The Peter Sheehan Diabetes Care Foundation (PSDCF) is committed to empowering communities around the world with the necessary tools to fight the diabetes epidemic . Inspired by the work of the late Dr . Peter Sheehan, PSDCF seeks to improve prevention and management of diabetes for at-risk populations, educate communities to prevent onset of diabetes and the complications, and organize forums for scientific ideas exchange to further the understanding of the disease .
For too long, diabetes care has been approached with generic solutions that do not meet the needs of a growing majority of patients . In order to reduce the significant health disparities that exist among racial and ethnic communities, PSDCF promotes comprehensive patient care, personalized diabetes education, and precision medicine research that considers the genetic, cultural, and health needs of people with diabetes .

Our StoryThe Peter Sheehan Diabetes Care Foundation (PSDCF) is a 501(c)(3) non-profit global organization that was founded in the memory of the late Dr . Peter Sheehan to continue his work and legacy as a visionary leader in the field of diabetes care . Since the start of his career as a physician, Dr . Sheehan dedicated himself to helping people with diabetes manage and prevent the debilitating complications associated with the disease . A former member of the board of directors of the American Diabetes Association (ADA) and President of the NYC Leadership Council, Dr . Sheehan focused his practice on preventing the lower
extremity complications of diabetes in patients with typically multiple co-morbidities . After his passing, those around him rallied his former friends, partners, and colleagues to join in supporting PSDCF and continue his efforts in this field . Today, his passion for diabetes prevention and his dream of educating and establishing comprehensive diabetes management to those in need inspire many through the activities of PSDCF .

The purpose of the PSDCF Private Membership Program is to build strong alliances between the Foundation and donors interested in supporting our work . We believe that, together, we can work to educate patients and physicians about diabetes prevention and management, reduce racial and ethnic health disparities in the US and around the world, and improve diabetes care for all .
Founded in 2015, PSDCF is a 501(c)(3) non-profit organization that seeks to reduce the global incidence of diabetes and improve the lives of the nearly 1 billion people affected with diabetes and prediabetes . If no proper action is taken, it is projected that, by the year 2040, an additional 600 million people will be struggling with the disease . PSDCF is passionate about addressing the global diabetes epidemic and improving quality of life for communities around the world .

Global Diabetes Project Collaborative global health project, in partnership with the SUNY Global Health Institute, promoting international diabetes care delivery and research to address the ethnic and racial health disparities associated with prediabetes, diabetes, and its complications .
Sheehan Diabetes Care Model
Inspired by Dr . Peter Sheehan’s vision of a community-based approach to diabetes care, the Sheehan Diabetes Care Model is a patient-centered, high-touch, comprehensive "hub-and-spoke" model for the prevention and management of diabetes in at-risk communities that promises to improve the standard of diabetes care .
GOALS
1. Create sustainable and effective linkages between clinical centers and at-risk communities for the extension of the care continuum into the community setting and the reduction of the use of emergency services .
2. Improve patient engagement, compliance, safety, and health outcomes with community access to preventive services, ongoing health and medication monitoring, and individual outreach through digital health points-of-care in the community .
3. Empower and educate patients with clinical lifestyle intervention programs and health and wellness support delivered through digital health and community-based strategies .
4. Collect health and patient-centered data for research to support physician decision-making, further personalize care, and reduce health disparities .
CARE MODEL STRATEGIES
Prevent Complications with the Community-Based Risk Assessments
The Sheehan Diabetes Care Model improves community-wide patient access to risk assessments for the prevention of complications associated with diabetes . Digital health strategies enable an efficient “hub-and-spoke” approach to community healthcare that eases patient access to care . Improved screenings

