2 Purpose of the DocumentThe purpose of this Project Initiation Document (PID) is to define how [insert PCT here] has decided to implement the NHS Summary Care Record and HealthSpace. The document will provide a framework for the management and assessment of the delivery of this Project at a local level. It will contain information about the project, relating to what, why, who, how and when.

This document is primarily aimed at defining the first phases of the project of the project, which will public information programmes (PIP) and SCR initial uploads for those GP practices using clinical systems with full rollout approval. The PID will be further developed following the SCR initial uploads to include remaining GP practices and the other aspects of the SCR project.

3 Communication and Engagement

Careful planning and delivery is necessary if we are to fully engage our community and avoid isolating hard to reach groups.

3.1 Communication Plan

The communication plan aims to involve both internal and external stakeholders in the roll out of the SCR project and incorporate the national and local messages in accompanying materials. An example communications matrix is included as Appendix 2.

The internal communications aims to ensure that employees affected by the roll out of SCR are fully informed about the upcoming changes and feel confident when talking to members of public about the scheme if asked any questions; also to provide support to employees who are participating in the accelerated rollout, for example, that enough information is provided and clear lines of communication are established if there are any queries.

3.2 Public information programme (PIP)

The external communications will incorporate a PIP which aims to inform the patients of the GP practices currently involved about the upcoming introduction of SCR and establish two-way communications to ensure that they are able to make an informed choice about whether to have a SCR record created and are given the opportunity to discuss the benefits of SCR and any concerns they might have.

There will also be a need to target hard to reach sectors of the community.

4 Project PlanThe project plan will be initially for the accelerated rollout only. This will plan activities up to the SCR initial uploads for those practices involved in the accelerated rollout. A comprehensive longer term plan which covers all Practices and end users will be produced which details the timescales and activities across the whole [insert PCT name here] health community.

It will be crucial for experience gained and lessons learnt from the accelerated rollout to be built into the ongoing full roll out.

5 Risk and Issue ManagementRisks and Issues will be captured and managed in a Risk and Issues Log which will be maintained by the Project Manager. This will be reviewed by the Project Board, where decisions will be made to escalate within the internal and external Governance structure.

The Risks and Issues Log will be aimed at the full implementation of SCR across the [insert PCT name here] health community.

The currently identified key Risks and Issues are identified below in Table 11.
