILL NOT BE ADMITTED TO THE PROM ON SATURDAY NIGHT.
