Addendum #1 Addendum No. 1 Project: Three Points Pump Station Upgrades Control No: 613 Issued: To all listed plan holders Date: August 4, 2017 This addendum forms a part of the Agreement described above. The original Contract Documents and any prior addenda remain in full force and effect except as modified by the following, which shall take precedence over any contrary provisions in prior documents. 1. Section 00330Sup– Unit Price Schedule DELETE Section 00330Sup, in its entirety, and REPLACE with revised Section 00330Sup, attached. In addition, the deleted page is of an obsolete version, and Pay item 7001 – Tree Removal and Disposal has been added. 2. The following are questions received prior to cutoff along with their responses: Question: Is blasting allowed for this project? Answer: No, this project is too close to adjacent homes. Question: What is the amount of flow that will have to be bypassed for this project? Answer: The specs call for 125% of peak flow. The bypass pump system will have to be capable of pumping 310 gpm. Question: The plans show on sheet 3 of 12 the gravity sewer line entering the pump station to be 10” DIP but on sheet 5 of 12 it shows it to be 12” DIP. Can you please clarify which size? Answer: The correct pipe size is 10”. Question: Is there any manhole closer than shown to possibly cut & plug? Answer: No Question: Is there any geotechnical information relative to the site? Answer: See soil boring information provided with this addendum. Question: What is the new tower height? Answer: Assume 100 ft tower height. 3. Clarification Items: Length of force main that will drain back when it is cut to make the tie ins - ~3500 feet (~5100 gallons)

Addendum #1

• Backfill material where existing structures are removed, must be dense graded stone placed in 8-inch (loose) lifts and compact to 98% standard proctor density at +/- 2% of optimum moisture content.

• The existing SCADA tower pad is to be removed by Contractor, and KUB will haul it to Middlebrook.

• Tower supplier will provide a SCADA pad / foundation detail for new tower.

• Contractor is to remove the existing tree just outside the northern corner of the existing fence. The tree has been marked with paint and ribbon. (See item 1 above, referencing this new pay item).

• The existing drainage swale on the north side of the site is to be stabilized to prevent further erosion. The swale shall be reshaped and lined with rip-rap in accordance with the “RIP-RAP OR LINED TRAPEZOIDAL SWALE DETAIL” on Plan Sheet 11. The swale is approximately 100 feet in length.

• At the northern corner of the existing fence, where the swale to be stabilized exits the site, the eroded area is to be stabilized as follows:

1. Remove the existing silt fence. 2. Excavate the silt from the eroded area. 3. Shape the excavated area to form a smooth transition between the

stabilized swale and the swale on the north side of the existing fence line. 4. Place 8 ounce non-woven heavy duty filter cloth over the excavated area,

anchored on all sides in accordance with manufacturer’s recommendations.

5. Place rip-rap (nominal 8-12 inch size) over the filter cloth at a minimum of 24-inches depth.

6. Budget 15 cu yd of excavation and 10 cu yd of rip-rap for repair of the eroded area.

This work is to be included in Pay Item 1005h – Erosion Control Plan & Implementation

4. Drawings: (pdfs of drawing sheets may be found at www.kub.org. Click the Procurement link, then Open Bids & Requests for Proposals, then scroll to find the above referenced project & its attachments)

• DELETE sheet 5 and replace with revised sheet 5 attached, which reflects 10” DIP, not 12”

Each Bidder/Proposer shall acknowledge receipt of this addendum by affixing his signature below, by noting this addendum on his Bid/Proposal Form, and by attaching this addendum to his Bid/Proposal. Failure to acknowledge this addendum could be cause for bid/proposal rejection.
