Organised population-based screening for cervical cancer, breast cancer and colorectal cancer has been conducted in Finland for several years. Cancer is suitable for population-based screening, if the disease is common, its precursors are established, and by detecting these, one can prevent cancer. Also gastric cancer meets all these requirements. In Finland, with a population of 5,4 million, 600–700 new cases of gastric can-cer are diagnosed every year. Because of a delayed diagnosis, this cancer typically has a poor prognosis, i.e., 5-year survival rate of 20 %.
The two most important risk factors of gastric cancer are Helicobacter pylori infection and atrophic gastritis (AG). Both conditions can be accurately diagnosed by a simple blood test, known as GastroPanel. The 13C urea breath test (UBT), stool antigen test and antibody tests do not detect atrophic gastritis which is caused by Helicobacter pylori infection or an autoimmune disease. The UBT and stool antigen test may give up to 50 % false negative results for Helicobacter pylori infection if the patient has a) AG, b) MALT lymphoma, c) bleeding peptic ulcer disease, or d) if the patient is currently receiving antibio-tics or PPIs.

Prevention of gastric cancer generates significant cost savings
test indicates atrophic gastritis, the subject is at risk for developing malabsorption of vitamin B12-, iron-, calci-um- and magnesium as well as some drugs. Malab-sorption of vitamin B12 increases the risk of dementia, Alzheimer’s disease, depression and polyneuropathies, all of which would benefit from early diagnosis to enab-le effective prevention and treatment.
The GastroPanel blood test enables early detection of these risk groups and their referral for further tests and specific therapy. (3,4). At national level, the GastroPanel screening of one specific age cohort, e.g. 50-year-old people (n=68.000), for the known risk factors of gastric cancer (Helicobacter pylori and atrophic gastritis) would generate savings of tens of millions of euros in the pub-lic health sector in Finland.

A test for Helicobacter pylori and atrophic gastritis
It is estimated that 15–20 % of Finnish population has Helicobacter pylori infection. In most cases, this infec-tion is asymptomatic, but it can also be responsible for a variety of nonspecific abdominal symptoms (dys-pepsia).
In the stomach mucosa, Helicobacter pylori infection starts as superficial gastritis, but if not eradicated it becomes chronic and eventually develops into AG and anacidic stomach. (1)
Atrophic gastritis is the cause of anacidic stomach, which increases the risk of gastric cancer up to 90-fold, when severe and affecting the entire stomach (antrum and corpus).
Currently, some 4 % of the Finnish population is af-fected by atrophic gastritis, but of people over 70 ye-ars of age, some 8 % suffer from AG. (2)

Test performanceCarefully shake the Sample Collection Tube. Unscrew and remove the white cap. This reveals a dropper. Drop 3 drops onto the Sample well of the Hb or Hb/Hp test strip in the cassette. The sample is absorbed in 5 to 15 minutes. Results can be read after 15 to 25 minutes.

Where does acetaldehyde come from and where is it found?
Acetaldehydeisthemostcommoncarcinogenintheworld.Itcanbefoundinmanytypesoffoodaswellasinalcoholandtobacco.
Acetaldehydeisnaturallypresentinmanytypesoffood.Itisparticularlyabundantinfoodproducedbyfermentation,suchasalcoholicbeverages,vinegar,dairyproducts,home-brewedbeerandmead.
Duetoitspleasantapple-likesmell,acetaldehydeisalsousedasaflavoringinsomepastries,fruitjuices,softdrinks,sweetdessertsanddairyproducts.
Themostsignificantsourcesofacetaldehydeexposurearehighconsumptionofalcoholicbeveragesandsmoking.
How does acetaldehyde act in the body?
Inanormal,healthystomach,hydrochloricacid(HCI)cankilltheacetaldehyde-producingmicrobes(yeastsandbacteria)ofthedigestivetract,whicharecarriedfromthemouthtothestomachwithsaliva.Incertainconditionsthesemicrobesformacetaldehydefromalcoholbyoxidationorbyfermentationofingestedsugar.
Insomepeople,acid-producingcellsofthemucousmembraneofthestomachdisappearduetoatrophyofthemucousmembrane(aconditioncalledatrophicgastritis),andthereforemicrobesareabletomultiplyinthestomach.Peoplewhohavetakenantacidmedicinesforalongtimetotreatacidicstomachconditionsarealsoatriskofmicrobialgrowthinthestomach.Unliketheliver,themucousmembraneoftheintestinesandthemicrobesofthestomacharenotabletoprocessacetaldehydeandturnitintoaceticacidandwater.Therefore,anabundanceofacetaldehydeaccumulatesinthesaliva,anacidicstomach,andthelowerdigestivetract.
Maximum amount of acetaldehyde
Ithasbeenestimatedthatthedailyacetaldehydeintakebyapersonofaverageweight(70kg)shouldnotexceed0.4milligrams.Thismeansthattheacetaldehydecontentofa100mlservingofanyalcoholicbeverageorfoodshouldnotexceed50μmol/l.Theacetaldehydecontentofsomefoodcanbemanythousandsoftimeshigherthanthesafetylimit.
Thehighestacetaldehydecontentscanbefoundincertainalcoholicbeverages(suchascalvados,sherryandciders),fermentedproductsandsomefruitsandfruit-basedproducts.
