Silicone or saline? That is the question that many women on the market for a breast augmentation face. When weighing the options, there have always been strong arguments for both sides. Thanks to new technology, the future of breast augmenta-tion is here with a new implant that has the best of both worlds.

Up until five years ago, the salt water filled implant (saline) has been the preferred implant. The standard silicone implant has since returned to favor, but there has always been that lingering doubt of what happens if it leaks.

SALINE IMPLANT BENEFITS:• Non-toxic in the event of leakage since

it is filled with salt water.• Size can be varied by merely adding

more fluid to enlarge it.

SALINE IMPLANT DRAWBACKS: • Does not feel natural.

• Tends to reveal itself more by producing ridges on the breast.

• Higher chance of leakage.

Over the past five years, many of my patients have chosen the silicone-filled implant because it does feel much more natural, and it tends to not show the ridges beneath the skin that the saline implant does. Nonetheless, it still causes some con-cerns if there was leakage, and the silicone was to leak into the breast tissue.

Currently, I feel the saline implants and the standard silicone implants will be obsolete within the next two years. This is due to the current availability of the revolution-ary, high tension, gel implant – AKA

the “Gummy Bear” implant. The term is coined from the fact that when cut in half, the breast implants are able to retain their shape with no leakage, similar to the popu-lar gummy bear candies.

The benefits of the “Gummy Bear” implant are so pronounced that it has become my implant of choice.

“GUMMY BEAR” IMPLANT BENEFITS:

• Smaller chance of a leakage of gel into the body if a rupture is to occur.

• Natural feel, similar to the standard silicone implant.

• Multiple shapes and sizes offered.• Cost is very comparable to the standard

silicone implant.• Some studies suggest that there may be

less of a chance of scarring around the Sientra “Gummy Bear” implant.

“What caused my acne scars, anyway?”

In the simplest terms, your tissue has been injured from active acne episodes, which create inflam-matory responses to the bacteria and dead cells in your plugged hair follicles. TYPES OF ACNE SCARS:“Ice Pick” scars. These deep pits are the most common of acne scarring.

“Box Car” scars. Angular scars that usuallyoccur on the temple and cheeks. Can either be superficial or deep. These are similar to chicken pox scars.

Rolling scars. Scars that give the skin a wave-like appearance.

Hypertrophic scarring. Raised, usually inflamed or reddened scars. More common on the back or chest. Appears thick and lumpy, sitting above the surface of the skin.

When can I expect to go through menopause?The definition of menopause is the spontaneous cessation of menstrual periods for one year. The average age of menopause is about 52, but can occur anytime from age 40 on. Some women don’t stop having periods until their late 50s. Furthermore, the perimenopausal symptoms can occur five to ten years before ac-tual menopause. Menopause can occur at any age if the ovaries are surgically removed for a variety of medical reasons.

Menopause is a “natural” part of aging, so no big deal, right?For a few lucky women, that is correct, but menopausal symptoms vary greatly from one woman to the next. Some women can find the symptoms very distressing. The flushing and sweating can be uncomfortable and embarrassing, and hamper their activities and desire to be around others. The changes in sexual drive and response, along with vaginal dryness can cause real issues in intimacy. The poor concentration and “mental fog” that many women experience can adversely affect job perfor-mance and ability to get things done at home. Women with underlying depression or anxiety problems often find their symp-

toms worsened. Top this all off with sleepless nights, and many women find the perimenopausal time a real challenge.

So, how can I tell if I will have trouble going through menopause?For the most part, you can’t. Genetics have a part to play, so if your mother had a tough time, you are more likely to also. Women who had difficulty with other hormonal changes (puberty, postpartum, PMS) are also more likely to have a more difficult transition into meno-pause. Women in good physical and mental health coming into menopause seem to get through easier. This includes regular exercise, eating healthy, quitting smoking, having a good support system, and keeping a good sense of humor.

How long will I have to endure menopausal symptoms?Many of the symptoms tend to improve with time. Hot flashes and night sweats usually lessen in frequency and intensity. Eighty-five percent of women have reso-lution within four to five years of menopause. An unlucky few will continue to have them indefi-nitely. Some symptoms such as vaginal dryness may continue to worsen if not treated.