through community points-of-care can serve as a community linkage to basic primary care services and identify individuals who poorly manage the disease and at-risk of developing serious complications .
Facilitating risk assessments allows communities to better manage diabetes, prevent hospitalizations, and increase primary care enrollment . Patients can more readily engage in the healthcare system, and receive expert specialty care consultation at each of the community points-of-care . The examinations are all non-invasive and can be coordinated by a healthcare provider, health promoter, or technician .
Monitor Patient Compliance to Plan of Care
The Sheehan Diabetes Care Model creates an interdisciplinary care team to assess patient compliance to plan-of-care, potential adverse events, and strategies to improve health outcomes . Improper prescription use is rarely reported to the primary care physician and is a common cause of treatment failure . Furthermore, in many ethnic communities, traditional medicines are taken in addition to the prescribed plan without the knowledge of the care team .
Behavior and compliance information will be input from the patient’s home and personal devices for a better understanding of the patient’s health needs . Individual outreach for intervention is facilitated through the digital and community-based services .
Support Patient Lifestyle Intervention, Education & Community-Building
The Sheehan Diabetes Care Model implements personalized support to help patients meet their health and wellness goals . A clinical intervention protocol is developed and adjusted to the patient's care plan, culture, and level of readiness for change . Digital health strategies are then utilized to empower patients in their mission to improve their lifestyle through the promotion of physical activity changes in eating habits, and community-building .
Support Physician Decision-Making & Academic Research
The extensive data collected in the Sheehan Diabetes Care Model provides a comprehensive ongoing picture of patient health and behavior that will ease the path for academia and industry to lead research for a more personalized, predictive, and preventive approach to diabetes care .
Data science can support physician decision-making, recognize important health and behavioral trends in communities and larger population groups, and make significant progress in reducing ethnic and racial health disparities .
Diabetes Innovation Accelerator
We are currently discussing seed funding with Empire State Development Corporation for the development of the Diabetes Innovation Accelerator and the purpose of job creation in New York State in digital health and healthcare delivery .

Community Health InitiativeThe Community Health Initiative (CHI) aims to promote education and prevention within at-risk communities to help families, from teenagers to elders, get engaged in their health, reduce their diabetes risk, and protect the health of future generations . CHI plans to address the educational, lifestyle, and cultural factors that plague communities with obesity and diabetes . Programs focused on engaging families about nutrition, physical activity, and preventive services can go a long way in reducing the impact of diabetes on all ages, and improving the health of the community as a whole .

ResearchTraditional Chinese Medicine Use in Chinatown, New York City
Working with the Empire State Patient Safety Network and Mount Sinai Beth Israel’s Asian Service Program, PSDCF is addressing undocumented use of traditional medicines in NYC’s Chinatown . Chinese patients struggle with earlier onset of diabetes, improper management of the disease, and early development of complications . We have collected data on over 100 patients and are proceeding with analysis of the data to determine any insight to improve health outcomes for the community .
Canebosem Project
Plant-derived cannabinoids, which are not toxic and devoid of psychoactive effects, may represent an effective new strategy in diabetes care . For patients with diabetes, managing their health is a struggle that can result in both acute hospitalizations due to hypoglycemia and also the development of symptomless long-term complications due to prolonged poor care .
The Canebosem Project is dedicated to researching cannabinoids and their role in improving the daily management of diabetes and prevention of its complications . We engage in translational cannabinoid research with in-vivo patients as an initial step in innovation towards further understanding the therapeutic value of cannabinoids in both managing diabetes, and preventing the onset of long-term complications like cancer, glaucoma, and lower extremity amputation .
The Canebosem Project seeks to understand the science of cannabinoids and its role in resolving the racial and ethnic health disparities seen in patients with diabetes . Many patients struggle with diabetes because care is generic, costly, and largely ineffective . PSDCF believes that cannabinoids can play an important role as a cost-effective tool in personalized care plans for patients struggling with diabetes .
Conferences & Symposia Wound Healing: Science & Industry
The annual Wound Healing: Science & Industry (WoundHSI) conference updates practicing, research, and industry scientists on the timeliest advances in wound healing . Procedural and pharmaceutical novel treatments are increasingly being utilized for acute and chronic disease in inpatient and outpatient practices, whereas patients require long-term wound care . WoundHSI is unique in that, unlike other programs that simply provide updates, the program goes beyond to offer information about emerging science and technology in the area of wound healing .
Cuba Diabetes Congress
In furthering its goals to foster the exchange of scientific advances, PSDCF co-sponsored the 2016 International Congress: Controlling Diabetes and its most Severe Complications . The four-day congress program took place in Cuba and presentation sessions focused on ways to improve prevention, costs, and patient outcomes . Expert scientists and health practitioners from around the world, including the United States, presented to an inherently interested audience comprised of international attendees representing many disciplines in diabetes .

International Scientific Exchanges
US-CUBA SCIENTIFIC EXCHANGE
PSDCF organizes Scientific Exchange Programs between American and Cuban scientists in the field of diabetes both to foster collaboration between the two countries and further scientific and technological advances for better treatment of diabetes and its complications . The first exchange took place in May 2016, when our U .S . delegation with high expertise in diabetes travelled to Cuba for intensive discussions and visits to clinics and hospitals in Havana and rural areas . PSDCF aims to establish collaborative programs that are of mutual interest to both countries .
