Program Facilitator

Do you have a good understanding of what is going on in our Community? Due to growth in our Service
We are looking for a Full Time Program facilitator to plan activities across our organisation.
The Program facilitator will be employed to create & plan activities, within the scope of the
participant’s individual NDIS plans
38 hours per week

The Leisure Company is a not for profit organisation that is a registered provider for the
NDIS. The organisation offers recreation, work and life skills opportunities to people with a
disability. The purpose of this position is to create and plan activities across the organisation
to meet the interests and goals of the participants.
Position Title Program Facilitator
Responsible for Planning a diverse range of activities to meet the individual needs and interest of the participants accessing The Leisure Company.
Award Social, Community, Home Care and Disability Services Industry Award 2010.
Classification Level Social and Community Services Employee
Level 3
Hours of Work 38 hours per week

Program Facilitator
Statement of Duties
Quality Services
Plan activities across all supports that The Leisure Company provides
Consult and liaise with Plan Coordinators to develop and organise activities that
reflect the goals of the participants NDIS Plan
Plan a diverse range of activities to meet the needs and interests of all participants
Compile and distribute a quarterly Activity Calendar for after hours and weekend
activities
Plan overnight and days trips as required.
Ensure that activities focus around integration and inclusion
Develop engaging and age appropriate activities keeping in mind participant’s
capabilities are considered.
Thorough and regular communication is maintained with relevant stakeholders
Group evaluations are up-to-date and actions are followed through
Plan appropriate activities for school holidays, cancelled activities and for
restructured groups
Monitor all activities in partnership with the Plan Coordinators
Respond to email suggestions, enquires and requests from Support Workers, Plan
Coordinator and Participants promptly
Attend weekly Plan Coordinators and Rostering Coordinator meetings – making
changes as required to group activities and procedures folders
Liaise with Rostering Coordinator for vehicle bookings
Coordinate room bookings for activities that are centred based
Organise venue bookings as required.
Organise instructors/guest presenters as required
Participants Focus
Participants receive activity information in a timely manner
Ensure the cost of the group spending is in line with the group contribution of
participants
At times you may be required to provide quality support to participants in a group
activity. These supports will need to be provided in line with participants support
plan
Provide support that is appropriate to the participants culture, religious beliefs, life
stage, sexual identity and behavioural needs
Support participants within the community positively
Use appropriate tools and technology to support participants skill development
Contribute to an effective workplace
Communicate appropriately with participants , co-workers and community members
Respect the privacy and confidentiality of participants, families and carers and co-
workers
Work in a self-motivated way and be accountable for your actions
Organise your own work tasks to meet priorities and participant outcomes
Be flexible to the changing circumstances of your job requirements and or
participants needs
Participate in training opportunities, both formal and informal, offered by The
Leisure Company
Assist in the orientation of new staff (co-workers) in the workplace
Show respect and work cooperatively with co-workers from diverse backgrounds
Contribute and work towards team objectives
Be flexible to the changing circumstances of your job requirements or participants
needs
Resolve conflict within the scope of your role.
Organisational Requirements

Organise your own work tasks to meet priorities and participants outcomes
Complete and maintain documents in a timely and accurate manner.
Maintain confidentiality for all participants files, information and organisational
resources and processes.
Develop and maintain community networks.
Ensure any concerns and issues are raised with the Human Resource Officer
Work, Health and Safety (WHS) Requirements
Follow all WHS policies and procedures.
Identifies risks as appropriate and report to Human Resource Officer
