CONVERGE® Pro Certificate of Destruction
In order to show adequate proof of destruction of the equipment, you must:
1) Include a photo(s) of the destroyed product, clearly showing the product’s serial number. Serial number can be found either on the
back panel or on the bottom panel depending on the year in which the product was manufactured. Product destruction is completed
by removing the PCB board and drilling holes in it, crushing it or breaking it into pieces.
2) Recycle the old equipment being submitted for the promotion and submit a receipt validating proper disposal or a Recycled
Certificate.
I the undersigned, hereby certify that I have taken the product listed below out of service and destroyed or recycled it. I understand that
only by destroying or recycling the equipment I am qualified to receive rebates towards the purchase of eligible products.
I understand that I must include a photo of the destroyed or recycled product with the product’s serial numbers clearly visible.