What can I do to decrease menopausal symptoms once they are present?Many lifestyle changes can be helpful. Keeping the thermostat a couple degrees cooler and dressing in light layers that can be removed may help. Avoid triggers such as alcohol, hot fluids, spicy foods, caffeine-whatever you notice seems to make symptoms worse. Learn how to relax and reduce stress. This is not the time to take on a bunch of new responsibilities at work or remodel the house. Use your support system to help get you through this rough time. Chances are some of them are going or have been through similar experiences. Try a couple of over-the-counter products. Also, consider a good medical checkup. Several medical condi-tions can have similar symp-toms, or worsen menopausal symptoms.

I’ve tried self-help and I’m still miserable! Can I safely use hormones?Short answer: Yes, for most wom-en. Newer regiments with lower doses and transdermal (through

the skin) absorption, instead of oral dosing, are safer and can benefit many women. Vaginal dosing for local symptoms is appropriate for nearly all women. There are some non-hormonal alternatives for women who don’t want to, or medically should not use estrogen. Two quick notes:1. Salivary testing of hormones is expensive and doesn’t reflect blood levels, and therefore not recommended.2. “Bioidentical Hormones” is a marketing term, not a medical term. The same hormones are available in FDA-approved formu-las, which are usually cheaper, and have better quality control.

Do it anywhere! Workouts can be done at a park, outside at home, the beach or at the local high school track. Get creative!

Circuit It! Circuits are a great way to keep workouts fun and effec-tive! For example, four people select four simple body weight exercises and bring a stop watch. For 30 seconds, each member will perform an exercise, and then recover for another 30 seconds before performing the next exer-cise until all four are completed. That is ONE circuit. Shoot for 2-3 circuits.

Circuit Ideas: squats, push-ups, jumping jacks, lunges and moun-tain climbers.

Use what you have! If the group is working out at a park, look for a bench to do some incline push-ups, dips or step-ups. A set of stairs is great to run up, and playground equipment can be used as well. Example: a chin-up bar to do pull-ups.

Be a kid! Make it fun and creative by playing some games! Play tag, find a rope and have a tug of war contest, create an obstacle course or have relay races. It won’t even seem like a “workout.”

End strong! Always try to run a lap for a great finish! Bonus points: treat yourselves to a healthy picnic when finished!

Share your success! By working out with a group, your workout partners will hold you account-able, motivate and encourage you. You will share success and frustrations, but you are more than likely to continue and be consistent with your workouts.

There’s no question that what you eat affects your health, including the health of your skin. If your diet is unbalanced or too heavy in unhealthy foods, your skin could end up paying the price. Keep these pointers in mind when planning a diet for the benefit of your skin!

Eat your antioxidants! Antioxidants help fight free radi-cals caused by sun damage, which can ultimately prevent wrinkles, dullness and other signs of aging. Great examples include: blueberries, carrots (and other yellow and orange vegetables), spinach, fatty fish, tomatoes and nuts.

Aim to drink eight glasses of water daily. Water helps keep cells hydrated, which allows cells to move nutrients IN and move toxins OUT.

Get that healthy fat! Your skin relies on healthy fat as part of it’s source for moisture. Shoot for at least 20 grams of healthy fat daily - the equivalence of two tablespoons of olive oil. Healthy fats can also be found in avocados, almonds, walnuts and fatty fish.

Substitute refined carbs for whole grains. Potatoes, pasta, white bread and rice may be taking a toll on your skin. Refined carbohydrates cause your body’s insulin levels to spike, setting off a chain of reactions that may ultimately lead to acne. Substituting healthy whole grains, such as brown rice, whole wheat pasta, oatmeal and whole grain breads, for refined carbohydrates may improve your skin’s appearance.

Concentrate on a healthy diet in general. Remember, many of the best foods for healthy skin also promote good health overall. Eat plenty of fruits and vegetables. Choose low-fat or fat-free dairy products. Include lean protein, nuts, seeds and beans in your favorite meals. Opt for whole-grain and limit sweets and refined carbohydrates. Strive for variety in your diet as you’re making healthy choices!
